Financial Agreement for CCDF Clients (PTQ 9.9)Go Bananas! Childcare proudly accepts payment via the Child Care Development Fund (CCDF) to helpensure all children have access to quality childcare. The CCDF payment will be applied towards thenormal childcare fee. Any remaining balance, is YOUR responsibility. The following document outlinesour complete financial agreement.I, , the legal guardian ofagrees to the following:Normal Fee:The normal weekly fee for childcare services is . The daily rate is . Yourportion must be paid in advance on the preceding Friday before each week. Your portion includesANY amount not covered under the CCDF reimbursement. This includes, but is not limited to; theoverage, the co-pay, provider paid time off, additional fees, etc. See below for details.New Client Deposit:A deposit equal to the first week’s payment must be paid immediately to reserve your spot. The depositreserves your spot up to two weeks from the date of deposit or the specific date noted in additionalcomments. If you choose not to attend Go Banana’s Childcare, your deposit will be forfeited. If youattend Go Banana’s Childcare, your deposit will be applied towards your last week’s payment. If anoverage exists, you will be refunded that portion after all CCDF funds have been received.Overage:An overage exists when the CCDF reimbursement rate is less that the amount charged by the provider. Itis your responsibility to pay the overage in advance on the preceding Friday before each week.The current weekly overage amount according to the voucher and provider PTQ rating is .Co-Pay:The co-pay is the dollar amount you are responsible for paying directly to the provider. This amount islisted on your voucher. It is your responsibility to pay the co-pay in advance on the preceding Fridaybefore each week. Providers are required to report any missed payments to the local Intake Agent.The current weekly co-pay amount according to the voucher is .Payment Methods:Process payment before 5:00pm on Friday through one of the following payment methods: CashCheck: Payable to Valarie Solomon.*Google Pay: Send to valarieannsolomon@gmail.com. (Free to setup and use.)*Zelle: Send to gobananaschildcare@hotmail.com. (Free to setup and use.)Online Credit Card Payment: Pay through square at www.gobananaschildcare.com.* To prevent payment delays, a debit card must be linked to Google Pay or Zelle accounts.Additional Fees:The following additional fees are not covered by CCDF reimbursement. You will be responsible forpayment as outlined below.

Failure to Pay On Time Fee:Childcare fees are due in advance on the preceding Friday before each week. If you do not make yourpayment on Friday a 15 daily late fee WILL be charged. If the childcare payment is not paid in full(including the late fee) by Monday morning normal childcare cannot resume. Childcare cannot beginagain until all payments have been paid in full. Failure to pay may result in termination of childcareservices. If childcare services are terminated, you will be responsible for all unpaid childcare fees, latefees, supply fees, and your two-week notice period. If unpaid fees are not paid in full, collection actionsmay be taken at your expense.Late Arrival Fee:The normal operating hours of Go Bananas! Childcare is 7:00am to 5:45pm Monday through Thursdayand 7:00am to 5:15pm on Friday of each week. A 15-minute grace period for pickup is provided. If youdo not pick up your child by 6:00pm you will be charged a late fee. The late fee is 1 per minute with aminimum late fee of 5 after 6:00pm. Payment of the late fee is expected immediately. Normal childcarecannot resume until this has been paid. You must contact Go Bananas! Childcare immediately if you willnot be able to pick up your child by 6:00pm. Please note that there will not be a grace period on Friday.The late fee begins at 5:15pm.NSF Checks:If a check is returned for non-sufficient funds a 25 fee will be charged. Childcare will be halted until theamount of the check, the NSF fee, and the failure to pay on time fee has been reimbursed in cash. Allfuture payments will be required to be made in cash.Supply Fee:Go Bananas! Childcare is excited to offer the Handwriting Without Tears-Get Set for School PreschoolProgram and the Pocket of Preschool Curriculum. These curriculums come at a considerable cost andrequire additional ongoing printing and material cost. In addition, I offer many learning/interest centers,personalized learning materials, and art supplies. In order to help cover the cost of materials, a supply feeis required for ALL children enrolled in full-time or part-time childcare. The 25 supply fee per child isdue upon *enrollment and on July 1st of each subsequent year. A 5 late fee may be charged for any feenot received by the due date.*Include the supply fee with the 1st regular payment.Rate Increases:Go Bananas! Childcare reserves the right to review rates periodically.Damaged/Broken Items:Go Bananas! Childcare understands that toys and equipment experience normal wear and tear and mustbe replaced with time. Replacements of these items are included in my normal operating budget.However, damages that children cause beyond the scope of reasonable normal wear and tear will not betolerated. All children must respect my property and home while they are in my care. If your child’sbehavior causes damage to my property or belongings, you may be asked to pay reasonablereplacement/repair costs on your next scheduled payment. If the destructive behavior continues, yourchild may be terminated from Go Bananas! Childcare.

