This directory provides a list of the community providers for VNSNY CHOICEManaged Long Term Care (MLTC) members. The VNSNY CHOICE network iscomprised of hundreds of qualified professionals and long term care serviceproviders throughout New York. All our providers have met strict licensurerequirements before they became a provider in the VNSNY CHOICE network.How Do I Find A Provider in the Directory?Providers in our directory have been organized by county and specialty.The directory uses symbols to indicate which providers offer wheelchair accessible facilitiesor services for the hearing impaired or visually impaired. Please look for these symbolsthroughout the directory. The symbol legend is listed at the end of the Table of Contents.Do I Need to Use Plan Providers?With a few exceptions, you must receive all services that are covered by VNSNY CHOICEMLTC from one of the providers in our provider network. You do not need to use planproviders in the following situations: As a VNSNY CHOICE MLTC member, you can choose your own Primary Care Providers(PCPs). You do not need to use a plan provider in an emergency. In an emergency, please call911 or get to the closest hospital or emergency room. Once the emergency has passed,please contact VNSNY CHOICE as soon as possible so that we can assist and review theservices you are receiving to best address any changes in your health needs. If you need a service that is covered by our plan, but there isn’t a network provider withthe specific expertise that you require; we will find the right provider for you, even if theprovider is out of the network. Following Medicaid and Medicare guidelines, if you are receiving health care servicesfrom an out-of-network provider at the time of your enrollment, you may continue to seethis provider until you complete the course of treatment for up to 60 days. If you are in the process of receiving treatment for a health problem and your providerends their relationship with VNSNY CHOICE MLTC, you may continue to see this providerto complete the course of treatment for up to 90 days.If you have questions, please call us at 1-888-867-6555 and TTY is 711, 9 am – 5 pm,Monday – Friday. Can I Choose or Change My Own Provider?You may choose or change a new network provider at any time. The easiest way to changeproviders is to ask your care manager for help. You can choose any dentist, optometrist orpodiatrist listed in the provider directory. If you make a change, please tell your care managerso that he/she can continue to coordinate your care.Can the List of Plan Providers Change?Yes, we may add or remove a plan provider from the directory. This provider directoryincludes all provider additions and changes received at the time of print. To make certain thata particular provider is in the VNSNY CHOICE network, please call Member Services at thenumber below or go to our website at www.vnsnychoice.org/choicemltc and use our onlineprovider directory.How Often Will I Receive A New Provider Directory?The provider directory is updated annually and we will send you a notice when a revisededition is available.
