Phone (850) talog and PoliciesStudents are expected to be familiar with the informationpresented in the enrollment agreement, catalog and anyaddenda to the catalog. This catalog serves as a handbook forthe student. By enrolling in Dental Assistant Pioneers Academy,the student agrees to abide by all policies of the institution. If atany time there is conflict between the enrollment agreement andthe catalog, the enrollment agreement is the binding contract.School Facilities7

Dental Assistant Pioneers Academy is located at 5107 Andrus Ave. Orlando,Fl. 32804. The total commercial office is 1686 sq. FT.The one story mid-century modern building has a modern reception/seatingarea located in the front of the building immediately entering the front thefront entrance. Attached is a hallway that leads to the main classroom thatcontains all the student desk, dry erase board, AV learning tools andprojector. The classroom space is approximately 800 sq. FT. to the right is adoorway that leads into the dental laboratory. In this space you will find allthe dental chairs and clinical equipment which include sterilizationequipment and x-ray machines. This room measures approximately 500 sq.FT. To the left of the dental laboratory is a doorway that leads into thecomputer lab. This space contains 5 computer stations with functioningcomputers and a conference table. The computer lab measuresapproximately 386 sq. FT.The dental laboratory houses a variety of different dental equipment capableof performing all expanded function/radiology duties. There are lab tables,laptops with internet access for student use . lab also equipped with amonitor, and audiovisual screen display.Hours of OperationThe hours of operation are as follows:Monday through Friday: 8:00 am to 8:00 pmSaturday- Sunday: 8am – 6pmProgram Advisory CommitteeDental Assistant Pioneers Academy holds quarterly meetings withlocal dental professionals to retrieve up to date information and ideasto help ensure students are trained with the most accurate andcurrent techniques.8

Admissions InformationAdmissions Requirements Minimum age of eighteen years old. Valid high school diploma or GED certification showingcompletion. Valid driver’s license. Completion of a background check. Completion of an interview. If prior dental experience: a letter from a board certified Floridadentist on office letterhead verifying dates of employment andduties. There are no physical limitations and the facility is handicapaccessible.Health Screening and ImmunizationsStudents are subject to obtaining Hepatitis vaccination series. Itis the responsibility of each student to complete the entire seriesbefore entering any dental clinic rotation. The cost incurred is theresponsibility of the student. Student is to show proof ofcompleted series before entering any dental rotation.Cardio Pulmonary Resuscitation (CPR) Training9

Students attending clinical rotations must maintain valid CPRcertification. Satisfactory completion of the American HeartAssociation Basic Life Support for Healthcare Providers courseis required. If at any time during the program the student’s CPRcard is near expiration, the student must re-train and provide theschool with proof of satisfactory completion of a new course. Thestudent should submit evidence of CPR certification prior tobeginning any clinical training. The CPR Course is included inthe Entry Level Expanded Functions/ Radiology and is coveredwith tuition. If student misses the class, student will have to payan outside facility to complete CPR certification. There will be nosecond class offeredEntry Level Expanded Function/ RadiologyOverviewDental Assistant Pioneers Academy offers an Entry LevelExpanded Functions / Radiology.Applicants are accepted on a first come basis onceadmissions procedures are completed and an applicant isdetermined to be qualified. Applicants are notified of theiracceptance by email, an official hardcopy letter, and a phonecall.Prior to externship, students are required to complete astatement of health which must be signed by a healthcarepractitioner indicating that the student is in good health.Tuberculosis screening must be done no sooner than 3months to the start of externship and immunizations must beup-to-date. The cost of this process is paid by the student.The following items are required to complete the applicationprocess:10

If the class isdelayed, the students will remain scheduled into the next startdate. If the class is cancelled, all students enrolled in that classwill be rescheduled into the next class being offered. The tuitionrates will remain unchanged. A student does have the right torequest a refund for that class if they choose to not continueenrollment in that class. In the event classes are permanentlycancelled by the school, the student will be entitled to a refund ofany tuition monies already paid for their cancelled classes.11

