ANNUALSTATISTICSCommunity Health Centers are all nonprofit health care providers that providecomprehensive primary health care. Theycomprise the largest national network ofprimary care providers, serving more than27 million people in more than 11,000service delivery sites. In Pennsylvania, thereare more than 300 FQHC sites in rural andurban areas of Pennsylvania, with locationsin 52 of the commonwealth’s 67 counties.There are four primary characteristics thatdistinguish community health centers fromother primary care providers:Fees based on ability to pay Quality health care for all (FQHCs areopen to all and they must meet federalreporting, performance and accountabilityrequirements) Highly competent health professionalteam (FQHCs offer other services tohelp patients stay well, such as casemanagement, pharmacy, lab testing,social services and translation) Community control to ensure theyare responsive to their patient population(at least 51% of people serving on thegoverning board of an FQHC must bepatients served by the FQHC)4EAST LIBERTY FAMILY HEALTH CARE CENTERThe East Liberty Family HealthCare Center is a FederallyQualified Health Center (FQHC)and receives support throughthe Health Resources andServices Administration ofthe Department of Health andHuman Services (HRSA).Our focus is people who are uninsured,underinsured and underserved. Women often prefer their OB-GYNphysician to be a woman, because they share a betterunderstanding of their health and feelings.This is not the first timeDr. Yandel worked at ELFHCC.Her first introduction to our healthcenter was when she was anAmeriCorps student with theBirth Circle, prior to her firstyear of medical school.Providing care with compassion and respect, aswell as treating each patient with top-notch,quality care is at the center of Dr. Yandel’spractice. From a young age, she thrived in heracademics, but also enjoyed playing basketball, running track, andbeing active in the music department. She traveled to Baltimoreto earn her undergraduate degree at Loyola University Maryland;received her medical training at Lake Erie College ofOsteopathic Medicine in Erie, Pennsylvania; and completed herresidency at Reading Health System in Reading, Pennsylvania.TotalEndowment 3.36 millionIn addition, Dr. Yandel also bringsvaluable skills to ELFHCC, includingproviding care to high-risk, pregnantpatients, and performing state-ofthe-art robotic surgeries.Dr. Yandel splits her time between ourHill District and Lincoln-Lemington officesand is accepting new patients.Financial numbers for 2016–2017 are audited and 2017–2018 are unaudited.The figures mentioned above are from the Uniform Data System (UDS). UDS is a reporting requirement for Health Resources and ServiceAdministration (HRSA) grantees, including community health centers. The data are used to improve health center performance andoperation and to identify trends over time. A MESSAGE FROM OURMEDICAL DIRECTORWhen I think back on ELFHCC’s 36 years ofproviding quality health care to our patients,I am often reminded of Ephesians 3:20 (ASV):Now unto him that is able to do exceedingabundantly above all that we ask or think,according to the power that worketh in us.Our Home Bound Outreachprogram provides quality servicesand continues to help some ofour most fragile patients. This iswhere we see the heart of Godreaching the most vulnerable inour neighborhoods.Our Health Center has grown from a small group of providers in thebasement of Eastminster Presbyterian Church to a four-site HealthCenter, employing approximately 90 people in varied and vital positions.I feel very thankful to witness this growth and see the many ways in whichwe have been privileged to provide quality health care to all our patients. Wehave dedicated staff with various gifts to serve our patients through our Mission.I am especially excited to welcome Amaris Yandel, DO, an obstetrician/gynecologistwho recently joined the ELFHCC staff. Dr. Yandel specializes in providing care tohigh-risk, pregnant patients and performing state-of-the-art, robotic surgeries. Thesegifts are a great asset of quality provided right in our neighborhood health centers.We have two full-time Licensed Clinical Social Workers and a Licensed ProfessionalCounselor providing behavioral health, as well as drug and alcohol counseling. ELFHCCis starting medically assisted treatment for opioid abuse that will greatly benefit ourpatients struggling with addiction issues. In addition, we have a part-time psychiatristseeing patients at our East Liberty and Lincoln-Lemington sites. This continues toexpand our Mission of whole-person care and integrate medical and behavioralhealth services.Our dental centers are staffed by two highly qualified Dentists and two Public HealthDental Hygiene Practitioners who are able to see patients independently for certaindental services.The Health Center is always looking to recruit family practice providers, especiallySpanish speaking providers for our growing number of Spanish speaking patients.Recently, a dietitian was hired toprovide our patients with informationand dietary support needs as it pertains toeach patient’s health.Dr. David Hall,Medical DirectorI am joyfully blessed to be part of thisongoing mission to serve God and continueto spread and expand medical, dental andbehavioral health services for our growingpatient population.Sincerely,Dr. David HallMedical DirectorNEW PEDIATRIC SUITEOPENS IN 2018Saturday, October 6, ELFHCC held aRibbon Cutting Ceremony to celebratethe completion and opening of its newPediatric Suite. The Pediatric Suite wasmade possible through generous grantsfrom Eden Hall Foundation, NimickForbesway Foundation and ELFHCC’sCapital Fund.A designated Pediatric Suite allows usto co-locate the pediatric team whoare currently seeing patients at separateoffices. By co-locating the pediatricteam to their own Pediatric Suite thiswill create a win-win-win situation. One,the pediatric team will be a united andorganized department. Two, this willopen up much needed space for our adultprimary care providers. Three, we will addtwo more adult medical exam roomsin order to optimize the delivery of carethroughout our Health Care facilities.This 400,000 project includessix Pediatric exam rooms, alab, a charting room a play/waiting room for thechildren and so muchmore. A MESSAGE FROM OURBOARD CHAIRPERSONBOARD OF DIRECTORSAND OFFICERSThe number one goal of everyone at the ELFHCC is to providetop-notch quality healthcare to our patients. Everything the healthcare professionals, Board of Directors, and staff does on a daily basisrevolves within that all-encompassing goal. We continuously seekways to enhance patient care, decrease wait times, and improveupon services provided to our patients. I am pleased to sharewith you so many positive changes that have occurred in thepast two years.review of the mission,value, strategic plan, andgoals of ELFHCC. We areproud to have a strong leadershipteam that carries out the strategic goalsin an inclusive manner—a leadershipteam that is structured in such a waythat it makes it realistic to accomplish thestrategic plan.In 2016, ELFHCC had an on-site visit with four reviewers from Health Resources andServices Administration (HRSA). This special visit included a thorough exploration ofELFHCC’s policies and procedures, financials, patient care, billing, accounts payable,and every other department within ELFHCC. I am proud to share with you that ELFHCCpassed the on-site review with no infractions.ELFHCC would not be where it istoday without the ever-present prayersand support of our many church partners.Most of these partners have beenbenefactors to ELFHCC for 36 years.
