INSTRUCTIONS FOR IMMIGRANT VISA APPLICANTSThis is an instruction packet listing all documents required for your immigrant visa interview. Any submission with missingdocuments will cause a delay in the processing of your visa application. Please note that at any time during this process, aconsular official may ask for additional information or documents, in order to determine your eligibility. All documents listedbelow should be prepared regardless of whether they were submitted to USCIS.PASSPORT: A signed passport should be valid for at least 8 months at the time of visa issuance. Any previous passportswith U.S. visas should also be submitted.PHOTOGRAPHS: Submit 2 photos taken within 6 months, with white background (5cm X 5cm sized). Please note thateyeglasses are not allowed in visa photos. Check photo requirements at https://travel.state.gov.CONFIRMATION WITH YOUR MAILING ADDRESS AND ‘UID’ NUMBER: Submit your Appointment Confirmation if you scheduled your appointment through the call center or at the U.S. VisaInformation Service website. Submit your Transaction Confirmation if your appointment was scheduled by the National Visa Center (NVC). You may print your Transaction Confirmation after you register your document delivery address through the U.S VisaInformation Service call center or at the website.CONFIRMATION PAGE OF DS-260: Please fill out and submit DS-260 (Online Immigrant Visa Application) on https://ceac.state.gov/IV before your medicalexamination, and bring the confirmation page to the hospital and to your interview. Be sure to enter biographicinformation as shown on your passport or travel document. IMPORTANT NOTICE: Please read the questions on your DS-260 carefully and answer accurately and completely. Thequestions below include (but are not limited to) examples that you may overlook. You must answer “Yes” if it applies toyou, no matter how long ago it happened. If you fail to answer one of these questions correctly, you may not beinterviewed on the scheduled date.- “Have you ever been in the U.S.?”- “Have you ever been refused a U.S. Visa, been refused admission to the United States, or withdrawn your application foradmission at the port of entry?”- “Have you ever been arrested or convicted for any offense or crime, even though subject of a pardon, amnesty, or othersimilar action?” You should mark “Yes” if you have ever been arrested or convicted of a crime, even if the charge was dismissed orsuspended.BIRTH CERTIFICATE: Submit an original or a certified copy of a birth certificate from the country of birth with English translation. If you were born in Korea, submit both Basic Certificate (Detailed) “Gibon Jeungmyongseo (Sang Sae)” (This videoexplains more about the acceptable Basic Certificate and you can download a translation sample here) and FamilyRelation Certificate (Detailed) “Gajok Kwankye Jeungmyongseo (Sang Sae)” (This video explains more about theacceptable Family Relation Certificate and you can download a translation sample here) issued within 1 year for eachindividual applicant with English translations. You can obtain these certificates at Gu(district) offices or Dong(subdistrict) offices. You can also obtain these certificates at “efamily.scourt.go.kr” or “minwon24.go.kr” if you have anauthentication certificate account. For IR5 and F4 visa applicants, you must submit an original or certified copy of petitioner’s birth certificate. If petitionerwas born in Korea, please follow the instruction above.

Also, obtain a police certificate from the country of your residence if your presencethere was for 6 months or more. Police certificates from other countries are also required if your stay was 12 months ormore after age 16. Check the reciprocity schedule for police certificates requirements for various countries. If you everbeen arrested in any jurisdiction, court records with English translations should also be submitted, even if the chargeswere dismissed or the sentence was suspended.KOREAN POLICE CERTIFICATE: Non-Korean nationals: For foreign nationals, Criminal Records Certificate �서)” must be obtained. This certificate can be obtained at any police station with a Foreign AffairsDivision (외사과). Your passport and one photo are required; no fee is charged. Applications take approximately oneweek to process, and you may pick up the certificate at the same police station. Application forms (Sample Applicationform) can be obtained at the police station. Korean nationals: Korean nationals must obtain a Criminal (Investigation) Records Check Reply ‘For permission offoreign country immigration and stay’ “Bomjoi-Soosakyongryeok Hoiboseo: Waekuk Yipkuk-Chaeryu Heogayong(범죄수사경력 회보서: 외국입국체류허가용).” These certificates can be obtained at any police station. A passport ornational ID card will be required for identification purposes. No fee is charged, and the certificates are issuedimmediately. Application forms (Sample Application form) can be obtained at the police station.*Note: The Korean police certificates are issued both in Korean and English. If you have ever been arrested in anyjurisdiction, court records with English translations should also be submitted.

