GENERAL INFORMATIONARRANGEMENTSAll arrangements are made at Johnson Funeral Home & Cremation Services, 661 English Road, Rocky Mount,NC 27804, (252) 937-8886. Our staff is always available to meet families for an arrangementconsultation. In addition to the information on the next pages, we suggest the family bring clothing, aphotograph of the deceased, cemetery deed and insurance policies. Consultation and coordination with theclergy will determine the time and place of the funeral service. Our funeral director will coordinate with allrequested local and out of state newspapers for placement of the obituary notice. The Funeral Director willcompose this notice based on information provided by the family of the deceased. This arrangementconsultation can be easier and families can rest assure that one’s wishes are honored completely ifprearrangements are made with our funeral home before our services are actually required.SOCIAL SECURITY BENEFITSWith every service our staff provides, we send a “Statement of Death by Funeral Director” to the SocialSecurity office. This statement initiates the process to discontinue Social Security payments. Presently, SocialSecurity pays a 255.00 death benefit only to a surviving spouse or minor child. The surviving spouse andguardian of minor children must contact the Social Security Office in person to arrange for benefits. Thelocal Social Security office is located at 1071 Hunter Hill Road, Rocky Mount, NC 27804, (866) 964-0784,(M,T,Th,F - 9am to 3pm; W - 9 am to 12 noon), www.ssa.gov or the National Office Toll Free at(800) 772-1213.VETERANS BENEFITSAll United States veterans, with other than an dishonorable discharge, are entitled to these non-monetarybenefits: The American Flag, a United States Government issued grave marker and a burial space in thenearest National Cemetery for the veteran and his/her spouse. Our services include assistance with theseclaims and to do so will require the veteran’s DD-214 or comparable discharge papers. Again, certainveterans may be eligible for additional monetary benefits and we suggest an appointment at the localVeteran’s Affairs Office at 121 S. Fairview Road, Rocky Mount, NC 27801, (252) 977-3350 or you may callthe National Office of Veterans Affairs at (800) 827-1000.DEATH CERTIFICATESThis is a legal document certified by the State of North Carolina and is used to file all claims, transfer ownership of any assets or to continue Social Security Benefits for a minor. It is safe to assume that eachinstitution will need a certified copy, a photocopy may not be sufficient. The State of North Carolina currentlycharges 10.00 for each certified copy of the Death Certificate. Our staff may be able to offer assistance ifadditional copies are needed in the future.INSURANCE CLAIMSOur staff also offers assistance in filing insurance claims. The majority of insurance companies allow thebeneficiary to assign all or a portion of the policies benefits to the funeral home to cover funeral cost. It isimportant that the beneficiary of all policies is up to date and changed if necessary.4

You may choose onlythose items you desire. However, any funeral arrangements you select will include a charge for our basicservices and overhead.
