Multiple channels between the same point,or multi-point channels, or extensions on mileage channels requested atthe same time are included under the same Service Ordering Charge.2.1.4Unless otherwise specified, the appropriate Service Ordering Charge isapplicable for a customer request for service and is in addition to any NonRecurring Charge (NRC) which may apply.2.1.5A Change of Record Order Charge will apply for each request for insidewire maintenance work from business customers who have not subscribedto the Inside Wire Maintenance Plan.2.2Line Connection Charges2.2.1Line Connection Charges are applicable but not limited to connecting,restoring or changing the following services:1.Main line services, including individual lines2.PBX trunks, manual or dial3.Channel services4.Key system trunksIn addition to the services listed, line connection charges are applicable toother services as specified in this Product Guide.Note: One line connection charge applies for connecting each line betweenthe appropriate cable terminals serving different premises in the samebuilding.Issued 4/1/165

Lumos of West Virginia Inc. d/b/a SegraOne Lumos PlazaWaynesboro, Va. 22980MD Pricing Guide2.2.2A line connection charge is applicable per line for telephone numberchanges made at the customer’s request.2.2.3Line Connection Charges apply each time service is established at alocation except for the following:2.31.When service is assumed prior to disconnection and no outstandingbalance/charges are due, and,2.When there is only a change in account name/responsibility.Premises Visit Charge2.3.1A Premises Visit Charge applies per account for each visit to a customer’spremises to perform work requested by the customer.2.3.2However, when a Company employee is on the customer’s premises forthe purpose of repair, maintenance or changes in class or grade of servicenecessitating a premises visit, or where the visit is initiated by theCompany, no Premises Visit Charges apply.2.3.3No Premises Visit Charge will apply for visits when the customer requestsinstallation of a Network Interface Device.2.3.4Payment of ChargesIf the total of all applicable service charges associated with a customerrequest exceeds 25.00, the Company may allow the payment to be madein three (3) equal monthly payments; however, the Company shall have thediscretion to allow payment of service charges over a longer time period ifnecessary to avoid undue hardship on a customer.2.4Termination Charge2.4.1A termination charge is determined by applying to the basic terminationcharge the percentage of the unexpired portion of the initial service periodrelative to the full initial service period. The basic termination charge andthe initial service period are indicated in the section of this Product Guidecovering the service items to which they apply.2.4.2When a subscriber cancels an order for service carrying a basic terminationcharge prior to the establishment of that service, a termination chargeapplies equal to the cost incurred by the Company in engineering, orderingand providing the equipment and disposing of it, less credits obtainedIssued 4/1/166

RegularService Charges apply to service reestablished in the old location afterdisconnection of service or after its establishment at another location underthe above conditions. Changes from rotary dial to touch calling service and touch calling service torotary dial. 2.6The establishment and discontinuance of custom calling service.Restoral of ServiceIn the event service is suspended for non-payment, service will be restored uponpayment of charges due or at the discretion of the Company. Service OrderingCharges and Line Connection Charges apply.When at the request of a customer, service is temporarily suspended, a ServiceOrdering Charge and Line Connection Charge will apply for the restoral of thatservice.2.7Network Interface Devices2.7.1Issued 4/1/16All new wiring connected to the Company network for single or two-linecustomers must be connected through a company provided networkinterface device (NID). This device must readily permit the connection anddisconnection of Customer Premises Wiring (CPW) for testing purposes, toand from the Company network using an industry registered jack, of a typeprovided for in FCC Regulations Part 68.7

All wiring on the customer’s premises that is connected to theCompany’s network shall connect to the network through the Companyprovided NID. Any necessary maintenance, repair or upgrade work to theNID shall be the responsibility of only the Company. The Company willmake the decision whether to place the NID inside or outside the customerpremises. In the even the customer requests that the NID be placed in alocation other than the location selected by the Company, any additionalcost to the Company will be charged to the customer. Additionally, thecustomer shall be responsible for premises wiring, which is not provided bythe Company that is connected to the NID.Where a NID exists, if the Company is able to test for Dial Tone and theproblem proves to be beyond the NID (within Customer premises), amaintenance charge is applicable as shown in Section 4, following. In theevent there is no NID and/or the Company is unable to test for a dial tone,then no maintenance charge will be assessed.2.7.3Issued 4/1/16Terminal equipment on the Customer’s Premises and the electric powerconsumed by such equipment shall be provided by and maintained at theexpense of the Customer. The Customer is responsible for the provisionof wiring or cable to connect its terminal equipment to the Company Pointof Connection.8

