A Message from Our Leaders11To Our Community:On behalf of CarePoint Health, we are pleased to present our inaugural 2020-2021 Annual Report. Sinceopening in August 2019, CarePoint Health has been building a strong foundation and expanding to deliverhigh quality team-based interprofessional primary care for the communities we serve in South West and EastMississauga. This report is reflective of a year of hard work and successful initiatives.In 2020-2021, CarePoint Health continued to grow and deliver excellent patient care, continuing to meet theneeds of our patients during COVID-19. We expanded our network by connecting more communityphysicians and their patients to our team. New services such as physiotherapy, chiropody, expanded mentalhealth supports, COPD education, and health promotion programs were added. We integrated PsychiatrySpecialists into our team to bring specialist and primary care closer together and we leveraged digital healthto optimize the care experience.Together with our Mississauga OHT partners, CarePoint Health met the challenges of a global pandemic. Wepartnered to establish a Cough and Flu Clinic at CarePoint Health, administered COVID-19 vaccinations asa regional pilot site, and supported the regional primary care planning strategy.We look forward to implementing our new two-year strategic plan with enthusiasm and purpose, as we buildon the great work shared with you in this report. None of this work would be possible without our amazingteam of health professionals and affiliated physicians who always puts the needs of our patients front andcentre. Creating a Health Care TeamBeyond Our Walls44Building a Strong FoundationConnecting to Community PhysiciansCarePoint Health has now affiliated with 65 Primary Care Physicians to care for theirpatients most in need.Access to team-based care varies significantlyin location and quality across Ontario.Through engagement with primary carephysicians, we have created a network thatconnects patients and community physiciansto team-based, interprofessional programs andservices.Integrating Specialist CareCarePoint Health has partnered withPsychiatric Specialists to provide drop-downclinics for patients within our network. Theinclusion of specialists in the primary carehub improves collaboration with primarycare. This promotes shared care models,knowledge transfer to primary carephysicians, reduces duplication and waittimes, and enhances care continuity.

Shaping the Future ofHealth Care5Ontario Health TeamsCarePoint Health is proud to be afounding member of the MississaugaOntario Health Team (one of the firstOHTs in the province). Together with ourpartners, we are working to make surepatients, families and caregivers havebetter access to the care they need, whenand where they need it.CarePoint Health is an active member ofthe M-OHT Collaboration Council andPrimary Care Working Groups, workingto build a more integrated health systemnow and for the future.Responding to COVID-19:M-OHT Collaborations in ActionMississauga Health Cough & Flu Clinic atCarePoint Health2432 Patients Served (Dec 1 - Mar 31)The Mississauga Health Cough & Flu Clinic is acommunity-based clinic that provides timely inperson care for people who need to be seen by a doctorin a safe environment to address their flu and COVIDlike symptoms and receive a COVID-19 test ifneeded. The clinic reduced pressures at hospital-basedassessment centres and emergency departments.The clinic was established and operated through acollaborative partnership between three of the MOHT Founding Primary Care Members, CarePointHealth, Summerville FHT and Credit Valley FHT, aswell as Trillium Health Partners and communityphysicians practicing in Mississauga.COVID-19 Vaccinations in Primary Care PilotCarePoint Health was one of ten primary careorganizations in Peel region to establish a COVID-19vaccination clinic in a primary care setting. Primarycare vaccine clinics offer a safe and comfortingenvironment for patients to receive vaccines and helpaddress vaccine hesitancy by leveraging the primarycare provider relationship with patients.The clinic was established through collaborativeplanning with the M-OHT Primary Care WorkingGroup and Peel Community of Practice Group.
