PATH Full Wellness SolutionSTEP 1:Online HealthSurveySTEP 2:BiometricScreeningSTEP 3:DataAnalyticsEmployees will answera questionnaire to learnmore about their lifestylehabits and how theyimpact overall health. Atthe end, they’ll scheduletheir biometric screeningappointment throughour secure portal. Allprogram communicationand marketing can betaken care of by yourdesignated ProgramCoordinator.Our team will comeon-site to offer a fullbiometric screeningpanel measuring bodycomposition, bloodpressure, glucoseand cholesterol.At the screening,employees will havea brief coachingsession—identifyingtheir most importanthealth goals. Questlaboratory and physicianform alternatives areavailable.Your employees’ healthdata is automaticallysent to our HIPAAcompliant analyticsplatform. Initialand final weigh-ins are included.Hearing and vision specialists will come on-site to performscreenings for employeesDiabetes Prevention ProgramPATH’s year-long program has received full recognition bythe CDC as an evidence-based program that meets theirstandards for ensuring long-term outcomes, boasting a 9%average weight loss sustained over 1 year.Health CoachingA Certified Health Coach works one-on-one with youremployees to help employees discover manageablelifestyle solutions that create lasting health improvement.On-site, telephonic and video coaching are available.Nutrition Consultation and Meal PlanningOur Registered Dieticians will provide a nutrition plantailored to each employee’s unique goals and dietary needs.The custom meal plan will be housed on an interactiveplatform that the coach and employee can each viewand adjust as needed. This class is led by a behavioralmodification specialist.Ergonomic ProgramThis program takes a holistic approach to joint andmusculoskeletal health. Mental Health Resources24 Hour Crisis LineBaptist Health has a free, 24-hour crisis line that connects callers with the local care theymay need. Call 904.202.7900 for assistance with crisis or to find hospitalization options formental health needs.Support Groups – Emotional HealthOur trained team of licensed professionals will guide employees through managing theirstress, balancing work and life demands, combating anxiety and more. PATH to Peak PerformanceEmpowering Leaders, Managers and Team Members with a strengths-based approach to success. Research shows that individuals whoknow their strengths are 6 times more likely to be engaged at work and 10% more productive in their role.Individual Strengths Assessment and Coaching Session –Team Workshop – Optimizing Our Strengths – Online Strengths Finders Assessment Strengths Insight & Signature Themes Report 30-40 minute Individual Coaching Session- Participants will learnabout their Top 5 Strengths and how to maximize their uniquetalents for optimal success Assessment may be offered to leaders, managers and/or teammembersTeams will learn how to maximize their strengths, identify areas ofimprovement and appreciate the strengths of fellow team members.Participants will discover how each person’s individual strengthscomplement each other’s and contribute to top team performance.Group workshops are typically 90-120 minutes in length. Individualcoaching sessions may be added.Managing for Performance Coaching Session –Managers will learn how their Top 5 Strengths Profile impacts theirleadership, management and motivational style. Your PATH to Safely Return to WorkWe’ve combined our medical expertise, top clinicians and the latest AI technology to ensure your employeesare returning to work safely and have the resources they need.Digital TemperatureScreening with Nurse TriageOn-site Screening withNurse TriageUtilize our facial temperaturescanners for an autonomous,efficient way to screen youremployees. Employees will standin front of the electronic scanner,the temperature will be read, andthe screen will flash green if thetemperature is under 100F or redif it is over.Our staff of medical professionalsand registered nurses will comeon-site to screen your employeesfor risk factors, includingtemperatures.
