MDTDavid Graduated in 1993 with a BS in Physical Therapy at OUHSC and an advancedMS with a Fellowship in Sports and Orthopedic Physical Therapy also at OU in 1998.Since that time he has been in practice as a manager in an outpatient setting and hasbeen a clinical instructor to more than 50 students. He was named clinical instructorof the year in 2013. He completed his athletic training certification in 1999, McKenziediagnostic therapy certification in 2001, Sports clinical specialist in 2003 andorthopedic clinical specialist in 2010.David has been an adjunct faculty at OU since 1999, and developed an OCS and mentoring program for newgraduates while in private practice. He served as a volunteer athletic trainer and PT at the Olympic trainingcenter in San Diego and Colorado Springs. He has and continues to provide medical coverage for highschool, college, and professional athletes. Until last year he organized the on course medical coverage for theOKC Marathon. David served on the Oklahoma physical therapy advisory board for 6 years, the last 2 aschair.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?David has been active in the OPTA since 1991 and looks forward to serving in the capacity as first VicePresident. Since this position is responsible primarily for educational opportunities through the OPTA, Hefeels that his background in education, leadership, and volunteerism make him an ideal candidate.SECRETARY: Cindi Cathey, PTA, BSCindi graduated from Oklahoma State University in 2002 with a Bachelor's degree inHealth. After working in a medical fitness facility, she became a rehab tech andpursued her PTA degree at Tulsa Community College in 2008. She worked at KaiserRehab as a PTA and Continuing Education Coordinator before switching to homehealth. Cindi has worked with the Geriatric population in home health for 9 years, 7 ofthose with Carter Healthcare. She is an active member of the OPTA/APTA havingserved as Eastern District Secretary, Chairperson for 2 terms, and Director. Cindi alsoserved as PTA Caucus Rep Elect/Rep from 2014-2017 and was slated for NationalOffice in 2016. She is currently serving as the OPTA Nominations Committee Chair,the APTA Academy of Geriatrics State Co-Advocate, and as a liaison for the mentaland cognitive health SIG. Cindi is a member of the Home Health and Geriatric sections of the APTA.In addition to her work with the OPTA, Cindi has actively served on the Tulsa Alzheimer's Walk committee forthe last 3 years, and on the TCC PTA Advisory Committee. Cindi was awarded OPTA Outstanding PTA of theYear in 2017, Clinical Instructor of the year 2017, and had the great honor of being the guest speaker for theTCC PTA 2017 graduation. Cindi lives in Broken Arrow with her husband, Brian, and 5 year old son, Bryson.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?One of the attributes I bring to the table is my experience of various leadership roles throughout the OPTA. Iam a very organized, structured individual who is very passionate about our profession. I am eager tocontinue to serve our association. CHIEF DELEGATE: Jeff Jankowski, PT, DPT, ATCJeff is currently serving in his second full term as a Delegate to the APTA House ofDelegates. Prior to serving as a Delegate, Jeff was the OPTA Chapter Presidentfrom 2008-2012. He also served as an Alternate Delegate during this timeframe sohe has been directly involved with House of Delegate issues for the past 10 years.He is also the current OPTA Secretary serves Federal Government Affairs Liaison tothe APTA, a position that he has held since 2008. He also serves on the OPTA StateGovernment Affairs Committee and has served with this committee since the late90’s.Jeff graduated from the University of Oklahoma Health Sciences Center in 1996 and has been an activemember of the OPTA throughout his career. He recently completed his tDPT through EIM Institute of HealthProfessions. He is employed by Redbud Physical Therapy in Tulsa where he serves as the CEO. Redbudcurrently has 15 locations throughout Northeast Oklahoma. He also serves on the Southern NazareneUniversity Board of Trustees as well as leadership in his local church and on the Northeast District of theChurch of the Nazarene. He has been married to his wife Julie for 24 years and has two children, Jordan andJustin.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?One of my main attributes for this position is having served as either a Delegate or Alternate Delegate inAPTA’s House of Delegates for the past 10 years. During this timeframe we have debated many importantissues that affect our profession on a daily basis and these policies will continue to impact us for years tocome. Having a willingness to engage in not only these floor debates, but also in the year-long online forumsto stay abreast of the latest developments, will help me to communicate with our OPTA members and fellowdelegates on the issues before us.DELEGATE: Lynn Jeffries, PT, DPT, PHDLynn Jeffries is an associate professor and director of clinical education at OUHSC.She holds a bachelor’s degree is in physical therapy, Master of Science degree is inpediatric physical therapy, and a doctoral degree in allied health sciences with a focuson interdisciplinary programs and pediatrics. She has practiced in a variety ofsettings with her focus being pediatrics. She is Co-PI for two multi-site grant-fundedresearch projects. She teaches in the OUHSC DPT and MOT programs, as well asthe DSc post professional program. She has served the OPTA in many rolesincluding, President, Chief Delegate, Secretary, delegate, and bylaw chairperson.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?Thank you for the opportunity to run for delegate. As a past delegate and chief delegate I feel I bring manyyears of experience to the working of the House of Delegates. At this current time, many policies andprocedures are being reviewed and updated for which having a background would be helpful. I bring over 30years of practice into this position with leadership at both state and national levels. I believe I have the skillsto help information the forward progress of the association as we move towards our centennial anniversarywhile continuing to represent Oklahoma and all of the members of our profession.