Provider Time Off:Alternate Care/Substitutes:Go Bananas! Childcare does not provide alternate care or substitutes in the event of my absence or yourchild’s. You are responsible for finding alternate care for your children when they are not able to attendmy daycare due to my time off/closure or your child’s own illness.Provider Holidays:Go Bananas! Childcare is closed all major holidays. The following are observed holidays: New Year’s Day (*Paid by CCDF)Friday Before Memorial DayMemorial Day (*Paid by CCDF)July 4th (*Paid by CCDF)Labor Day (*Paid by CCDF)Halloween Day – Early Closure at 5:00pmThanksgiving Day (*Paid by CCDF)Day after ThanksgivingRosh Hashanah (Dates vary each year; an annual calendar will be provided.)Yom Kippur (Dates vary each year; an annual calendar will be provided.)Christmas EveChristmas Day (*Paid by CCDF)Please arrange for back-up care if needed. These are paid holidays and your fee will not reduce duringdaycare closure. If the holiday falls on a weekend, I will follow federal guidelines to determine the date Iwill close. These dates will be posted on the annual time off calendar.CCDF will reimburse the 6 holidays *noted above. You will be responsible for your daily rate for all otherholiday closures. This fee must be paid in advance on the Friday before the holiday.Provider Vacation Days:Each year I will take off up to 15 days of unpaid vacation time. I will provide a calendar at the beginningof each year with the dates Go Bananas! Childcare will be closed. These dates may be taken in weekly ordaily increments. The weekly rate will be reduced or prorated for these dates. You will be responsiblefor back-up childcare. Holidays do not count towards vacation time. If a holiday falls during my vacationperiod, that time will continue to be paid.Provider Personal/Sick Days:Go Bananas! Childcare will be closed for up to 6 days each year for personal/sick time. An annualcalendar will be provided and any scheduled personal dates (ex. doctor appointments) will be posted asquickly as possible. In the event that an unexpected emergency occurs, or I am too ill to care for yourchild, Go Bananas! Childcare will be closed. You will be contacted ASAP via text message or a phone call tothe contact number provided. You will be responsible for back-up childcare. The normal weekly rate willnot be reduced or prorated for these dates. In the rare event that more time off is required, you will notbe charged. Your fee will be pro-rated.Provider personal/sick days are not covered by CCDF reimbursement. You will be responsible to payyour daily rate for all personal/sick days. If the personal/sick day is scheduled, you will pay your dailyrate in advance on the Friday before the personal/sick date. Unscheduled personal/sick days must bepaid the following Friday.

Provider Bereavement:Go Banana’s Childcare will be closed for up to 3 days in the event of a death of an immediate familymember. Immediate family includes a spouse, child, father, father-in law, mother, mother-in law, son-inlaw, daughter-in-law, brother, sister, stepfather, stepmother, stepbrother, stepsister, stepson,stepdaughter, grandchild, grandmother, grandfather, grandmother-in-law, grandfather-in-law. This ispaid time off. The normal weekly rate will not be reduced or prorated for these dates. You areresponsible for back-up childcare.Provider bereavement days are not covered by CCDF reimbursement. You will be responsible to payyour daily rate for all bereavement days. Your daily rate must be paid on the Friday before thebereavement days or on the Friday following notification.Provider Jury Duty:Go Banana’s Childcare may be closed in the event of a jury summons. Jury duty is a civic responsibilityand a requirement by law. Childcare providers are not exempt from jury service in the state of Indiana. IfI receive a jury summons, I will be required to call the court each day after 5 (for a week) to see if I needto report for service the next business day. I will report my findings on my Facebook page. If I amrequired to report for service, you will be responsible for backup childcare. Any time missed due to juryduty is paid time off. Your weekly fee will not be reduced or prorated.Provider jury duty days are not covered by CCDF reimbursement. You will be responsible to pay yourdaily rate for all jury duty days. Your daily rate must be paid on the Friday following jury duty service.Family Time Off:Please remember as a licensed provider I am only allowed to have a limited number of children in mycare. In order to guarantee a consistent income, I must remain full at all times. When you joined GoBananas! Childcare, you were reserving a spot. I cannot fill spots easily for short term absences;therefore, you must pay your weekly fee even if your child does not attend. This is including but notlimited to, vacation time, sick time, personal time, maternity leave, medical leave, job loss, and summerbreaks.The CCDF reimbursement program allows you to use up to 20 days for your child’s own absence. If youdo not use eligible days for time off, you must pay your daily rate for all days missed by the Fridayfollowing your child’s absence.Termination from Go Bananas! Childcare:If you decide to remove your child from Go Bananas! Childcare, a two-week written notice is required.You are required to pay for these two weeks even if your child does not attend.CCDF Reporting Requirements:It is your responsibility to follow all CCDF reporting requirements. This includes clocking in and outevery day on the CCDF reporting device, and entering in personal days immediately following yourreturn. You also need to review your activity online weekly and make sure no activity was missed.Failure to do so could result in delayed or missed payments. These payments will become yourresponsibility. Failure to consistently report accurate time and attendance could result in terminationfrom Go Bananas! Childcare.
