The directory is the result of joint efforts put forth by thevarious subcommittees and workgroups organized through the Suffolk County Division of CommunityMental Hygiene Services. Suffolk County possess a vast array of services located throughout ourgeographical footprint, so we have provided a glossary of terms to assist in understanding the availableservices.The past year has been a most challenging one for our County, as our nation has battled the COVID-19Pandemic. The amount of pivoting from the comfort of “what was known” to the frequently changing“what must be” has been daunting for all and the impact to overall health and wellness cannot be stressedenough. Sadly, the rates of suicides and overdoses increased, and the services that the behavioral healthcommunity provide have become more essential than ever. In light of the safety of all, our agenciesquickly adapted to the Social Distancing needs by transitioning to Telehealth and Telepsychiatry. Althoughthere was a major shift in methods of service delivery, most programs were able to maintain services andin some cases even expand services to those in need. ADULT SINGLE POINT OF ACCESS (A-SPOA)ASSISTED OUTPATIENT TREATMENT (AOT)As per Section 9.60 of Mental Hygiene Law (a/k/a “Kendra’s Law”), AOT “provides for court- ordered outpatienttreatment for certain people with diagnoses of mental illnesses who, in view of their treatment history and presentcircumstances, are unlikely to survive safety in the community without supervision.” A person may be ordered toobtain Assisted Outpatient Treatment (AOT) if the court finds that he or she is: At least 18 years of age and suffers from mental illness; and Is unlikely to survive in the community without supervision, based on a clinical determination; andHas history of non-compliance with treatment for mental illness which has led to either 2 hospitalizationsfrom mental illness in the preceding 3 years, or resulted in at least 1 act of violence toward self or others orthreats of serious physical harm to self or others within the preceding 4 years; andIs unlikely to accept the treatment recommended in the treatment plan; andIs in need of AOT to avoid a relapse or deterioration that would likely result in serious harm to self orothers; andWill likely benefit from AOT.The AOT Order includes a comprehensive Treatment Plan which includes outpatient program attendance andmedication management in addition to monitoring and supervision by the Suffolk County Division of CommunityMental Hygiene Services AOT Unit. An individual receiving care coordination services can expect assistance withaccessing services that support an improved quality of life. Care coordination can involve assistance withhousing, outpatient treatment, vocational services, day programs and disability benefits. Additionally, the AdultSPOA unit provides referrals for care coordination services for those individuals without Medicaid coverage,and is available to offer assistance as you navigate the mental health system. Clients with Medicaid may alsomake a direct referral to a Health Home for care coordination services. All requests for clients withoutMedicaid are made through the Adult SPOA unit.