NOMINATIONS COMMITTEE: Kari Collier, PT, DPTKari graduated from the University of Tulsa with a BS in Exercise and Sports Sciencein 2002. She went on to further her education at OU where she obtained a Master'sin PT in 2005, and graduated from Boston University with a Doctorate in PT. Kari is alicensed PT in Oklahoma and Arkansas, and is an accomplished healthcareprofessional with several years of experience in management and operations ofmajor patient care services. She is the Founder/Owner/CEO/President of Access2Hralthcare Solutions, Inc. She is responsible for all aspects of the home health andpediatric therapy business to include: financial performance, contracts, clientrelationship management, continuing education and consulting services. Currentlythey support pediatric clinics, hospitals, nursing homes, schools, home healthcompanies, and Sooner Start kids. Kari is an OPTA member, served on Ethics Committee, and is aTherapeutic Horseback Riding Instructor for Children with DisabilitiesCandidate question: What attributes and experiences do you bring to the table to fulfill this position?I have been an active OPTA and APTA member. I own my own Therapy company so I am skilled in findingquality therapy and manager candidates. I have experience in the interview and selection process. I stay upto date on current Therapy trends and rule changes. I am a hard worker and am always open to learning newthings. I feel I can offer an unbiased opinion in assisting with the selection process for new positions withinOPTA.NOMINATIONS COMMITTEE: Jason Manning, MSPT, FAOMPT, ATCJason graduated high school from Putnam City North in 1997. He attended OklahomaState University, graduated with a Bachelor of Science in Athletic Training, in 2002.Jason went on to Texas Woman’s University, a premier physical therapy school, andgraduated with a Master of Science in Physical Therapy in 2004. He began hisphysical therapy career at Oklahoma Physical Therapy Spine Care-rehab in 2004. In2009, he completed a fellowship in orthopedic manual therapy, which is one of thehighest credentials that a physical therapist can obtain. He became manager ofOklahoma Physical Therapy Spine Care-Rehab in south Oklahoma City from 20062008. In 2008, he was given the opportunity to start-up a physical therapy clinic inYukon, OK. He enjoyed this opportunity until he was offered the opportunity tobecome part-owner of Oklahoma Physical Therapy Spine Care-Rehab. Jason is the managingpartner/director at the north location. Jason has a strong background and knowledge of spinal dysfunctions,but also knows sports injuries from his time as a student athletic trainer at Oklahoma State University. He ismarried to Dana Manning, wife of 15 years, and 2 children, Hudson, 6, and Ellison, 3.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?The main attribution I bring to the table to fulfil this position is my desire and eagerness to serve, not just God,but my colleagues, profession, and community. I am ready to be a contributor to OPTA. I have been aphysical therapist for 14 years. I have grown as a clinician and added to my responsibilities. My ability tomanage and delegate these responsibilities has and will allow me to do the same for the NominationsCommittee.