Lumos of West Virginia Inc. d/b/a SegraOne Lumos PlazaWaynesboro, Va. 22980MD Pricing GuideSection 3 – Description of Services3.1 Directory ListingsThe rates and regulations for directory listings in Sections 3 and 4 apply only in connectionwith primary and additional listings in the alphabetical section of the telephone directoryand/or listings in the information directory.The alphabetical list of names of customers is designed solely for the purpose ofidentifying customers and those entitled to use the customer’s service as an aid to the useof the telephone system and special prominence or arrangement of names is notcontemplated.The Company reserves the right to limit the length of any listing in the directory by the useof abbreviations when, in its judgment, the clearness of the listing or the identification ofthe customer is not impaired thereby.Listings must conform to the Company’s specifications with respect to its directories.One listing without charge and additional listings are regularly provided only in connectionwith the following classes of service: business, private branch exchange service and hotelservice. When two or more lines are arranged in sequence, only the first or primary linewill be listed.3.2 Regular Additional ListingsIn connection with business service, regular additional listings are available only inthe names of those persons having the right to use the customer’s service. Inconnection with hotel service, regular additional listings are available only to thecustomer-agent or his employees except that in connection with service at hotelsand boarding houses, listings of permanent season guests may be arranged bythe management.3.3 Non-Published ServiceUpon request, a customer may have his name omitted from the directory butincluded on information records (non-listed) or omitted from both the directory andinformation records (non-published). Such arrangements are discouragedbecause of their effect on incoming calls, but they will be accepted subject to thefollowing rates and conditions:Issued 4/1/169

Lumos of West Virginia Inc. d/b/a SegraOne Lumos PlazaWaynesboro, Va. 22980MD Pricing GuideNon-published, non-listed service will be furnished at a monthly charge plusthe applicable service charge for establishing the service.The applicable service charge will be made each time a non-listed, nonpublished, telephone number is changed to another number to be eithernon-listed or non-published.No charge will be made to change a non-listed or non-published number toa listed number.Calls to subscribers with non-published numbers will be initiated by theCompany only when the number is given by the calling party. TheCompany will not be liable for failure or refusal to complete any call to suchtelephone when the call is not placed by number.When non-published service is provided, the Company will not disclose thesubscriber’s telephone number to any person except as follows:1.When presented a court order by duly authorized representativesof law enforcement agencies.2.To the Company’s own employees who are required to know thenumber in the conduct of its business.The Company will try to prevent the disclosure of the number of suchtelephone, but will not be liable should such number be disclosedinadvertently.The following customers and/or service will not be subject to the monthlycharge and non-recurring charge.1.Business subscribers who have their primary telephone numberpublished in the Company’s directories and have other telephonenumbers, associated with the same business, deleted from theCompany’s directories.2.Foreign Exchange Service where the subscriber is also furnishedexchange service from the normal central office.3.4 Customized Number ServiceGeneralSubscribers of the Company may request assignment of specific telephonenumbers. If the telephone number or numbers requested by the subscriberare available, the Company may assign the number or numbers to thesubscriber.Issued 4/1/1610

Lumos of West Virginia Inc. d/b/a SegraOne Lumos PlazaWaynesboro, Va. 22980MD Pricing GuideThe Company reserves and retains the right:1.To discontinue, change or reassign telephone numbers in anyexchange area wherever it is necessary or appropriate in theconduct of business, or in accordance with the rules andprocedures of the Company.2.To reject or refuse any request for specific telephone numbers forany reason including, but not limited to, numbers that may, in theCompany’s opinion, be offensive.3.Of ownership of all telephone numbers and prohibits theassignment of the use of a telephone number by or from anysubscriber to another, except as otherwise provided in this ProductGuide.The Company shall not be liable to any subscriber for direct, indirect orconsequential damages caused by a failure of service, change of numberor assignment of a requested number to another subscriber whether prioror after establishment of service. In any case the Company shall not beliable to any subscriber for an amount greater than the subscriber has paidto the Company for the Customized Number Service.Application of ChargesThe Customized Number Service Charge applies whenever a subscriber:1.Requests a telephone number other than the next available numberfrom the assignment control list.2.Requests a number change from the present number to acustomized telephone number.Subscribers who request telephone numbers they formerly used forbusiness service will not incur the Customized Number Service Charge ifthe telephone number requested is available for assignment and is listedin the present local directory under the same name as the new application.3.5 Custom Calling FeaturesCustom Calling Services consist of optional service features for use in connection with acustomer’s local exchange service.3.5.1Issued 4/1/16Regulations11

