The data set, which reflects calendar year 2013, contains information on payment andutilization for more than 15,000 SNFs with about 2.5 million stays, resulting in about 27 billion in Medicarepayments. The release reveals Medicare Part A claims for the fee-for-service population. It doesn’t include patientsenrolled in Medicare Advantage plans.Nursing homes are paid per diem by Medicare through the SNF prospective payment system for the first 100 days of apatient’s benefit period. The claims are adjusted for the complexity of care provided and for regional wage differences.Below, we looked at how New York City nursing homes stacked up.Please note: Pulse Extra is optimized for the Web, and some features may be limited in printouts of the page. As a proxy for quality,we used the five-star star rating system CMS offers as a tool to help consumers. That metric takes into accounta facility’s health inspections, staffing and 11 quality measures. Critics have called the grading unfair becauseit is part of a normal distribution, restricting the number of five-star providers to the top 10% and requiring thebottom 20% of SNFs to receive one star.To compare cost and quality, we looked at how the 155 city SNFs were distributed across CMS’ five-starsystem. Then we found the average payment per stay. This measure does not include beneficiaries’ out-ofpocket contributions. From there, we took the median number at each star level. The city SNF that received the most from Medicare was the Boro Park Center forNursing and Rehab, a Brooklyn provider run by Centers Health Care. ArchCare's Mary Manning Walsh Home inManhattan received the sixth-largest amount from Medicare, but received a one-star grade. The nursing homeswith the highest payment per stay were those providing specialized services. One example is the now-closedRivington House, which provided long-term care to people with HIV/AIDS and received 40,169 per stay in2013. As more information becomes available about cost and quality, hospitals and insurers are seeking topartner with SNFs that offer the best outcomes at the lowest cost, said Dr. David Friend, chief transformationofficer at BDO. “If I’m the hospital or insurer, why do I want to send business to the guy who’s really expensiveand not going to give me good quality, when now with new data I see this other guy is cheaper and better?”Friend said.
