Letter from thePresident & CEODear Friends,Sea Mar Community Health Centers is pleased to share the 2015 report to the community, illustrating highlightsand services over the last year.The Affordable Care Act has brought many positive changes in healthcare. Thanks to the ACA, there are anestimated 609,395Washingtonians who were formally uninsured, but now have insurance. Since the launch of theACA in 2013, Sea Mar alone has assisted 162,919 individuals with insurance enrollment and Sea Mar’s uninsuredpatients decreased by 47% from 2013 to 2015. Expanded health coverage means patients have increased access ffordablefor those without insurance. At Sea Mar, we’ve responded to this increased demand by opening new medical clinicsin Battle Ground, Vancouver and Elma, and expanding medical and adding dental services in Bellevue. These newsites have also allowed us to expand the provision of services at 91 sites across Washington.Affordable housing is a significant issue faced by the communities we serve. We’re proud that over the last year wewere able to open a new affordable housing development in Des Moines, WA to complement our current housingservices in Seattle and Pasco. As the affordable housing crisis continues to grow, we are making future plans to addadditional affordable housing developments in other parts of Washington state.Sea Mar is committed to the national Triple Aim of improving the health of the patient, improving the health ofpopulations, and reducing the per capita cost of health care—a model that will help bring Sea Mar to the nextlevel. We are placing top priority on population health management and improving health indicators, as well asimproving customer service. We are putting strategies in place to improve the overall health of our patients andthe community by focusing on clinical quality measures that are designed to improve preventive health outcomes,chronic conditions, and overall health.This includes implementing a team-based model of care where providers aresupported by a team of clinical staff that help them meet patient goals and a focus on integrated care so patients areable to easily access services under one roof. Sea Mar’s goal is to be the provider and employer of choice, and overthe last year we’ve focused our efforts on tactics that will directly improve the patient and employee experiences toensure “Exceptional service. Every person. In 2015,Des Moines Affordable Housing project opened its doors to its first residents. BeforeLatonia became the first person to call Des Moines a home, Latonia was living inher car with two kids with no hope for a brighter future. Latonia suffered anaccident on her job and was unable to continue working. Without enoughincome to afford housing, she eventually became homeless. Living in a carwas not a good environment for raising her two kids. What followedwould change her life forever.Now Latonia is back on the workforce and her oldest daughter wastransferred to a better high school where she is getting straight A’s.“Iowe this place everything,”Latonia says. Latonia recommends Sea Marto others who are going through hard times because of the service shereceived. Sea Mar was able to help her get back on her feet in the faceof significant challenges.“I’m so grateful to everyone who contributedto making this possible.”8

HighlightsHow We’ve GrownDuring fiscal year 2015, 22,928 new patients were addedto the Sea Mar care community. This growth is due inpart to the opening of the following clinics below:Received HRSA New Access Point funding to open new clinicin Battle Ground. This year’s report shows that through SeaMar’s online and mobile health services; 803,051lab test results were viewed online, 4,303 were refilled.“The portal has reduced stress as patients simplylog into their secure portal and view their labresults virtually as quickly as it is viewed by theirprovider. Likewise, waiting for a lab result can bestressful for patients,”Doug added.This access toknowledgereducesstresslevelscausedbydelays;no more phone-tag or fret over missing a callfrom our clinic.Sea Mar patients can send messages directlyto their provider’s care team from their portal.Often these are updates or easily answeredquestions regarding a recent visit. Highlights2015 REPORT TO THE COMMUNITYPopulation Health & Care Managment: Innovations For a New EraThe term“population health”is receiving a lot of focus atSea Mar. The 2010 passage of the Affordable Care lthsystems in a generation. As its impact reverberates fromthe federal level into state governments and beyond,Sea Mar is assuring the organization is well positionedfor this new era of health care reform. The old waysof paying for individual health care services through thefee-for-service model is being challenged and a arterm within population health, the‘Triple AIM’initiativedemands better care for individuals, better health forpopulations, and lower per capita costs. For this reason,Sea Mar has adopted a population health approach as acentral pillar in providing exceptional healthcare to thepatients we serve. Moving beyond providing services toan individual, we focus on how to strategically engagepatients for more effective health outcomes. It is theprimary mindset we now employ in our approach toworking with patients and designing our systems andservices.Care management has been proven to work. At thestateleveltheCareManagementHealthHomeprogramis improving health and showing Medicare savings.Thereport released by the Centers for Medicare & MedicaidServices (CMS) indicates 21.6 million in savings inMedicare/Medicaid costs, of which the state couldreceive approximately 10.8 million.“This initiative notonly drives positive improvements for individuals withchronic and complex health needs, but also providesshared savings to the state for reducing Medicare healthcare costs,” said Bill Moss, DSHS Assistant Secretary.Since the program’s inception, 1,063 patients have beenenrolled in the program.These patients have benefitedfrom dedicated care coordination time being spent sand moving towards a healthier lifestyle in the future.Care management is composed of an interdisciplinaryteam of nurses, community integration specialists andcase managers.This team approach is person-centeredand successfully engages difficult-to-reach patients byproviding a more comprehensive understanding of thechallenges they face, working outside the clinic setting.Care managers reach out to patients in their homes, aswell as in other settings such as food banks, hospitals,homeless shelters and supported living facilities. Theteam then works closely with clinical staff to ensurepatients are connected with their primary physician andthe in-clinic care coordination teams to meet healthgoals and increase health outcomes. Sea Mar is excitedto see this program continue to grow and contribute toour goal of “Exceptional Service. Every Person. gramasaway to more effectively address population health. Caremanagement focuses not only on the medical factorscontributing to a person’s health, but also on the socialdeterminants of health: the conditions where peoplelive, work, learn and play that impact their health status.Sea Mar’s care management model aims to improvethe transition of patients who are ‘high-utilizers’ of theemergency department to outpatient care and ensurethey continue to get the medical and other cialneedsarenot met through the current fragmented health caresystem.This initiative not only drives mplexhealth needs, but also provides shared savings to thestate for reducing Medicare health care costs.11