ETHICS COMMITTEE: Ian Blake, PT, DPT, MTC, OCSIan D. Blake graduated from the University of Oklahoma with a Master ofPhysical Therapy in 2004. In 2007, he became a board certified orthopedicspecialist. He completed his Manual Therapy Certification in 2017 and hispostgraduate Doctor of Physical Therapy in 2018, both through Evidence InMotion. Prior to becoming a Physical Therapist, Ian practiced as a licensedPhysical Therapist Assistant in orthopedics for 5 years. In 2010, he became coowner of Physical Therapy of Tulsa, an independently owned orthopedic clinicwith a mission of providing expert orthopedic physical therapy to the Tulsacommunity. Starting with only 3 therapists, PT of Tulsa has grown to include 11 clinicians including 3orthopedic specialists, a certified pelvic floor specialist, and a vestibular specialist, as well as providing inpatient physical therapy for Tulsa Spine and Specialty Hospital. In addition, he treats patients at SouthernHills Country Club. He is trained in a variety of techniques including soft tissue mobilization, specializedexercise prescription for treatment of movement impairments, trigger point dry needling, PosturalRestoration , and the Graston Technique . Ian has also completed advanced clinical training in the use ofhigh speed video to diagnose movement dysfunction and to determine readiness for return to sport throughthe Movement Performance Institute led by Dr. Christopher Powers, PhD, PT, FACSM, FAPTA.He has presented at the Oklahoma Academy of Physician Assistants annual conference; taught continuingeducation courses to physical therapists, physical therapist assistants, and personal trainers; led communityhealth education events; and teamed with local running stores for free injury clinics. He has been a clinicalinstructor for the University of Oklahoma, Tulsa Community College, and Texas Women’s University. He is anactive member of the Oklahoma Physical Therapy Association and the American Physical TherapyAssociation.Candidate question: What attributes and experiences do you bring to the table to fulfill this position? Ibelieve the greatest attributes I bring are nearly 19 years of clinical experience and continued passion for thepractice of physical therapy. That experience and passion have taught me the enormous responsibility webear as physical therapy professionals. We are on the front-line of today’s health care environment and assuch must be held to a standard beyond that of those who have come before us. As we strive to become thePrimary Care Practitioners in all areas of musculoskeletal pathology, we must ensure that we uphold thestrong ethical spirit that has allowed us to come this far.ETHICS COMMITTEE: Kelli Trobaugh, PTAI have been a PTA for 10 years. I have worked for two different home healthagencies, the most recent being Encompass Home Health for the past 8 years. Ireceived PTA of the year in 2013 from the Oklahoma Association for Home Careand Hospice. In 2014 I received my Advanced Proficiency in Geriatrics through theAPTA. I also became LSVT Big certified in 2014. I have served as the president ofthe PTA SIG from 2015 to 2017. At Encompass Home Health I am a certifiedpreceptor and have trained employees in Oklahoma, Kansas, Florida, Kentuckyand Arizona. This year I was awarded Preceptor of the year for our Tulsa office forthe second year in a row.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?I have a good understanding of ethical values with implementing policies at work with new hires. I haveperformed numerous chart audits and assist new hires with adhering to ethics with proper documentation andpatient care.

ETHICS COMMITTEE: Eddy Traylor, PT, M.Ed, DPT, OCS, CEEAA, SSRCEddie earned a Bachelors of Science (BS) degree in Health, Physical Education, andRecreation, and a second BS in Physical Therapy from Langston University, Masters ofEducation (MEd) in Professional Health Occupations Education from the University ofCentral Oklahoma, and a Doctorate of Physical Therapy (DPT) from Alabama StateUniversity. He is Board-Certified Orthopedic Clinical Specialist, an Exercise Expert forAging Adults through the Section on Geriatrics of the American Physical TherapyAssociation, and Sports Specific Rehabilitation Therapist by Donatelli Sports-SpecificRehabilitation Certification Program. He has worked in a variety of clinical areas includingacute care, inpatient rehabilitation, and most recently in a rural outpatient orthopedic privatepractice. He has been involved in physical therapy education as either adjunct faculty, parttime faculty or full time faculty since 1994. He currently is an Assistant Professor in theSchool of Physical Therapy at Langston University, where he teaches Biomechanics andHuman Motion Analysis and the Physical Therapy Procedures courses .Candidate question: What attributes and experiences do you bring to the table to fulfill this position?I have over 25 years experience as a Physical Therapist, in