All rights reservedPresident’sMessageThe recentlyapproved U.S.Health Reformpackage includesprovisions that increaseemphasis on prevention,positive health outcomes, bettercoordination of care and comparativeeffectiveness research, including personalizedmedicine. What does this mean for theclinical laboratory and how does Sysmex'sproduct offering fit into this new landscape?Clear evidence suggests that we will see ashift from the traditional paradigm of chronicdisease diagnosis and management to one ofdisease prevention, early treatment, tailoredtherapy, and genome/proteome/cellulomeguided disease management. Examples ofthis transition are well-defined in breastcancer, leukemia and cardiovascular disease.A test for the HER2 protein in breast cancerpatients has permitted better targeting oftherapies— resulting in a 50 percentreduction in recurrence and a 30 percentreduction in death for such patients. Teststhat read the DNA structure in acutelymphocytic leukemia, the most commonform of leukemia in children, have helpedboost the 10-year survival rate from fourpercent in the 1960s to more than 80 percenttoday. A test that identifies whether a patientis especially sensitive to Warfarin, whichprevents blood clots, could eliminate as manyas 17,000 strokes and 85,000 serious bleedingepisodes per year.For some time, Sysmex has pursued researchand development projects which support theabove paradigm shift with focus given to thecompany's technology “bank”. Sysmex also has developed the world'sfirst protein chip that can simultaneouslymeasure twenty types of proteins for use inpredicting the effects of anticancer drugs.Clearly, a manufacturer's research anddevelopment efforts will remain critical to aclinical laboratory's survival in this new era. Assuch, we at Sysmex will continue to view ourcustomers' needs from a forward-lookingperspective. In other words, how is the clinicallaboratory going to evolve over the years tocome and what can we do to shape and supportthat development? Eventually Sandywould like to see e-Supply include the consumables, QC and/orreagents for the SP-1000i (Slide Maker Stainer) as well as theflexibility to create a reagent order without receiving an alert, sheadded. Currently Mobile Infirmary processes 350 – 400 samplesdaily.Here is how e-Supply works.Prior to e-Supply start-up, each customer is asked to provide acompleted information sheet and a pre-determination of theminimum level of stock inventory or “safety stock” for each product.Recommended inventory level will consider reagent usage based oninstrumentation, volume and ordering pattern. This baseline will becompared with the customer's actual usage to ultimately determinestock level and reagent delivery.Stock level also considers factors such as maximum adjust days(How close to an existing order can this reagent be ordered?), maximumsupply days (How many days maximum supply should the customerhave on hand?) and minimum supply quantity (How many daysminimum supply should the customer have on hand?). If the safetystock is reached, an automatic e-mail will be generated at least fivedays before re-order point to notify the customer. If the customerdoesn't accept or reject the initial order notification, a second emailnotification will be sent; followed by a call from the SysmexCustomer Service Center if necessary.Once a customer has reviewed and approved an order throughSysmex's Customer Resource Center (CRC), the order will beshipped from Sysmex to the customer within the site's delivery timeframe.Delivery timelines consider factors such as minimum days betweenorder alerts, consolidation adjustment window, customer orderapproval window, estimated shipping time and the customer'sminimum stock levels. The analyzer provides a total of 34 parameters and like the Sysmex XE-5000, a Body Fluid specific mode,delivering rapid, reliable results essential in patient diagnosis and therapeutic monitoring. The success of the XT-Series hasbeen shown by more than 1,700 customers currently utilizing the XT-2000i and the XT-1800i.The unique features of the Sysmex XT-4000i include the IG (Immature Granulocyte) parameter, which provides reliabledetection and quantification of circulating immature granulocytes that may be used by physicians as an early indicator of acuteinfection, inflammatory response or myeloproliferative disorder.
