Verizon Wireless reserves the right torevise, cancel or amend this Program at any time without prior notice.Under the Program, targeted sales activities will be rewarded with ”Bonus Bucks” that can beredeemed at hundreds of on-line stores or that can be traded for Gift Card(s) to be used at favoritenational retailers and restaurants.As a sales employee of a Participating Agent, you must register at www.vzwbonusbucks.com tocreate their individual account to participate in the Program (“Eligible Program Participant”).Whenever you log into the system, you will use your Email address and password. This Emailaddress will be used to communicate program updates, award winners, and tax information.Your eligible sales activities must be entered at www.vzwbonusbucks.com, where they arereviewed, declined/approved, and then, if approved, you will receive Bonus Bucks, via Email, thatcan be redeemed at www.VZWrewards.com immediately or accumulated to purchase a higherpriced item.It's just that simple!You can check your account balance at anytime by logging into your personal account.When you're ready to redeem your Verizon Wireless Bonus Bucks, go towww.VZWRewards.com.2.Sales Qualifiers and Dollar ValuesIn any given month, when a Program incentive is in place, Eligible Program Participants will benotified of the current incentive by email. For all approved sales, you willreceive, via Email, your VERIZON WIRELESS Bonus Bucks certificate! This certificate includesa 16-digit number to be redeemed at www.VZWRewards.com. It’s that simple!Additional Qualifier Details¾If identified as a targeted sales activity during a Program period, new Activations are requiredwhen selling a new Calling Plan (for example: If a 5 spiff is being offered on all CallingPlans 39.99 , then only new activations that meet this criteria can be entered. All others willbe declined.¾If identified as a targeted sales activity during a Program period, all features added to a new orexisting account must remain on that account for a minimum of 30 days in order to receivecompensation through the Program.¾Eligible Program Participants entering sales information into the Program website must onlyenter sales that they have completed. Eligible Program Participants are not permitted tocombine sales. Participating Agents may not assign one person to enter in all sales, this goesagainst the rules and regulations of the Program In the event this occurs, all sales for theParticipating Agent and/or Eligible Program Participant may be denied for the month theywere submitted, and the Participating Agent and/or the Eligible Program Participants may bedeemed ineligible to further participate in the Program.¾Any MTN entered incorrectly will be denied. Additionally, the eROES ID and Outlet IDentered into the Program website must match the information in Verizon Wireless’ datawarehouse. If the information does not match, the entry will be denied.¾Cash-out period (usually the 3rd week of the month following the incentive month) all pendingsales will be flagged as denied. If you feel that a sale entered into the Program website islegitimate, the following process needs to occur for an internal review of the transaction: Once the cash-out period takes place, the support link on the bonus bucks site willinclude a discrepancy form that can be filled out. The period to submit/review anyexception(s) will be open for two (2) weeks. Once this period ends, no exceptions will bepermitted. Please submit the following information and the reason for the exception. 3.User InterfaceThis is the website you will submit all of your new targeted sales activities and review youraccount balance! The URL is www.vzwbonusbucks.com. Please be sure to enter all informationcorrectly. Remember, only targeted sales activities within the West Area will be verified.Logging into the systemAll participants must register to create individual accounts in the program to be deemed anEligible Program Participant. Whenever you log into the system, you will use an individual Emailaddress and password. This Email will be used to communicate program updates, award winners,and tax information.Submitting salesAll Eligible Program Participants may enter sales for the present month within the followingtimeframe:Throughout the current selling month, Eligible Program Participants have 7 days to enter theirtargeted sales activity information. PLEASE NOTE: Due to the reporting cut-off, the last dayto enter any end of the month sales information will be the 4th day after the close of themonth.Example:If you sold a qualifying rate plan on Jan 1, you would have until Jan 8 to enter that informationinto the Program website.If sold a qualifying rate plan on Jan 31, you would have until February 4 (the last day) to enteryour information into the Program website.We ask that you be prompt when submitting sales so that we can verify the sales and send awardsin a timely manner.Editing Sales – Eligible Program Participants can edit sales information (for updates andmistakes) within the same month.Example:If you sold a qualifying rate plan on January 5 and the customer added a qualifying feature onJanuary 15 of the same month, you can go to reports, choose the Mobile Number and click on“edit”. By doing this, you can now add your feature to this sale.Checking reportsThe Eligible Program Participant logs into the system to verify which sales their manager hasapproved and their earned