If during the promotional period the customer requeststhe feature or features be disconnected, neither the applicable monthly rate norany other applicable charges will apply. If the customer retains any of the featuresbeyond the 30-day promotional period, the monthly rates for the feature or featuresretained will apply from the end date of the promotional period.3.6 Feature DescriptionsThe following Custom Calling Services are available at rates shown in Section 4.8,following.3.6.1Call ForwardingCall Forwarding is an arrangement whereby incoming calls may betransferred to another telephone number by dialing a prefix code and thetelephone number of the service to which calls are to be transferred. Callsforwarded by this feature are subject to local (where applicable) and longdistance message charges. This service contemplates that normaltransmission performance quality cannot be guaranteed to all calls.3.6.2Call WaitingCall Waiting is an arrangement whereby a customer who is using anexchange line arranged for call waiting is alerted, by means of a tone signal,when another caller is trying to reach that line. The customer, by flashingthe switchhook, is able to have alternate conversation between parties.3.6.3Extension IntercomExtension Intercom is an arrangement whereby a customer may makeanother telephone on his line ring.3.6.4Enhanced Call ForwardingCall Forwarding – Busy enables the Company to forward a call to apredetermined directory number (DN), which activates when the calledparty’s line is busy. If the customer requests a change in the DN to whichthe call is forwarded, a service order change charge will apply.Issued 4/1/1612

The Company on an office-widebasis establishes the number of ringing cycles before the call istransferred. If the customer requests a change in the DN to which the callis forwarded, a service order change charge will apply.Call Forwarding – Busy and Don’t Answer This service is a discountedbilling arrangement that allows customers to select both Call ForwardingBusy and Call Forwarding-Don’t Answer into one package.3.6.5Speed CallingSpeed Calling is an arrangement that provides for the calling of a local orlong distance telephone number by dialing an abbreviated code. Twoarrangements are available, either an eight-code capacity or a thirty-codecapacity, but not both on the same line.3.6.6Three-Way CallingThree-Way Calling permits an existing call to be held and a second call tobe established and added to the connection for conferencing. This servicecontemplates that normal transmission performance quality cannot beguaranteed to all calls.3.6.7Three-Way Call TransferThree-Way Calling permits an existing call to be held and a second call tobe established and added to the connection for conferencing. This servicecontemplates that normal transmission performance quality cannot beguaranteed to all calls.3.6.8Toll DenialThis service blocks access to the Long Distance MessageTelecommunications Network, either by direct or operator assistance. Thisprevents the customer from dialing an operator for all purposes includingemergencies, assistance and the placing of toll calls. The customerindemnifies and saves harmless the Telephone Company from any and allclaims, losses or damages by restriction of access to operator services.Issued 4/1/1613

When the customer dialsthe cancel code, a distinctive dial tone is returned and a call is then placednormally. During this call,Call Waiting Service is inactive so that anyone calling the Call Waitingcustomer will receive the normal busy treatment, and no Call Waiting toneswill interrupt the customer’s call.3.6.10Call HoldCall Hold allows a station user to put any call in progress on hold by flashingthe hookswitch and then dialing a hold code. This frees the line to originateanother call. Only one call per station may be held at a time. The held callcannot be added to the other call. Dialing the hold code a second time canretrieve the original connection. If the station user hangs up with a party onhold, the user is automatically rung back and reconnected when answered.3.6.11Automatic Call BackThe customer subscribing to this service can, after reaching a busy station,hook flash and dial the activation code. When the busy station goes onhook and the calling station is on-hook, the calling station receives adistinctive ringing pattern. When answered, a call will automatically becompleted to the previously busy station.Automatic Call Back is available on a flat rate or usage sensitive basis.Under the usage sensitive offering, the customer will incur an activationcharge regardless of whether the customer chooses to advance orabandon the call. Upon the customer’s request, blocking of this feature isavailable at no charge.3.6.12Enhanced (Distinctive) RingingThis allows distinctive ringing to be applied to individual lines in addition tonormal ringing by assigning two directory numbers to the same line. Adistinctive ring for each number allows the customer to determine whichnumber is being called.Issued 4/1/1614