General InformationStatement of NondiscriminationDental Assistant Pioneers Academy admits students withoutregard to race, gender, sexual orientation, religion, creed,color, national origin, ancestry, marital status, age, disability,or any other factor prohibited by law.Student Rights and ResponsibilitiesAll students have the right to know: The school’s licensing agencies. The school’s programs, facilities, and faculty. The cost of attending school. The payment programs available if any. How to submit appeals under various school policies. The school’s method of determining satisfactoryacademic. Student completion and retention rates, placementrates, and student body diversification.All students have the following responsibilities: To review and consider all aspects of the schoolprogram before enrolling. To provide additional documentation, verification,correction, etc. as requested by the school or agency. To read, understand, and keep copies of all forms12

received. To notify the school of any change in their financialcircumstances. To understand the school’s satisfactory academicprogress policy. To understand the school’s refund policies. To sign all required certification statements.New Student OrientationOrientation is typically held prior to the first day of class orduring the first week. During orientation the student will havethe opportunity to learn about the instructional and supportpersonnel associated with the school and to review schooland academic policies. It is important that students areaware of all of the resources that will be available to assistthem throughout the educational process.Student Dress CodeThe institution maintains a professional dress code for allstudents for reasons of safety as well as appearance. Thedress code is designed so that students convey a messageof professionalism, discipline and pride. You are to wear thefollowing whenever you are participating in school activitiesunless notified otherwise by your instructor or programdirector. This includes all times when you are coming toschool for scheduled laboratory classes, unscheduled timeswhen you are coming for extra hands-on practice either withor without instructor guidance, and all clinic courses.13

Scrub top and bottom. White turtle neck or white T-shirtmay be worn underneath the scrub top if morecomfortable, but it must be tucked into the scrubbottoms and at no time is to be seen hanging beneaththe scrub top. White socks. White shoes that tie with closed toes and closed heels(Shoes may have a logo such as the Nike swish thatare in another color but the shoe is to be white, notwhite with colored stripes or contrasting colors. When indoubt, check new shoe purchases with your instructorprior to wearing so they can be returned unworn if theyare inappropriate.) Hair must be no longer than neck length or is to be tiedback. Only natural looking hair colors are acceptable. Single pierced earring in ears only. Earrings are to bevery small hoop earring style or stud style, preferablygold or silver appearance Nails are to be no longer than 1 4 inch past fingertip,preferably with no polish or with clear polish only. Noartificial nails are allowed. Jewelry is limited to wedding bands or single ring only. A watch is required at all times. White lab coat will be provided by the school for clinicaltraining.Student Conduct14

All students are expected to respect the rights of others andare held responsible for conforming to the laws of thenational, state and local governments and for conductingthemselves in a manner consistent with the best interests ofthe institution and the student body.Dental Assistant Pioneers Academy reserves the right todismiss any student for any reason including but not limitedto: Failure to maintain satisfactory academic progress. Failure to pay school fees or tuition in a timely manner. Disruptive behavior. Posing a danger to the health or welfare of students,faculty or staff. Failure to comply with any published school rules orregulations. Use of profanity. Threatening behavior of any type. Violation of the school’s drug and alcohol policy. Theft of property.Student Appeal ProcedureDental Assistant Pioneers Academy tries its best toformulate and enforce school policies that support themission of the institution. At times, a student may be unableto meet the school’s policy as it is described due toextenuating circumstances. A student who wishes to appeal15

the school’s decision must submit their request in writingwithin three (3) business days of receiving notification of theschool’s decision. A student should explain all mitigatingcircumstances that may have existed. The school will reviewa student’s request and make a determination if the studentwill be reinstated to satisfactory status. The student will benotified in writing of the decision within 10 business days.The decision is final and may not be appealed.Grievance ProcedureIt is the hope of Dental Assistant Pioneers Academy staffand faculty, that all students will have an enrichingexperience while completing their program, however if astudent has a concern or grievance, they are encouraged toresolve the grievance whenever possible, in the followingmanner:1.A complaint or grievance involving an individual, fellowstudent, faculty member, or other staff member shouldbe taken to that individual first in an attempt atresolution of the issue.2.If a complaint involves a policy, procedure, or financialissue, the student should address the concern to theperson tasked with enforcing the school policy.3.Should the complaint not be resolved after attemptingto address the concern, the student should make anappointment with the administrative office or write aletter to that individual outlining the points of concern,doing so within three (3) business days of the offendingoccurrence. The student should receive a response tothe letter within three (3) business days.4.If the complaint is still unresolved after working with the16

administrative office, then the student should provide awritten letter (or e-mail) grievance petition indicating theconcern to the owner for the appropriate campus withinthree (3) business days of the office’s response. Thepetition needs to describe the problem and indicate theperceived deficiency of the school, or school personnel,and request a specific remedy to the unresolvedsituation. The owner should respond to the letter, or email, within three (3) business days of receipt.Electronics and Cellphone PolicyAll electronic devices: iPods, cell phones, pagers, etc. are toremain in the “OFF” position during class time. Cell phonesand pagers may be checked during breaks. If you have aspecial circumstance that requires that you be reachedimmediately, please see your instructor.Drug and Alcohol PolicyThe school prohibits the possession, use, or distribution ofillegal drugs and/or alcohol on school premises or at anyschool event. Additional details of the health risks, legalpenalties, sanctions and information on counseling andtreatment can be requested from the program director orowner.Inclement WeatherDental Assistant Pioneers Academy will follow the directivesof the public school system regarding school cancellationsand late opening as a result of inclement weather unlessotherwise directed by the owner.Smoking17