award total. You are able to view your individual account balance aswell as account history.Eligible Program Participants can also view a monthly sales report. Go to reports and select themonth you wish to view.SupportShould you have any questions or concerns regarding your individual accounts, contact your HelpRepresentative at support@vzwbonusbucks.com. You can shop by category, see acomplete list of Member Merchants, or use our Find-a-Gift Search Engine to locate a specificitem. You can also Shop at VZW for the latest in wireless products or trade in your Bonus Bucksfor gift cards at your favorite national retailers and restaurants. When you visit one of our MemberMerchant sites, just choose Bonus Bucks as your payment method and enter your 16-digitcertificate number.How do I know which Web sites accept Verizon Wireless Bonus Bucks?All of our Member Merchants are listed here on your rewards site so you should always startshopping at www.VZWRewards.com.What if I don't spend the full amount of my Verizon Wireless Bonus Bucks?Not to worry! Your remaining value will always be available to you. 6.Terms and ConditionsEligible Program Participation AgreementFor Eligible Employees of Participating AgentsVerizon Wireless Bonus BucksIndirect Sales Incentive ProgramI (“Eligible Program Participant”) hereby acknowledge and agree to the terms and conditions ofthe Verizon Wireless Bonus Bucks Indirect Sales Incentive Program (the “Program”) as set forthin the Program Terms and Conditions and Rules and Regulations below.6.1 Eligible Program Participant agrees to provide the information required by Verizon Wirelessfor the purpose of the Program and for tax reporting purposes under the Program. EligibleProgram Participant consents to the use of information provided by Eligible Program Participantfor the purpose of the Program. Eligible Program Participant represents and warrants that theinformation provided to Verizon Wireless or its vendors as part of the Program shall be accurateand complete, and shall be updated by Eligible Program Participant to maintain accuracy. Theinformation required shall include legal name, address, social security number, employerinformation and applicable Verizon Wireless sales data. Eligible Program Participant shallprovide information by registration via the “VZW Bonus Bucks” Program web site (“ProgramWeb Site”).6.2 Eligible Program Participant acknowledges that the Program Web site shall be operated by,including the direct issuance of all Bonus Buck certificates, a third-party vendor selected C(“GIVEANYTHING”).GIVEANYTHING shall have access and use the information provided by Eligible ProgramParticipant to implement and run the Program.6.3 Eligible Program Participant acknowledges and agrees that the monetary value of all BonusBuck certificates may be reported to the IRS as taxable income to Eligible Program Participantand may be subject to federal, state and/or local income reporting requirements. Such reportingwill be prepared and delivered by GIVEANYTHING, as payor, to Eligible Program Participant, inaccordance with applicable federal, state and/or local income reporting requirements. EligibleProgram Participant will be solely responsible for the payment of any federal, state and local taxesarising from the issuance and/or redemption of the Bonus Buck certificates.6.4 Eligible Program Participant acknowledges that there is no guarantee that communications ortransactions conducted online will be secure. Eligible Program Participant is solely responsible formaintaining the confidentiality of Eligible Program Participant’s password or PIN code for theProgram Web site, and agrees that Verizon Wireless and GIVEANYTHING have no obligationswith regard thereto. Eligible Program Participant further acknowledges that there may be systemfailure that may limit Eligible Program Participant’s ability to use the Program Web Site. EligibleProgram Participant agrees to assume all risk and liability arising from use of Program Website,including the risk of breach in the security of the communications or transactions conductedthrough Program Website. THE PROGRAM WEB SITE IS PROVIDED "AS IS" WITHOUTWARRANTY OF ANY KIND, WHETHER EXPRESSED OR IMPLIED, INCLUDINGWITHOUT LIMITATION THE IMPLIED WARRANTIES OF MERCHANTABILITYFITNESS FOR A PARTICULAR PURPOSE OR OTHERWISE.6.5 IN NO EVENT SHALL VERIZON WIRELESS OR GIVEANYTHING BE LIABLE TOELIGIBLE PROGRAM PARTICIPANT OR ANY THIRD PARTY FOR ANY SPECIAL,INDIRECT, INCIDENTIAL, CONSEQUENTIAL, PUNITIVE DAMAGES, LOST PROFITS OR5Last Updated 8/27/08
ANY DAMAGES WHATSOEVER, WHETHER IN AN ACTION IN CONTRACT,NEGLIGENCE OR OTHER TORTIOUS ACTION, ARISING OUT OF OR IN CONNECTIONWITH THE USE OR PERFORMANCE OF THE PROGRAM, THE PROGRAM WEB SITE ORTHE INTERNET GENERALLY.6.6 Eligible Program Participant hereby expressly releases Verizon Wireless andGIVEANYTHING and their employees, agents and representatives, and all persons acting with orin concert with them, or any of them (collectively, the “Releasees”), and each of them, jointly andseverally, of and from any and all causes of action, damages, claims, liabilities, and demands ofwhatever kind or nature, in law or in equity, both known and unknown, whether contingent orfixed, that Eligible Program Participant has had or may have, now or in the future, against theReleasees arising from or relating to this Program. Eligible Program Participant expressly agreesthat the release set forth in this paragraph extends to all claims of any nature and kind whatsoever(known or unknown, suspected or unsuspected) arising out of the Program.6.7 The construction, interpretation and performance of this Agreement shall be governed by andconstrued in accordance with the laws of the State of New York without regard to any conflicts oflaw principles that would require the application of the laws of any other jurisdiction6.8 Eligible Program Participant accepts and agrees to abide by the Program Rules andRegulations as set forth below. Verizon Wireless’ interpretation of the rules shall be final.