Key product informationDeductiblesAetna Summit 1750You must pay a standard annual excess amount of USD2,000 / SGD 2500 for any one or more claims in each planyear. This is the total excess that you will pay for any oneor more claims in the plan year. A higher or lower annualexcess may have been selected for you by your plansponsor, this will be shown on your Benefits schedule,section 26.1.An additional deductible may apply for treatment orservices received outside of the network, see section 26.8in the Benefits schedule.Aetna Summit 2500, 4000 and 5000You must pay a standard coinsurance of 10% up to amaximum of USD 2,000 / SGD 2500. This coinsurance isapplied to each claim. The maximum applies to any one ormore claims you make in the plan year. A higher or lowercoinsurance may have been selected for you by your plansponsor, this will be shown on your Benefits schedule,section 26.2.An additional deductible may apply for treatment orservices received outside of the network, see section 26.8in the Benefits schedule.Duration of the planThe plan is a 12-month policy starting from a date agreedbetween us and the plan sponsor or any subsequent planrenewal date, as applicable. Your start date will be advisedto you by your plan administrator once we have agreed tocover you.Area of coverThe available Areas of cover for people based in Singaporeare shown below. For more detail please refer to yourHandbook. Area 4 includes cover for all the locations in Area 5, plusAustralia, Qatar, New Zealand, Singapore and the UAE Area 3 includes cover for all the locations in Area 4, plusChina Area 2 provides worldwide cover excluding the US Area 1 provides worldwide cover including the USAll plans provide some cover for emergency medicaltreatment outside the Area of cover. The benefit limits foreach plan are shown in section 25 in the Benefits schedule.Please note that Area 1 cover is only available to non-UScitizens living outside the US. Only Aetna Pioneer 5000 and5000 are available with Area 1.If you require additional cover in the USA for trips notexceeding 180 days, please consider extending your plan toinclude the Aetna Travel plan.PremiumsThe plan administrator must choose how often the Aetnasummit plan premiums are paid from the payment optionsavailable. The plan administrator is responsible for payingall premiums.Premiums may be affected by material changes such asmoving to another country or changing occupation. Youmust disclose any material facts to us. Premiums may alsochange as a result of adding or removing members.Key product provisionsThe following are major provisions found in the plan. Thisis only a brief summary and you are advised to refer to theactual terms and conditions found in the Handbook. Pleaseconsult your insurance advisor or us should you needfurther explanation.EligibilityThe Aetna Summit plans and add-on plans are availableto people of most nationalities. We cannot cover peoplesubject to sanctions. Our plans are not available to citizensof the United States (US) who reside in the US. Pleasecontact us if you need further information. Plans may notmeet specific visa requirements. Cover may also be illegalunder local laws. It is the plan sponsor’s responsibility toensure that any plans chosen meet your needs.You must have continuous membership under the AetnaSummit plan and any add-on plans.If you will be aged over 65 at your plan start date,you will be subject to medical underwriting and mustanswer some medical questions for us to consider youreligibility for cover. Once accepted by us, no furthermedical underwriting will be applied whilst you remain acontinuous, eligible, insured member.Waiting periodsWaiting periods begin on the date that a benefit was firstintroduced on your plan or your date of joining, whicheveris later. For example, a waiting period of 182 days may applyto the routine and major restorative dental benefit. Waitingperiods may apply to other benefits, please see yourBenefits schedule for more information.Premium guaranteeDuring a plan year premiums will only change in linewith any changes to the plan. For instance, if you changecountry of residence or occupation premiums may beadjusted to reflect the change in risk. Before each renewaldate premiums for the next plan year are calculated inline with our rates at that time. We will send a renewalquotation to the plan administrator with the new premium.Many factors, including medical inflation and the age ofmembers on cover, influence the premiums we charge.11

TransfersIf a new person wants to transfer cover from another insurerto apply for CTT underwriting terms with us, a Groupmember Application for CTT must be filled in, and we willneed an original certificate of insurance from their previousinsurer, which shows: their original start date with that insurer, their underwriting terms, and any special terms that may have applied.If there is a break in cover between the end date of theprevious insurance plan and the application to us, we willnot offer a transfer of previous underwriting terms.RenewalWith our agreement, the plan administrator may renewthe plan each year. The plan administrator must tell us allmaterial facts about the plan sponsor and all membersbefore the plan renewal date.If the planholder wants to renew the plan, they must tell usin writing before the plan renewal date. We may change thedefinitions, benefits, plan terms, conditions and exclusionsthat apply to the plan. These will be sent to the planadministrator together with the renewal quotation at leastsix weeks before the plan renewal date.Renewal premiums must be paid on or before the planrenewal date. If premiums are paid by instalments, the firstpayment must be paid on or before the plan renewal date.A child will no longer be eligible as a dependant under theplan at the next plan renewal date if any one or more of thefollowing apply: they marry, they are not in continuous full-time education and theyare 18 to 26, or they reach the age of 26.With our agreement they can apply to have their own planby completing an Application. As long as there is no breakin their insurance cover their date of joining will stay thesame. Their application will be governed by the definitions,benefits, plan terms, conditions and exclusions in forceat the time they move to their own plan together with anyspecial terms they accept as offered in their quotation.CancellationThe plan administrator must tell us all material facts beforewe accept an application, make changes to a plan or renewa plan. The plan administrator must tell us immediately inwriting about any change that affects information givenin connection with the application for cover under a plan,including information about you. If there is a change in riskthat the plan administrator has not told us about, yourcover may be cancelled, the plan may be cancelled, or anyrelated claim may be reduced or rejected.We will cancel a plan if payment is not received within 30days of the premium due date.12If your area of cover is Area 1 and you are a citizen of theUnited States of America (US), we will cancel your cover ifyou have spent more than 90 days in the US in any one planyear.If we receive new information that shows a claim we havealready approved is ineligible, no costs will be paid. If anycosts have already been paid, we will recover the costsand no further costs will be paid. Any approval we havegiven during the preauthorisation process may also bewithdrawn. After we have given notice that you must repayany costs, this must be done within 14 days, failing which,we reserve the right to cancel your cover under the plan,subject to applicable laws.If the plan administrator wants to cancel a plan, they mustconfirm this in writing. If the Aetna Summit plan is cancelled,any add-on plans will also be cancelled.If coverage provided by this policy violates or will violate anyUnited States (US), United Nations (UN), European Union(EU) or other applicable economic or trade sanctions, thecoverage is immediately considered invalid. For example,Aetna companies cannot make payments or reimbursefor health care or other claims or services if it violatesa financial sanction regulation.