There is no smoking inside the school, including therestrooms and break areas. Please smoke only indesignated smoking areas. Properly dispose of yourcigarettes and matches by not leaving them scattered on theground and ensure they are properly extinguished.Communicable Disease PolicyShould a student contract a communicable disease, thatinformation must be provided immediately to DentalAssistant Pioneers Academy. Depending upon thecircumstances, the student may be administrativelywithdrawn from certain courses, particularly clinic/labcourses, or externship and then return upon resolution of thedisease.Sexual Harassment PolicyThe school is committed to treating its students andemployees with dignity and respect. All students andemployees have the right to be free from unwanted verbal,physical or sexual advances. Sexual harassment violatesboth federal and state laws.If it is determined that a student or employee appears tohave been harassed, there will be appropriate disciplinaryaction taken. If a student is involved, there may be expulsionfrom the institution and if an employee is involved, it mayinclude termination from employment. If you experience anysuspected harassment, it should immediately be reported tothe owner of the academy.Rights to Privacy and Access to Records and18

Procedure to Inspect Educational RecordsThe Family Educational Rights and Privacy Act (FERPA)affords students certain rights with respect to theireducational records.1.Each student enrolled at Dental Assistant PioneersAcademy shall have the right to inspect and review thecontents of his/her educational records, includinggrades, records for attendance, and other information.Parents are not entitled to inspect and review financialrecords of their children. Parental access to a student’srecords may be allowed without prior consent if thestudent is a dependent as defined in Section 152 of theInternal Revenue Code of 1986.2.A student’s educational records are defined as files,materials, or documents including those in electronicformat, that contain information directly related to thestudent and are maintained by the institution, except asprovided by law. Access to a student’s educationalrecords is afforded to school officials who have alegitimate educational interest in the records. A schoolofficial is defined as a person employed or engaged bythe school in a administrative, supervisory, academic orsupport staff position (including law enforcement unitand health staff); a person or company (including itsemployees) with whom the school has contracted (suchas an attorney, auditor, consultant or collection agent);a trustee serving on a governing board; or a personassisting another school official in performing his or hertask. A school official has a legitimate educationalinterest if the official needs to review an educationalrecord to fulfill his or her professional responsibility or19

commitment to the school.3.Students may request a review of their educationalrecords by submitting a written request to theadministrative office. The review will be allowed duringregular school hours under appropriate supervision.4.Students may request that the institution amend any oftheir educational records, if they believe the recordcontains information that is inaccurate, misleading or inviolation of their privacy rights. The request must bemade in writing and delivered to the administrativeoffice, with the reason for the request. Grades andcourse evaluations can be challenged only on thegrounds that they are improperly recorded. Theinstructor or the administrator involved will review therequest, if necessary meet with the student, and thendetermine whether to retain, change, or delete thedisputed data. If the student requests a further review,Dental Assistant Pioneers Academy will conduct ahearing, giving the student an opportunity to presentevidence relevant to the disputed issues. The studentwill be notified of the final decision. Copies of studentchallenges and any written documentation regardingthe contents of the student’s records will be retained aspart of the student’s permanent record.5.Directory information is information on a student thatthe school may release to third parties without theconsent of the student. Dental Assistant PioneersAcademy has defined directory information as thestudent’s name, address(es), telephone number(s), email address(es), birth date and place, program ofstudy, dates of enrollment, attendance, and graduation,grades, honors and awards as well as credentials20

awarded. If student does not want his or her directoryinformation to be release to third parties without thestudent’s consent, the student must present such arequest in writing to the administrative office.6.The written consent of the student is required beforepersonally identifiable information from educationalrecords of that student may be released to the thirdparty, except for those disclosures referenced above,disclosures to accrediting commissions andgovernment agencies, and other disclosures permittedby law.7.A student who believes that Dental Assistant PioneersAcademy has violated his or her rights concerning therelease of or access to his or her records may file acomplaint with the U.S. Department of Education.TranscriptsA transcript of final grades will be retained with the student’spermanent record. An official transcript is provided to thestudent upon completion of his/her program at no charge.Requests for additional transcripts may be made in writing tothe administrative office. There is a charge of 5.00 for eachtranscript. A student is not eligible to receive a diploma ortranscript if he or she is not in good financial standing withthe school.Transferability of Credit to Other SchoolsThe Course at Dental Assistant Pioneers Academy isoriented towards job preparation, not continuing education.Transferability of credit is at the discretion of the acceptinginstitution, and it is the student’s responsibility to confirmwhether or not credits will be accepted by another institution21