Every Time.When Sea Mar Community Health Centers welcomedits first patients in 1978, its sense of mission was pecializingin service to Latinos.” Sea Mar was founded in 1978 bya group of Latino community leaders to address tyand cost. It involves looking at every aspect of how careis delivered, including how the patient first comes intocontact with Sea Mar. Efficient processes and strongclinical outcomes are critical components of a patient’sexperience, but these alone are not enough to achievean excellent experience.Patient experience is a recognized component of highquality care for Sea Mar. Continually learning to stay current with industrystandards, best practices and technology.What Sea Mar Patients Are SayingIn 2015 Sea Mar established the use of a eportsofpatients’experiences.This survey became the foundingmodel for measurement of the patient experience andis being conducted broadly throughout Sea Mar. As oftoday, Sea Mar has surveyed 3,250 patients. Results fromthe survey proved that 83 percent of Sea Mar patientsreceived “Good” to “Excellent” service.Sea Mar’s goal is to bring this score to100% in 2016 llent”experience.12

Moving from Good to Excellentwill be designed to help familiarize employeeswith Sea Mar’s mission, vision, service culture andorganizational goals.In 2015, Sea Mar launched an organization-widecampaign to improve the patient experience. SeaMar’s“Improving the Patient and Employee ExperienceCampaign” is meant to engage all employees and givethem the tools they need to provide “Exceptionalservice. Every person. Every time.” New employee recognition programs for thosewho exhibit exceptional customer service. ��Exceptional Service. Every Person. Every Time.”2015 REPORT TO THE COMMUNITYIn response to this challenge Sea Mar embarked on istingofpatientexperienceprofessionalsandservesas a patient experience advisory resource for criticalinitiatives across Sea Mar. Continued employee and patient focus groups tobetterunderstandwherepatientsandstaffbelievethey fit into the the Sea Mar experience and“movethe bar” in terms of patient satisfaction.The Improving the Patient and Employee testamentto the power of the positive; not just positive thinking,but positive meaningful action. Most importantly, it isa reflection of the power of partnership of patients,families and providers working together to redesign thesystem through the patient’s eyes.There is much to learnand each one of us has something to urvey to hear directly from staff about the methodsand tools they need to create an exceptional helpingshape the campaign’s priorities.The survey pointed outa need for Sea Mar to focus on enhancing not only thepatient experience, but also the employee experience.Thanks to the feedback of Sea Mar employees ook action to plan and implement various initiatives: The service improvement committee will look nges to improve service delivery. AIDET customer service training as theorganizational patient communication tool toimprove service excellence by engaging all staff ina structured methodology that facilitates patientcommunication and decreases the number ofpatientcomplaintsrelatedtocommunicationgaps. Our FutureSea Mar continues to grow to serve the needs of the communitywith the expansion of exciting projects:New Sea Mar Community and Cultural CenterOn November 24, 2015 Sea Mar attended the signing for thelegislationthatwillprovide 28milliontoarts,heritageandhistoricalpreservation projects throughout King County.The center will be located at 9635 Des Moines Memorial Drive in the South Park neighborhood of Seattle. Planscall for a two-story, 22,000 square feet, multi-purpose complex that will include an education and community centerand Latino cultural museum.The education center will offer meeting space, office space, classrooms, art rooms, andan athletic center that will be home to the Sea Mar youth boxing program. The Latino Cultural Center will displayexhibits including film, artifacts, textual narrative and interactive displays that will tell the story and history of Latinosin the Pacific Northwest and their unique story of migrating to the region.New ClinicsConstruction has begun on new clinics located in Federal Way, Vancouver and Ocean Shores that will open laterin 2016. In late 2015, Sea Mar received a federal grant from the Bureau of Primary Health Care that will help fundthe construction of the Ocean Shores project. With these new locations, Sea Mar is committed in serving thehealthcare needs of those who need it the most throughout Washington state.I think Sea Mar is abeautiful organizationErica Chavez-SantosGrowing up in the small farmworker town of Pateros,WA, Erica witnessedfirst hand how health disparities affected her own community.With thepassion and determination to make a change in the healthcare system,Erica decided to pursue her pre-medical studies at the University lscholarshipand was able to graduate with a dual degree in biology and publichealth.She is now part of Sea Mar’s HealthCorps program and will soon ifulorganization.Everyone has a story and I learn from working with them. Patients helpme understand the current inequities in our healthcare system,” shesays. For Erica, Sea Mar has given her a better understanding anddeliveringholisticcare.142015 REPORT TO THE COMMUNITYSea Mar is a grateful recipient of one of the 100 grants being awarded through4Culture as a result of this legislation. The grant will be used for the construction of SeaMar’s Community Center and Latino Cultural Museum, opening early 2017.