“VZW Bonus Bucks Rules and Regulations”This “VZW Bonus Bucks” indirect sales incentive program (the “Program”) is a promotion forSales Associates and Store Managers of Participating Agents, who have registered on the ProgramWeb Site and have provided all required information, only (“Eligible Program Participant(s)”).1.Participating Authorized Agents are authorized Verizon Wireless agents on current contractswho agree to allow their employees, subagents and subagent’s employees to participate in theProgram. Sales made prior to the execution of all applicable agreements, whether by agent,sub-agent or participant, shall not be eligible for rewards under the Program.2.All Eligible Program Participants must be continuously employed by, and in good standingwith, the Participating Authorized Agent of Verizon Wireless or Participating AuthorizedAgent’s subagent for the entire length of this Program, including the issuance of the incentiverewards.3.To qualify for incentive rewards, Eligible Program Participants must register at the ProgramWeb Site and must submit required personal information and sales activity information to theWeb Site, subject to verification and approval by Verizon Wireless, which shall be at its solediscretion.4.All sales activity will be tracked by Eligible Program Participant’s Social Security number.Failure by Eligible Program Participant to provide his/her Social Security number or any otherrequired information upon registration at the Program Web Site will result in ineligibility forthe Program. Verizon Wireless and GIVEANYTHING shall have access to all EligibleProgram Participants’ information; including, but not limited to, social security numbers, forthe purpose of providing the Program.5.The eligible Products are:Eligible ProductsPrice plans, products and features that Verizon Wireless chooses to reward6Last Updated 8/27/08
6.No split sales are permitted. Only the Eligible Program Participant who submits the salesinformation to the Program Web Site shall be eligible to receive the incentive rewards for thesale. Transfers of incentive rewards, and incentive reward credits will not be permitted. Noneligible participants may not “donate” or transfer their sales to Eligible Program Participant.7.Upon request, copies of valid Verizon Wireless Subscribers’ sales receipts must be submittedas evidence of each Eligible Program Participant’s sales activity history.8.Verizon Wireless will be the sole judge of eligible sales and Program disputes and all disputesregarding award eligibility and award issuance made by Verizon Wireless are final. VerizonWireless will audit all sales information submitted for accuracy. Participation is based onEligible Program Participants adherence to all rules of this Program. Any evidence that totalintegrity was not maintained throughout the Program will be grounds for disqualification ofParticipating Agent and/or Eligible Program Participants, at the sole discretion of VerizonWireless.Eligible Program Participants will earn incentive rewards, subject to verification and approvaland subject to the terms and conditions of this Agreement. Incentives are only awarded inBonus Buck certificates redeemable at the Program Web Site for merchandise at participatingonline and retail merchants only. Rewards certificates are not redeemable in cash.11. Incentive Rewards will not be issued until the Program period has expired and Verizon Wirelesshas verified and approved the Eligible Program Participant’s sales activity. Fraudulent reportingshall result in immediate disqualification in the Program.12. Upon completion of the Program and verification and approval by Verizon Wireless, EligibleProgram Participants will be issued the incentive rewards in the form of a “Bonus Buckcertificate”, delivered via email, which may be redeemed at the Program Web Site formerchandise at participating online merchants or gift cards for retail merchants and restaurantsonly.13. The monetary value of all Bonus Buck certificates may be reported to the IRS as taxable incomeand may be subject to federal, state and/or local income reporting requirements, which reportingwill be prepared and delivered by GIVEANYTHING, as payor, to Eligible Program Participants,in accordance with applicable federal, state and/or local income reporting requirements. EligibleProgram Participants will be solely responsible for the payment of any federal, state and localtaxes arising from the issuance and/or redemption of the incentive rewards.14. Verizon Wireless reserves the right to revise, cancel or amend this Program at any time withoutprior notice. Verizon Wireless is the final and sole authority for making and interpreting the rulesand regulations and to determine eligibility of Program participants and issuance of the incentiverewards. All decisions are final.15.
