Introduction to the Financial Aid OfficeThe Duke School of Medicine Office serves the student’s enrolled in the Doctor of Medicine, Doctor of PhysicalTherapy, Doctor of Occupational Therapy, Master of Management in Clinical Informatics, Master of BiomedicalScience, Master of Biostatistics, Master of Health Science in Clinical Leadership, Master of Health Science clinicalResearch, Master of Health Science Pathologist Assistant, Master Health Science Physician Assistant, OphthalmicTechnician Certificate Program, Certificate in Population Health Sciences Research, Master of Management ClinicalInformatics Certificate, and the Cardiac Ultrasound Certificate Program.The Financial Aid Office is located on the Third Floor of the Seeley Mudd Building. Office hours are from 7:30 am to4:30 pm Monday through Friday. The majority of our office is working remotely. Purpose and Philosophy of the Financial Aid OfficeThe Duke University School of Medicine makes financial assistance available to accepted students who due toeconomic circumstances could not otherwise attend the university. The Duke School of Medicine recognizes,however, the responsibility of the individual and the family to provide funds to achieve the objective of a medicaleducation. Thus, the school does not consider parents to have discharged the full financial obligation for thecontinuing education of their son or daughter upon the completion of the undergraduate degree. Financialassistance for medical students is available in the combined form of grants and loans, and all awards are made onthe basis of demonstrated need to eligible U. S. citizens. Financial Assistance for allied health students is availablethrough low interest educational loans. For federal loans the student must be a U. S. citizen.c.Policies and Procedures Development ResponsibilitiesThe Director of Financial Aid is responsible for establishing Institutional policy development surrounding the deliveryof financial assistance. Policy development adheres to federal and state laws and regulations as well as the missionof Duke University. Financial Aid policy is reported and approved by the Vice Dean of Medical Education whenappropriate.i. This permission will be obtained prior to any discussions or exchange of information.iii.Treatment of Correspondence/FormsA financial aid counselor is responsible for routing all administrative correspondence and processing of all aidapplication materials. If an appropriate recipient is not identifiable, the correspondence should be given to theDirector for evaluation and delegation.FAFSA applications are received electronically through PeopleSoft. Electronic copies are available on -line.Tax documents are retained in the Perceptive Content file.CSS Profile documentation is available through CSS Profile.Award letter notifications are noted within People Soft and can be accessed through DukeHub.Mail or email correspondences are scanned in put in the Perceptive Content student file.iv.8 Pag eConfidentiality of Student RecordsA student may request his information from his own financial aid file. This information may be copied for thestudent’s individual use but no other student may have access to someone else’s fi le. If a student calls on thetelephone to request information we request the student provide a written request that states their Unique ID orUpda te : 05/ 2022

EMPLID as verification. The request may be made via the student duke email account which will serve as anelectronic signature. All student information that is received but is not transferred to the file is destroyed. A copy ofthe request is kept on file.The School of Medicine Financial Aid Office follows the university FERPA standards.e. Records Management and RetentionInformation collected for a medical student’s financial aid application includes the FAFSA, CSS Profile, parental andstudent spouse if applicable, and student tax documents. Information for all Allied Health students includes theFAFSA and student tax documents only if the student is chosen for verification. The FAFSA application iselectronically downloaded and stored in People Soft. All tax documents received are scanned, linked to theindividual student and stored in Perceptive Content. Both People Soft and Perceptive Content require securityaccess and individual login by staff and administration. Perceptive Content will retain students that leave orgraduate in an archived folder within. Records will be purged manually by FAO administration based on federalregulations. FAFSA and CSS Profile data are stored within People Soft and are able to be accessed through thearchived data.All institutional loan notes are available at the Student Loan Office which is located at 2127 Campus Drive Annex. Allfederal Perkins loan note signatures reside at the Student Loan Office outside servicer ECSI.i. Safeguarding Electronic RecordsAll electronic documents (FAFSA) are housed in People Soft which is maintained by the SISS office. The SISSoffice follows university policy for maintaining and safeguarding electronic records. SISS sets up andmonitors system security such as passwords, access, user point tracking, and random audit surveys.f.Information Sharing and the Family Educational Rights and Privacy ActDuke University adheres a policy of compliance with the Family Educational Rights and Privacy Act. The policy (1)permits students to inspect their education records, (2) limits disclosure to others of personally-identifiableinformation from education records without students' prior written consent, and (3) provides students theopportunity to seek correction of their education records where appropriate.A copy of Duke Policy and Procedures can be found on the Duke University Registrar Office website.9 Pag eUpda te : 05/ 2022

