Theyhave also testified for car seat bills.

Ann Kloter sent an email to thecenters detailing the submission process,however the trauma centers have notsubmitted data. The upload option isfunctioning but there is no way to specifywhich date range to submit so eachcenter would be submitting their entireregistry.ACTIONDr. Gregg will contact AnnKloter to have DI fix theupload issue.Two questions were raised by committeemembers: are there any threats to thelongevity of the registry, and how woulddata from non-designated hospitals becollected. Dr. Gregg assured thecommittee that there is a strong interfaceand infrastructure to the registry andthere should be no longevity issues. Asfor the non-designated hospitals, thetrauma centers need to have a seamlessprocess first. A needs assessment willhave to be done to determine what datathese hospitals are able to collect.Legislative UpdateHB-5163This year, the field guidelines and NTDBData Dictionary aspects will be insertedinto the DPH technical bill HB-5163 andhas been referred for a public hearing.After the hearing, it will go the DPH for avote, then onto the House and Senate,and finally the governor. Once passed,work on Collector updates and dataDr. Gregg will keep thecommittee informed ofprogress.

The DPH supports theStop the Bleed campaign. Committeemembers reported the courses that havebeen held in the various trauma centersand in the community. There is someresistance from Boards of Education dueto liability issues.Although military data is extraordinary,there is some concern about how totranslate the military experience to thecivilian population. Very little data existsand there is concern about how thelayperson will respond in a stressfulsituation.Dr. Campbell read a message from Dr.Jacobs about other states issuing aproclamation for Stop the Bleed Day onMarch 31 and the suggestion thatGovernor Malloy be contacted aboutissuing a proclamation for the State ofConnecticut.Dr. Gregg will contact DPHand will keep the committeeinformed of progress.Injury PreventionSubcommitteeThe committee decided that data from thetrauma centers need to be submitted tothe state registry first. The data can thenbe analyzed to determine potential injuryprevention initiatives.Hemorrhage controltraining for EMSCEMSAB felt that Stop the Bleed wasgeared more toward the layperson andthe EMS providers would benefit fromformal hemorrhage control training. Thecourse was developed by the educationtraining subcommittee and brought toCEMSMAC and referred to the StateTrauma Committee for comment.Dr. Gregg asked committeemembers to send commentsabout the course to Dr.Kamin.Liaison to the ACSCOTThe liaison position to the ACS COT isopen. This is a legislative position thatwas held by Dr. Jacobs. Committeemembers felt that Dr. Gregg should holdthe position. Dr. Gregg relayed that theDr. Gregg encouragedcommittee members to applyfor the position ACS-COTliaison position.

Chair of the State Trauma Committee isnot a legislative position and is appointedby DPH, whereas the ACS-COT liaison isappointed by the Governor. The focus ofthe two positions are also different; theACS-COT has their charter for trainingand trauma systems, and ACS matters,whereas State Trauma Committee is aregulatory board with a focus on stateregulations.Duties and Goals for2018The Duties and Goals for 2018 weresubmitted to the committee forsuggestions or changes.EducationalOpportunitiesHartford Hospital is sponsoring anASSET Course in June. Instructions for thetransition to the NTDB dictionary weresent out to the EMS agencies.A symposium, “Opioid OverdoseEpidemic: the EMS Role: will be held May30 at Foxwoods.Overdose is a reportable occurrence byEMS. DPH is working with HIDTA todevelop a reporting tool. In August, a pilotprogram between the Connecticut PoisonControl Center and AMR will begin wherethe EMS crew will call into the PoisonControl Center while on scene with anoverdose.The committee is working to determinehow paramedicine will be delivered in thestate. Based on community needs, theagency would file an application to meetthe perceived gap.There were no updates.HB-6153 passed the House and SenateGo Live date is October 1,and is now with the Governor. This bill2018.supersedes the regulations and allows forthe National Trauma Data Bank datadictionary to be the data dictionary for thestate trauma registry. This also permitsannual NTDB dictionary updates and willallow for transition to Collector V5 for thestate trauma registry.

It was noted that although data from2012 forward is to be submitted, there isno way to delineate the time frame for thesubmission file.Hospitals in the state will transition toCollector V5. Funds have beendesignated for data migration from CV4.Digital Innovations will work with eachtrauma center to ensure that hospitalspecific data points are also migrated tothe new version.Training has been updated to include thefeedback from the committee.A potential candidate for the position hasbeen identified.The Governor’s Proclamation for NationalStop the Bleed Day 2018 was distributedto the committee.Once the level of support from the stateagencies is identified, Stop the Bleed willbe brought to CEMSAB to determine thedrive for a public health effort. Thecommittee’s concerns are fundingsources and the lack of evidence-basedresearch in the efficacy of tourniquets inthe civilian population.Ann Kloter will keep thecommittee informed ofprogress.Digital Innovations willprovide an update that willallow for date rangedetermination.Dr. Gregg will keep thecommittee informed ofprogress.

Collector V5 has beenpurchased, and what is now needed is alist of thing that the trauma centers needto do prior to upgrade from ConnecticutCollector V4. A conference call withDigital Innovations will be held to get thedetails. Dr. Gregg requested that traumacenters email him with a list of questionsprior to the call.Data migration was discussed. Ten yearsdata will be migrated from V4 to V5 at 3750 for migration and 1750 formapping. Custom fields will be an addedcost. The state budget for migration is 21,000. The plan is to distribut V5 to thetrauma centers first, analyze data, thendevelop a plan to distribute to the nontrauma centers.Liaison to ACS COTpositionWill need a V5 update go-live date first.Migration can be done at a later time.This is a governor-appointed position. Nomovement has been made.Dr. Gregg will keep thecommittee informed ofprogress.

Gregg, M.D.Trauma centers have been runningcourses. The goal now is to encourageEMS to conduct courses to educate thecommunity.For mass casualties, field triageguidelines are not to be used. It was therecommendation of the committee thatthe field triage guidelines should not bedirectly referred to but should includelanguage that critical patients should betransported to the hospital best capableof handling critical patients.The draft Resuscitation Initiation andTermination guideline (6.15) has beensimplified. Under Signs of Death,absence of heart sounds is listed but thisis not in the EMT skill set. Under Factorsof Death, major blunt or penetratingtrauma incompatible with life is notspecific enough. Deforming brain injuryand extensive full thickness burns shouldbe defined further. Dr. Gregg suggestedretaining the language from the currentguideline that gives specifics so as toreduce unnecessary transports.Dr. Gregg will bring thecommittee’s recommendationto CEMSMAC.Dr. Gregg will bring thecommittee’s recommendationto CEMSMAC.