Depending on the hospital schedule it could take up to 2 weeks before an appointment.You must have completed your exam before your interview. Once the medical exam is completed, the hospital will electronicallysend the medical report to the Immigrant Visa Unit. On occasion, the hospital might inform the applicant additional testing isrequired. If so, please follow the hospital’s instructions for testing. In this situation, please reschedule your appointment for a dateafter all testing will be completed. There is no cost to reschedule your visa interview appointment.The validity of your visa is tied to your medical exam’s validity, which is 6 months from completion of the medical exam.If a Class B TB condition exists, the visa is valid for 3 months from the time the evaluation is completed. If you are found to haveTB history, you will be subject to additional testing, which will take up to 8 weeks.U.S. immigration law requires immigrant visa applicants to obtain certain vaccinations prior to the issuance of a visa. Currentimmigrant visa vaccination requirements are available on www.cdc.gov. You can also read Frequently Asked Questions aboutour medical examination requirements.Please note that you are required to disclose any arrests and/or criminal convictions involving drugs or alcohol (including anyDUIs) to the panel physician at the time of your exam. Based on this information, an additional exam might be requestedaccordingly by the hospital.The range of medical examination fees at panel physician hospital is as follows: 330,000- 414,500 for age 15 or older; 240,000- 360,000 for ages between 2 and 14; and, 136,000- 255,000 for age 1 or younger depending on the procedures ortests determined to be necessary by the panel physician. Fees for any required vaccinations are additional. The price is subject tochange without notice.Note: You must choose the hospital from the list above. Once you start your examination at one hospital, please follow thedoctor's instructions during the examination and DO NOT change the hospital during the process under anycircumstances until you complete your medical examination. If you need to re-do your medical examination, you must do it atthe hospital where your previous medical examination was completed.For any inquiries regarding medical procedures, please contact the U.S. Embassy through U.S. Visa Information Service.Military Records (If you served in the military of any country): Military records must contain a complete record of theapplicant's service and conduct while in the service. The record must show any convictions of crime before militarytribunals. For information on how to obtain military records from various countries, visit the U.S. Department of State’sTravel.state.gov webpage on Military Records.AFFIDAVIT OF SUPPORT WITH EVIDENCE OF INCOME: Family-based visa applicant: Please learn more about the financial documents on https://travel.state.gov. Please bring allrequired documents, which may include:Form I-864 Affidavit of Support for each financial sponsor (petitioner and joint sponsor, if necessary).
Photocopy of each financial sponsor’s IRS transcript or a complete copy of IRS Form 1040 including all schedules (clickhere to learn more about the filing requirements). If the sponsor is not required to file a U.S. tax return, complete a Statementof Not Filing U.S. Federal Income Tax Return. This statement should be signed and an original copy.Relevant W-2s of each financial sponsor's Form 1040; if sponsor submits a tax transcript, no W2 is required unless his orher status is "married filing jointly".Form I-864A Contract Between Sponsor and Household Member for each financial sponsor who use the income of otherhousehold members to qualify.Proof of your U.S. petitioner’s status in the United States (photocopy of a U.S. passport, naturalization certificate, or lawfulpermanent resident card).Proof of your U.S. petitioner’s domicile in the United States, especially when the petitioner does not currently live in theUnited States. Employment preference (E) applicant: No Form I-864 is required unless a relative is the petitioner or has a 5% or moreownership interest in the petitioning business. Instead, a job offer from the petitioning company should be submitted. Thejob offer should be original and signed within 6 months. Child of U.S. citizen (IR2 who is under 18, except for step child) and widow(er) of the U.S. citizen (IW) cases: TheForm I-864W must be filed instead of I-864.PAYMENT: Each immediate relative, family preference and employment preference applicant must pay the statutoryimmigrant visa application processing fee. Please bring sufficient funds in cash (US or KRW), money order, or credit cards toyour appointment. Personal checks are not acceptable. A separate visa fee is required of each person immigrating, regardless ofage. If the immigrant visa processing fee was already paid to the National Visa Center (NVC), you do not need to pay it again onthe day of your appointment. Returning Resident (SB-1) visa applicants will be scheduled for an interview by the Immigrant Visa Unit of the U.S.Embassy once the DS-117 (Application to Determine Returning Resident Status) is approved. Family preference (F) and employment preference (E) visa applicants must schedule through the following steps.Step 1Please submit your appointment request online at https://kr.usembassy.gov.Step 26-10 weeks after you submit your appointment request, you will be notified if you are eligible to schedule an interviewappointment.Step 3After being notified, schedule the interview through the U.S. Visa Information Service call center or at the website.PLEASE NOTE: If you schedule your appointment before being notified, your appointment will be cancelled.A following-to-join applicant whose principal applicant was issued an immigrant visa at the U.S. Embassy in Seoul shouldschedule the interview through the U.S. Visa Information Service call center or at the website.YOUR APPOINTMENT:All immigrant visa applicants are seen by appointment only. You may enter the U.S. Embassy 15 minutes prior to your scheduledinterview time. Please note the Embassy has implemented a “One Cellphone Only” policy. Please check the items allowed and notallowed in the Embassy and the related security regulations at the U.S. Embassy website. It is your responsibility to come fully
prepared with all necessary documents in hand. At your interview, you will be sworn in as to the truth of your application. If youare found eligible for a visa at the time of your interview, your visa will be delivered by courier.
