WESTCHESTER COMMUNITY COLLEGE TEST OF ESSENTIAL ACADEMICSKILLS (TEAS) REGISTRATION FORM/NURSING REGISTRATION FORMPLEASE READ THE ENTIRE PACKET CAREFULLY!Your application for admission to the Spring 2022 Nursing Program at Westchester CommunityCollege will only be considered upon receipt of the following: *The completed TEAS Registration Form(last page)*Official High School Transcript with graduation date or High School Transcript (with no graduationdate) with High School Diploma or GED Certificate and Scores, providing clear evidence of graduationor completion of requirements for certification.*Official College Transcripts if you have taken courses at another college and not a student at WCC.Transcripts should be sent directly to WCC’s Office of the Registrar’s for evaluation.*A printout of your Course History from your MyWCC Student Portal. As an accepted WCC student,your Student Portal will show past, current and transfer coursework. The candidate should submit theCourse History in lieu of the college transcript only if the college transcript has been officiallyevaluated and posted in the candidates Student Portal.*A 125.00 non-refundable testing fee -money order only, made payable to WCC FSA F.U.F. Account#3.Please Note: All NON-W.C.C. students must file an admissions application NO EARLIER THAN SEPT 1AND NO LATER THAN SEPT 16 (POSTMARKED). The candidate must be an accepted student toWestchester Community College before you submit the attached TEAS Registration Form.Very Important: Your application must have the required documentation. Science:All candidates must have High School Biology and Chemistry or the collegeequivalent completed prior to completing a TEAS Registration form and testingfor the TEAS. Candidates cannot be enrolled in either biology and/or chemistryduring the same semester as the TEAS Examination.All developmental coursework must be completed prior to the start of theprogram. Candidates must be college level math ready to begin the program.Candidates CANNOT sit for the TEAS exam if they have not completed alldevelopmental course work.Please Note: Anatomy and Physiology and or Microbiology does not replace theprerequisite High School or College Biology.Please Note: Although Anatomy and Physiology 1 and 2 are no longer prerequisites, they still have a Five-Year limit. If either course has been completedprior to the start of the program, it cannot be transferred to WCC’s program if itis more than five years. Students that have not taken Anatomy and Physiology 1and 2 are highly discouraged from taking the TEAS. Although Microbiology is nota prerequisite, it also has a Five-Year limit.THE FOLLOWING DOCUMENTS ARE REQUIRED WITH THE SUBMISSIONOF THE TEAS REGISTRATION FORM:*HIGH SCHOOL TRANSCRIPTS:An official High School transcript (should be on file with the Office of Admissions) with agraduation date is required by the State of New York. If the transcript does not have agraduation date, a High School Diploma must accompany the transcript. If either document isnot on file with the Office of Admissions, the student must submit an official copy at the timethey submit their TEAS application. For the purpose of the TEAS registration, a copy of thetranscript is needed.*COLLEGE TRANSCRIPTS:A PRINT OUT OF YOUR COURSE HISTORY FROM YOUR MyWCC STUDENT PORTAL.Please note: You are required to send official college transcripts to the Registrar’s Office forevaluation of transfer credit. Once evaluated, transfer credits are posted on the student’sportal. The student will then download the Course History from their student portal andsubmit it with other required documents. This process must be done early to meet the
