Additionally, Contractor(s) is required to pay employees weekly and submit weeklycertified payroll reports to SMHA (see below). Therefore, by submitting a bid, each Bidder isthereby agreeing to and verifying that he/she will not pay his/her employees at rates less thandetailed within the following table below or with less frequency than detailed herein. Attachment A:Scope of Work/SpecificationsPLEASE NOTE: SMHA is providing the brand name included herein as a sample only so that Bidders have a betterunderstanding as to the minimum product standards that SMHA desires. As required by HUD regulation, Bidders maypropose an “equal” or “same as” product, as long as such product is substantially equivalent to the products identifiedabove. Also, all specifications are subject to normal manufacturing tolerances.

Stark Metropolitan Housing AuthorityScope of Work/Specifications1.0 General SpecificationsThe below listed specifications are intended as general guidelines to be followed by the contractorin preparing a proposal and during the entire project. The brand name materials listed are intendedas a standard to be met and contractors may submit alternates that are equal to or superior to theproducts listed.1.1 In all circumstances the work to be performed shall be of first class quality byemployees skilled in the respective trade and knowledgeable about the product andequipment being used. These general specifications are intended to list the major tasksto be completed and the contractor is to be responsible for completing all other tasks inorder to achieve a first class project even if each specific task is not listed herein.1.2 All parking lot surfaces shall be power cleaned removing all loose gravel, asphalt, oilspots, grease spots, and debris.1.3 All parking lot edges shall be dressed out and all weeds and grass removed from theedges and disposed of.1.4 All surface exposed cracks shall be hot pour filled prior to seal coating. This shallinclude all cracks 1/4” or more. Cracks shall be filled with any of the approvedfollowing products or equals to: CrackMaster Supreme Hot Pour Crack Sealant SMT190 by Sealmaster, Deery 102P Hot Applied Sealant by Deery American Corporationor Crafco Road Saver 534 Crack and Joint Sealant by Crafco Incorporated.1.5 Care shall be taken so as to not cause damage to SMHA property and any adjoiningproperty with equipment or materials.1.6 Where the asphalt sealing abuts concrete curbing or concrete flat work, special careshall be taken to prevent the asphalt sealer from spraying or being applied to theconcrete. Provide a clean, neat application. Failure to do so may result in pricedeductions.1.7 The parking lot shall receive one (1) coat of coal tar sealer with any of the approvedfollowing products or equals to: SealMaster Coal Tar Concentrate Pavement SealerSMT-100 by Sealmaster, Jennite Coal Tar Pavement Sealer by Neyra Industries, orSeal-Pave by Surface Coatings Company.

1.8 Application should not be made unless pavement and ambient temperatures are above50 degrees F and good drying conditions are present or can be provided for the next 24hours.1.9 Schedule of work shall be such that no vehicular traffic is normally on parking areasfor 24 hours.1.10 All areas with surface markings shall be striped overlaying the same layout usingyellow marking paint. The paint material used shall be traffic zone latex paint sprayedon for asphalt surfaces. All handicap accessible spots must be marked in blue and whiteand meet all marking requirements of the Americans with Disabilities Act.General Information/Requirements1. The parking lot and drive area square footages are approximates and all contractors shallmake their own field measurements prior to bidding. Your bid shall be based on theactual field measurements and not on the approximate amounts of the square footagesinvolved. No extras will be allowed because of the difference between SMHA supplied squarefootages and the actual field measurements of the contractors. Each contractor shall supply theactual field measurements that they have recorded for the parking lot proposal form whereindicated.2. The oil and grease spots must be cleaned and scraped to remove as much of thismaterial as possible. These spots should then be treated with an oil spot primer andallowed to dry. Acrylic emulsion based primer that bonds tightlywith oil-spotted asphalt surfaces and stops them from coming through seal coatings.3. Where applicable a brand name or equal specification as used in this solicitation, the brandname specification is for the purpose of describing the standard of quality, performance andcharacteristics desired and not intended to limit or restrict competition. Should a contractorobject to the specified product the burden of proof to demonstrate an equal to the specifiedproduct falls upon the challenging contractor to show his product to be equal to or better than theone specified.4. Law, Rules and RegulationsThe work shall comply with all federal, state and local codes, rules and regulations including allordinances and other statutory provisions pertaining to this class of work. Handicap accessiblespots must meet all requirements of the Americans with Disabilities ACT with regard to size,number and markings. Such Rules, Codes, Regulations and Ordinances shall be considered a partof these specifications. Any conflict between the RFP requirements and the aforementioned rules

shall be noted by the contractor at the time the proposal response is submitted.