various clinical settings. During that time I havewitnessed many changes to the profession as it continues to grow as a medical discipline. In order for us tocontinue that growth and maintain the respect of other professions, Physical Therapists must continue to alwaysdo the right thing. As an educator, it is my responsibility to be a role model in demonstrating high levels of ethicalbehavior in clinical practice, and to teach students about the importance of moral and ethical values as they jointhe profession of Physical Therapy.PT ADVISORY COMMITTEE PT POSITION: Kelly Berry, PT, MPH, Cert. MDTKelly is a Summa Cum Laude, 1992, graduate of the University Of Oklahoma College OfAllied Health, Division of Physical Therapy. She earned her Masters of Public HealthDegree in Health Administration and Policy from the University of Oklahoma in 1999. Shereceived her Credentialing in Mechanical Diagnosis and Therapy from McKenzie Institute in1997, became a Certified Ergonomic Assessment Specialist from Back School of Atlanta in2000, and a STAR certified clinician through Oncology Rehab Partners in 2013. She hasbeen a member of the APTA since she was a student and has served in various capacitiesat both the district and state level within the Oklahoma Physical Therapy Association.She has worked at St. John Medical Center since 1992, initially as a staff therapist, then asboth industrial rehabilitation and education coordinators. She is currently manager ofoutpatient rehabilitation for both St. John Medical Center and St. John Owasso, overseeing approximately 40multi-discipline clinical and clerical staff, and maintaining a small caseload of spine patients. She developed akeen interest in the rehabilitation of the oncology patient, since losing a therapy colleague to cancer in 2004. Shepursued continuing education in the field, learned of the Survivorship Training and Rehab (STAR) program fromOncology Rehab Partners, and convinced administrators that more collaborative oncology rehab services wascrucial for the future state of patient outcomes. She successfully launched the 1 st STAR program in Oklahoma in2013, and continues to be passionate about facilitating oncology rehab program development.Candidate question: What attributes and experiences do you bring to the table to fulfill this position?I have 25 years of experience as a clinician and clinical educator for PT and PTA. I have been a manager for 13years, of a large, hospital based outpatient rehab, with up to 40 direct reports of varying disciplines, responsible forassuring staff competency and appropriate clinical practice, ethical and legal business practices, and forecastingthe needs of our business and profession. I have served several offices within the OPTA, including state 1 st VP,and have been involved, at the local level, in forwarding the OPTA’s legislative agenda, as it relates to the practiceof physical therapy. I have participated in focus groups, over the years related to altering the language of ourpractice act, including continuing education requirements, and have served on the Eastern District subcommitteefor continuing education approval for over 10 years. I believe that it is our duty, as clinicians, to promote andmaintain the integrity of our profession and to hold each other accountable for appropriate conduct and practice ofPhysical Therapy.

PT ADVISORY COMMITTEE PT POSITION: Joe Streich, PT, MSPTTherapy Specialists of Oklahoma’s founder and co-owner Joe A. Streich values thePhysical Therapy profession greatly and brings 18 years of experience in privatepractice and business ownership. He is a long time active member of the OklahomaPhysical Therapy Association as well as a member of the Private Practice Section ofthe America Physical Therapy Association. Joe has served in many roles on theOPTA Board of Directors in the past as well.Candidate question: What attributes and experiences do you bring to the tableto fulfill this position?Being a business owner, in addition to the many OPTA leadership roles I have held has prepared me to be agreat candidate for the PT Advisory Board. I am a huge advocate for our profession, and I am excited to havethe opportunity to serve on the PT Advisory Board to be a voice for the PTs and PTAs in our state.PT ADVISORY COMMITTEE PT POSITION: Samer Zantout, PT, MPT, OCS,FAAOMPTSamer “Sam” Zantout is a practicing physical therapist and co-owner of TherapySpecialists of Oklahoma. He earned his Masters of Physical Therapy in 2005 fromthe University of Oklahoma Health Sciences Center in 2005. Sam is a lifelonglearner, and enjoys furthering himself in the profession of physical therapy. Whileworking as a PT, he entered into an intensive three-year post-graduate fellowshipprogram for orthopedic manual physical therapy. After completion of his fellowshipprogram, Sam was awarded with the distinction of Fellow of the American Academyof Orthopedic Manual Physical Therapy (FAAOMPT) in 2010.
