Most of these planshave benefits for services you get from doctors who are both inside andoutside our network of providers. Others only cover doctors who are inthe network. And the doctors,hospitals, and facilities in our network are the heart of our PPO plans.These network providers have agreed to accept a lower rate for carecovered by your plan. (We negotiate this rate with them ahead of time.)They won’t bill you for anything above this lower rate we’ve worked out with them.You are responsible for your deductibles, copays, and for any charges not covered by your plan.Providers in our network will also file your claims and bill us directly. So you don’t have to dothis time-consuming and often confusing job.Most of our PPO plans let you get care from a provider who isn’t in our network. Please beaware that you’ll pay more out-of-pocket for these services. And you may have to pay for thecare when you get it, and then submit your own claim forms to Anthem. (If you’re enrolled in anEPO plan you don’t have benefits for care you get from non-network providers.)EPO planIf you’re a member in our EPO plan, you have benefits only when you see a network provider.The only care that’s covered from non-network providers are emergency services, urgent careor an authorized referral. Members enrolled in an Advantage PPO plan can choose the network tier hospital thatworks best for them. Tier 1 has PPO network hospitals that charge an even lower rate. Tier 2 isthe rest of our large Anthem PPO hospital network. Tier 3 has non-network hospitals. You’ll findTier 1 (T1) and Tier 2 (T2) hospitals in this directory. Your plan base is the ACO network of doctors, medical groups and hospitals which have the lowest overallcosts.