Lumos of West Virginia Inc. d/b/a SegraOne Lumos PlazaWaynesboro, Va. 229803.6.13MD Pricing GuideToll ControlThis service provides individual line and multi line hunt customers with thecapability to control placement of chargeable calls on the basis of thecalling line identity and the caller themselves by the "authorized" userdialing a preselected access code.3.6.14Rotary Line BreakThis service is available to customers with multiple incoming lines or trunks.It provides a customer the ability to decrease the total number of trunksavailable for incoming calls. Feature activation and deactivation isaccomplished by dialing an access and an activation or deactivation codeon a trunk member. When requesting service, the customer must specifythe number of incoming calls to be allowed after feature activation.3.6.15Caller IDThis service enables the customer to receive the calling party’s directorynumber, if within a central office base unit, during the ringing cycle and todisplay the number on customer premises equipment.3.6.16Call Trace (Customer Originated Trace)At the request of the customer, for up to 60 days, the Company will activatethe call trace feature. This service allows the subscriber receiving anobscene, harassing or threatening call the ability to request an automatictrace of the last call received. The call trace record is recorded on a printerlocated on the Telephone Company premises. The Telephone Companywill provide this information to the appropriate law enforcement agencyupon receipt of a valid court summons. This service is billed to thecustomer per call traced.3.6.17Selective Call AcceptanceThis service allows the subscriber to accept calls only from a pre-definedlist of up to twelve directory numbers. Callers with directory numbers notfound in the screening list will receive a message that the called number isnot accepting calls at this time. Call Acceptance can be turned on and offby the subscriber.Issued 4/1/1615

Callers with directory numbers found inthe screening list will receive a message that thecalled number is not accepting calls at this time. Call Block can be turnedon and off by the subscriber.3.6.19Selective Call ForwardThis service allows the subscriber to only forward calls from a pre-definedlist of up to twelve directory numbers. Callers with directory numbers notfound in the screening list will receive standard ringing or busy indication.Selective Call Forwarding can be turned on and off by the subscriber.3.6.20Selective Call WaitingThis service allows the subscriber to provide call waiting indication onlyfrom a pre-defined list of up to twelve directory numbers. Callers withdirectory numbers not found in the screening list will receive busy indicationif the line is busy. Selective Call Waiting can be turned on and off by thesubscriber.3.6.21Return CallThis service allows a subscriber to automatically recall the last incomingdirectory number to their line. This feature is accomplished without knowingthe directory number of the last calling party.Return Call is available on a flat rate or usage sensitive basis. Under theusage sensitive offering, the customer will incur an activation chargeregardless of whether the customer chooses to advance or abandon thecall. Upon the customer’s request, blocking of this feature is available atno charge.3.6.22Priority Call (Distinctive Ringing/Call Waiting)This service allows the subscriber to provide a distinctive ringing indication(or distinctive call waiting indication, if applicable) when calls are receivedfrom a pre-defined list of up to twelve directory numbers. Priority Call canbe turned on and off by the subscriber.Issued 4/1/1616

Excludedfrom this volume discount plan are Toll Denial, Rotary Line Break, and CallTrace. The specific volume discounts applicable for two or more serviceswill be provided to the customer as a monthly credit. Monthly creditsavailable under the plan are detailed in Section 4, following.3.6.24Message WaitingFor subscribers utilizing a compatible Voice Message Service, this featureprovides a message indication at the subscriber’s station that messagesare waiting. The message-waiting indicator will be provided in the form ofstutter dial tone or as a visual indication dependent upon the type ofcustomer premises equipment used.3.6.25Per Call BlockingPer Call Blocking allows a calling customer to block the display or deliveryof his/her directory number on a terminating or called party set which hasCaller ID as a purchased feature.Activation of this feature is affected by dialing an activation code prior toplacing a call. When the caller hangs up the feature is deactivated.Per Call Blocking will be provided subject to availability of facilities andequipment both at the serving central office and customer’s premises.3.6.26Anonymous Call RejectionAnonymous Call Rejection is an arrangement, available to Caller IDcustomers and non-Caller ID customers, that allows a called party to rejectcalls from parties that have activated the per call blocking feature to preventthe display of their telephone numbers to Caller ID subscribers WhenAnonymous Call Rejection is activated, such calls will be routed to anannouncement which tells the calling party that the called party will notaccept calls from callers who have chosen to prevent the display of theirtelephone numbers. The calling party will be instructed to hang up andplace the call again, without activating the per call blocking feature.Customers may activate or deactivate this arrangement by dialing anactivation code.Issued 4/1/1617
