Dental Health Home Coordinator – facilitates all the dentalactivity within primary care clinic. Providing oral health screening, education toparents/families and pregnant mother and helping familiesnavigate to dental home following patient care to completion Providing inter-professional oral health education toincrease knowledge of oral health diseases. Helping patients to access care/services focusing on patientbarriers to care, cultural issues

Goal is To have “Healthy Smiles for Healthy Families” to prevent,maintain and/or eliminate oral diseases that results from dentalcaries, periodontal disease and other barriers to care.Focus is commitment to promote overall health for our patientsfamilies including:1. Integration and collaboration with primary care clinic and other communitypartners.2. Identifying patients barriers to care or other cultural issues to followingpatient care to completion.3. Education to increase awareness of early prevention (to decrease earlychildhood dental caries), better oral hygiene and healthy eating,4. Oral health maintenance enabling families to live vibrant and healthy lives witha healthy smiles.