III.FINANCIAL AID PROGRAMSThe Financial Aid Office participates in a variety of financial aid programs. Assistance may include scholarships, grants, andloans. Scholarships and grants are gift awards which do not have to be repaid unlike loan programs which require repayment.The type of aid awarded depends upon the student’s financial need and is generally a combination of gift and self-helpassistance for medical students, and self-help for graduate health professions students.a. Institutional Eligibility RequirementsAs a private nonprofit institution, Duke University has been authorized by the United States Secretary of Educationto participate in the financial aid programs authorized by Title IV or the Higher Education Act of 1965 as amended.A copy of the Program Participation Agreement (PPA) is kept by the Assistant Vice Provost & Director of financial Aidat the Duke Undergraduate Financial Aid Office. The Director tracks the expiration and coordinates the recertificationprocess. The Director of School of Medicine Financial Aid notifies the Director of UGFA of changes to location andany new programs.A copy of the Institutional Methodology policy can be provided upon request.i. Program EligibilityThe Program Participation Agreement between Duke University and the Department ofEducation entitles the Duke University School of Medicine Financial Aid Office to participatein the following federal programs:1.2.Federal Family Education Loan ProgramAlternative Loan Program.Student enrollment is monitored by the School of Medicine Registrars’ Office in conjunction with facultyand student advisory deans. The degree option leads to a Master of Health Sciences in ClinicalLeadership, a professional degree awarded by the School of Medicine at Duke University.Master in Health Science Clinical Research Program requires completion of 24 credits. This is a one-yearprogram broken into semesters. A candidate can take up to six years to complete requirements. The degreeoption leads to a Master of Health Sciences in Clinical Research, a professional degree awarded by theSchool of Medicine at Duke University.Master of Biostatistics Program enrollment is measured by credits. The academic year spans 12 months and runs July through June.Certificate of Population Health SciencesThe Population Health Sciences Research is a new non-degree program. Evaluation of New ProgramsThe evaluation of new programs is handled through the Office of the Registrar, Office of Curriculum, andDr. Buckley. For additional information regarding the evaluation of new programs, please refer to Office ofCurriculum.3. Administrative CapabilityThe School of Medicine Financial Aid office has the capability to administer the programs adequately.Funds are reviewed annually. Set up and changes for all loan programs are handled by the Duke UniversityStudent Loan Office. Audits are run by the Student Loan Office that audit awarding and disbursement. FAOStaff have query enrollment and academic progress on their students. Failure for a student to meet thecriteria for the program results in an adjustment. Security to award and adjust eligible funds to ourstudents is set up and monitored by the Duke SISS Office.4. ProvisionsDuke School of Medicine administers the Title IV programs in accordance with all applicable statutory andregulatory provisions. There are no special arrangements, agreements, or limitations into which it hasentered.5. AdministrationThe Director of Undergraduate Financial Aid is responsible for administering and coordinating theinstitution’s financial aid programs. Each School has a representative to administer the Title IV program.The Assistant Director is responsible for administering Title IV funds for School of Medicine students. TheFinancial Aid Officer is responsible for Physician Assistant Program, Doctor of Physical Therapy Program,the Pathologist’s Assistant Program, Ophthalmic Technician Program, Master in Clinical Informatics, Masterin Biomedical Science, Master in Clinical Leadership, Master in Clinical Research Programs, and the CardiacUltrasound Certificate program.Monthly Graduate and Professional meetings are held with the Director of Undergraduate Financial Aidand the Graduate Professional Schools. Information that impacts a student’s Title IV eligibility received iscommunicated during this meeting.6. Responsibilities of Institutional OfficesThe Student Loan Office is responsible for the set-up of Title IV loan funds. The Undergraduate FinancialAid Office is responsible for set up of all other Title IV funds in the People Soft system. Approval, eligibility,and awarding of Title IV funds for the School of Medicine are described above. The SISS Office isresponsible for disbursing funds to the student account. Submission of reports for all Title IV funds is doneby the Director of Undergraduate Financial Aid.7. Separation of DutiesIndividuals that have security access to award Title IV funds do not have access to disburse Title IV funds.The School of Medicine is responsible for the awarding Title IV funds to its students. The Student Loanoffice is responsible for administering the draw-down of Title IV funds. The SISS office disburses the fundsto the student account. 8. RecordsRecords are kept in the School of Medicine Financial Aid Office for all active students. Once the corrected FAFSA is received, Title IV funds will be awarded.11. Reviews and ProcessesCopies of all program reviews conducted by DOE, audits conducted by ED, internal audits, andany findings made in any criminal, civil, or administrative proceeding are kept in theUndergraduate Financial Aid Office. Duke is currently in good standing.12. Cohort Default RatesDuke’s Federal Family Education Loan (FFEL), Federal Direct Loan (Direct Loan), and/or FederalPerkins Loan cohort default rates do not exceed the thresholds defined by ED.