of the student’s choice. Dental Assistant Pioneers Academydoes not imply, promise or guarantee that any credits earnedat Dental Assistant Pioneers Academy will be transferable oraccepted by any other institution. You should assume thatcredits earned at Dental Assistant Pioneers Academy arenot transferable to other institutions unless you havepersonally spoken to an authorized representative of theinstitution to which you wish to transfer any credits andobtained assurances that it will accept Dental AssistantPioneers Academy credits. Each institution has its ownpolicies governing the acceptance of credit from otherinstitutions. Students should inquire as to policies on credittransfer at any institution to which they seek admission.Reasonable Accommodations Policy –Individuals with DisabilitiesSECTION 504 OF THE REHABILITATION ACTDental Assistant Pioneers Academy does not discriminateon the basis of disability in its programs and activities inviolation of Section 504 of the Rehabilitation Act, asamended and the implementing regulations (Section 504).Programs and activities subject to the nondiscriminationprovisions of Section 504 include admissions andrecruitment, treatment of students, academic adjustments(academic requirements, course examinations, and auxiliaryaids), financial and employment assistance to students, andnonacademic services. The person responsible forcoordinating our efforts to comply with Section 504 theowner; who is also the person responsible for (1) explainingto applicants and students how to obtain information aboutthe process to secure academic adjustments and (2)receiving requests for academic adjustments.22

Financial InformationTuition and FeesStudents may pay by cash, check, money order or creditcard for educational costs. Tuition is due based on the termsin the student enrollment agreement and any supplementalpayment plan agreed to during the interview. Students whoare unable to make timely tuition payments must speak withthe owner to continue in their program of study. Unexcusedlate payments may be cause for suspension or dismissal.Employer SponsorshipApplicants currently employed are urged to look into anytuition reimbursement benefits offered by their employers.Tuition reimbursement is paid by the company directly to theemployee, not to the school. Most companies require theemployee to pay for their program and then reimburse theemployee after completion of their program of study.Payments due to the school are not dependent on tuitionreimbursement.Cost of AttendanceEntry Level Expanded Functions /Radiology Tuition Cost: 2,450 Registration fee: 150 (Non- Refundable)CANCELLATION AND REFUND POLICY23

Should a student’s enrollment be terminated or cancelled for any reason, all refunds will be madeaccording to the following refund schedule:1. Cancellation can be made in person, by electronic mail, by Certified Mail or by termination.2. All monies will be refunded if the school does not accept the applicant or if the student cancelswithin three (3) business days after signing the enrollment agreement and making initial payment.3. Cancellation after the third (3rd) Business Day, but before the first class, results in a refund ofall monies paid, with the exception of the registration fee (not to exceed 150.00).4. Cancellation after attendance has begun, through after completing more than 40% completionof the program, will result in a Pro Rata refund computed on the number of hours completed tothe total program hours.5. Cancellation after completing 40% of the program will result in no refund.6. Termination Date: In calculating the refund due to a student, the last date of actual attendanceby the student is used in the calculation unless earlier written notice is received.7. Refunds will be made within 30 days of termination of students’ enrollment or receipt ofCancellation Notice from student.Program Termination or Cancellation PolicyIn the event classes are permanently cancelled by theschool, the student will be entitled to a refund of any tuitionmonies already paid for their cancelled classes.Enrollment CancellationA student may cancel his enrollment within three businessdays of signing the enrollment agreement. In the event that astudent cancels his/her enrollment, all tuition and fees paidwill be refunded. If a student cancels his enrollment afterthree business days, but prior to the first day of class, thestudent will receive a refund of all tuition and fees paid,except for the registration fee. If a student cancels hisenrollment after three business days and after classes havebegun, the institutional refund policy will be applied.24

Students are asked to submit requests for cancellation inwriting to the school. If the school rejects a studentapplication for enrollment, all monies paid will be refunded.Insufficient Funds PolicyThere is a 25 fee for any checks returned for insufficientfunds.Academic InformationGraduation Requirements Completion of all required lecture, laboratory, andexternship clock hours as required by the course. A final percentage of 80% or higher in all courses. All fees have been
