General prosthetic advice for Ankylos In order to obtain utmost benefit from the uniquerestorative features of the Ankylos implant system,please observe the following guidelines: A void occlusal overload, aim for initial infra-occlusionto ensure clearance of contacts in function. I f possible, use B-implants or larger for singlemolar restorations. U se gingiva height 0.75 and 1.5 only if really necessary, and in correspondence with thin gingiva. M inimize the functional occlusal surface in buccallingual dimension to avoid lateral levers. T ake adequate precautions to avoid occlusal overload in correspondence to any prosthetic cantilevers. B ear in mind the resilience of neighboring teethwhen planning for single tooth restorations. Establishfull contact on the implant crown only during maximal clenching ensuring an even load distribution onall teeth during maximum chewing force. C heck for parafunctional habits. Consider changesduring recall appointments. I n case of any changes to the occlusal scheme inother areas, evaluate the consequences for theimplant restoration and, if required, take appropriatemeasures. C/XConstruction principles of abutmentsGingiva formerMost gingiva formers are one-piece components for insertion into the implantwith the 1.0 mm hex screw driver.The Balance Anterior C/ sulcus former is a two-piece component with separatescrew to allow for customization and fixation in the desired position. Use the 1.0mm hex screw driver to fasten the screw.Temporary abutmentsThe Balance C/ temporary abutments are made from zirconia-reinforced PEEK astwo-piece components with separate screw (1.0 mm hex).Transfer postsTransfer posts are available for pick-up and repositioning technique.Transfer posts C/ transfer the implant position without index. Transfer posts /Xtransfer the implant position and the orientation of the index. Therefore they arelonger. All transfer posts show a 1 mm hex at the occlusal aspect, which should beused only for loosening of abutments.Transfer posts C/ for pick-up technique are two-piece components withseparate screw. The transfer posts C/ for repositioning technique are one-piececomponents.Transfer posts /X always have an integrated fixation screw.PROSTHETICSProsthetic abutmentsMany of the prosthetic abutments (e.g. Regular abutments) have an integratedfixation screw. This fixation screw is mobile but not removable. Abutments withintegrated fixation screw are always screwed into the implant with the 1.0 mm hexscrew driver.One-piece abutments (e.g. Balance Base narrow, straight) are inserted with specialinstruments depending on the abutment head.All Ankylos prosthetic components are laser-marked to indicate their use: Components marked with “C/” only use the cone for the connection and arenot indexed. This means that the abutment components can be positionedas desired and are completely friction-locked by the cone in order to preventrotation. Components marked with “/X” are indexed. The index is used to position theabutments in the implants in one of six possible positions. In this case, it is alsothe cone that ensures optimum stability and rotation locking. Components marked with “C/X” are used for indexed or non-indexedprosthetics.Please note:Ankylos C/X implants must only beused with components that are lasermarked with “C/X”, “C/” or “/X”, orthat belong to the following productgroups:Ankylos Balance Anterior and Posteriorabutments and reposition-ing posts,Ankylos Cercon Balance, Balancetemporary abutments, BalanceAnterior sulcus formers and SynConeabutments.7

Soft tissue management and abutment assemblyTransition implant-abutment displaced toward the centerSome special characteristics have to beconsidered for soft tissue contouringand the choice of the final abutment: D ue to the taper connection, thediameter of the Ankylos abutmentson the implant level is markedlysmaller than the diameter of theimplant itself. Soft tissues are also located underthe flanks of the abutment.Selection of the correct gingiva height T he illustration shows the correctusage of a gingiva former. If the gingival level lies in the areabetween the two dotted lines, thecorrect gingiva former has beenselected. If the level is above the upper dottedline, a higher gingiva former shouldbe selected; if it is below the lowerdotted line, a lower one should beselected.Selection of the gingiva former corresponding to the abutment The gingiva former and final abutmentshould be selected so that they correspond in prosthetic range and gingivalheight. The description of the height of thegingiva former as shown above (GH 3.0)only refers to the region from the inter-face level to the edge of the crown. The entire gingiva former is alwaysapproximately 1.5 mm higher than thenominal height, viewed from the interface level (here: overall height 4.5 mm),in order also to shape the first part ofthe crown profile.If the soft tissue passage formedis smaller than the profile of theabutment used, this may result in anavoidable compression of the gingiva.This effect occurs when:1. The gingiva former is markedlyhigher than the gingival level and theabutment used later on.2. The gingiva former is suitable to thegingival height, but the gingival heightof the final abutment is lower thanthat of the gingiva former used (seeillustration).3. The final abutment has a largerdiameter then the gingiva former used(e.g. in case of a different prostheticrange).When transferring the taperedconnection without index, a transferkey safeguards the exact position ofthe abutment in the implant.14
