Historical BackgroundBefore 1937, the treatment of fractures in large and small animals was confined mainly to casts and splints.Between this period and the mid-1960s, fixation by Stader and Kirschner-Ehmer splints, bone sutures, and theoccasional Sherman or Lane bone plate was added to the veterinary armamentarium. The functional and anatomicalresults were less than desirable in many cases, and nonunion, malunion, and stiff joints were not uncommon.This mirrored the state of the art of internal fixation of fractures in man at the time.Fracture treatment, however, took a tremendous leap forward in 1958 when the Swiss Association for the Study ofInternal Fixation (in German: Arbeitsgemeinschaft für Osteosynthesefragen—AO) defined the biomechanical principlesfor successful internal fixation of fractures, and implemented these principles with the creation of an entire system ofstainless steel implants and instruments. Their applied fracture research was carried out in the Laboratory forExperimental Surgery in Davos, Switzerland, based on metallurgical expertise gleaned from the watch-making industryand expert European toolmakers.Documentation in Bern of more than 100,000 clinical cases using these AO methods, and based on a comprehensiveclassification of fractures, confirmed the reliability and quality of the system. The central theme was the rapid return offunction after fracture treatment. This was accomplished by anatomical reduction, stable internal fixation with implantsutilizing interfragmentary compression, buttressing, medullary splintage, and preservation or enhancement of the bloodsupply in the fracture zone by early exercise and early bone grafting. There was usually no necessity for casts orexternal splintage. Emphasis was also placed on preoperative planning, operative asepsis, gentle soft tissue handlingand atraumatic exposure of the fractured bone, and early postoperative mobilization of the part and the patient.Protected weight-bearing and careful radiographic monitoring until union was achieved completed the regime.The bridge between human and veterinary fracture treatment, and indeed orthopaedics, began to be built early in the1940s by Kuntscher with the experimental work on intramedullary nailing in dogs. Dr. Jacques Jenny, as a resident inZurich in 1943, assisted Dr. H. Knoll and Dr. H. Willenegger with the first clinical canine femoral intramedullary nailing.In 1965, Drs. J. Jenny and R.B. Hohn, encouraged by Dr. Howard Rosen, introduced AO techniques in large and smallanimal orthopaedic surgery at the University of Pennsylvania and the Animal Medical Center in New York, respectively,utilizing human AO principles and instrumentation.Following participation in 1969 at a human AO course in Davos, a number of active American and Swiss veterinarysurgeons initiated courses for veterinarians at Bettlach, Waldenburg and Davos in Switzerland, and at the Ohio StateUniversity in Columbus, Ohio, in the United States. Veterinary AO sections were started in these two countries with thespecial help of Fritz Straumann, and were united with groups from other countries as an international organization in1970. Basic and Advanced courses have been held regularly in Switzerland, the United States, and other countries.Research led to the design of new implants which would conform to the anatomical and functional requirements ofanimals, and to the definition of techniques specific for the various animal species. The original AO principles haveproven to be as successful in large and small animals as in humans. 1-1The AO set system permits a reasonable number of basic instruments to be used for many applications and the ability to combinethese standard devices with additionally available instruments and implants for special cases and applications.Standard Instrument SetsThese sets contain basic instruments and implants which permit the treatment of most fractures. Their use is described in the latesteditions of the AO Principles of Fracture Management in the Dog and Cat and the AO Principles of Equine Osteosynthesis. If thetechniques described in these manuals are properly followed, the chances for successful fixation are greatly improved. It isimportant that the standard instrument sets should always be complete so that during an operation an important instrument orimplant is not missing. While the number of screws or plates in the implant sets can be reduced or adapted to the usage of theclinic, it is not advisable to delete certain lengths or certain types of plates.Note: Because of the continuing development of the AO Instrumentation by the AO Technical Commission, it is possible thatshape, form and dimension of the instruments and implants or the contents of certain standard sets are changed fromtime to time.
