California Department of EducationNutrition Services DivisionChild and Adult Care Food ProgramCACFP 19 (REV. 02/2018)Invitation for Bid (Delivery)INTRODUCTIONThe sample materials contained in this Invitation for Bid (IFB) and Contract package are supplied for use byagencies in the Child and Adult Care Food Program (CACFP) who contract for meals from vendors whosecontracts for those meals would total more than 250,000.Agencies should familiarize themselves with procurement procedures and requirements as found in Title 2, Code ofFederal Regulations (2 CFR), sections 200.318–326 and Title 7, Code of Federal Regulations(7 CFR), Section 226.22.These materials and the processes described within meet the regulatory requirements prescribed by 7 CFR, sections226.21 and 226.22 and 2 CFR, sections 200.318–326. (Make several copies of the bidpackage for dissemination to prospective bidders.)4.If applicable, develop a bid advertisement or public notice inviting bids (may use Attachment I) andpublish the notice at least once 14 days before opening bids. Publish the notice in the LegalNotices section of a local newspaper that has general daily circulation in the Agency’s area or in anappropriate trade journal.5.Solicit, in writing, bids from known food vendors in the Agency’s area. The solicitation must bedocumented and contain the same information found in the notice.6.Receive an adequate number of bids—two or more. (If you do not receive an adequate number,contact your assigned CACFP Specialist.)7.On the bid closing date and time announced in the public notice, open all sealed bids received.8.Complete page 7 (Attachment II).9.Mail, scan, or fax the following copies to your assigned CACFP Specialist:a.Each published noticeb.Documentation of all solicitationsc.The complete IFB (pages 5–34), including the federal, state, or local health certification forthe facility in which the bidding vendor proposes to prepare meals, from each biddingvendor. Complete the Contract for Vended Meals (Section A), but do not sign.10.Wait for your CACFP Specialist to fax or scan a signed copy of page 6 (Attachment III). (Thestate must respond within 10 working days of receipt.) This marks approval of the chosen foodvendor.11.Once you have receipt of approval, inform the chosen vendor in writing that you have selectedthem.12.Fax or scan your CACFP Specialist a completed and signed copy of page 8—the executedcontract itself (Section A of Attachment III).MENU PLANNING: When planning menus, all CACFP agencies must follow CACFP Meal Pattern guidance foundon pages 27–30 as well as the Key Recommendations available on the 2010 Dietary Guidelines for Americans Website at www.dietaryguidelines.gov.4

This IFB must appear in a newspaper having general circulation in the Agency's area or inan appropriate trade journal. At least 14 calendar days should elapse between the notice oradvertisement and the closing date for submission of bids. (These requirements are found in 7 CFR,Section 226.22.)The Public Notice must include:1. A complete description of the product subject for bid2. The Agency’s name and the address at which the bid is to be submitted3. The closing date and time for acceptance of bids4. Information that only sealed bids may be accepted5. Information that the bid opening will be made public at the time and place referred to in the publicnotice6. Information that the contract will be awarded to the lowest, most responsible, and responsivebidderIn addition to the Public Notice or Advertisement Inviting Bids, bids must be solicited from two or moreknown suppliers within the Agency's area. The solicitation must be made in writing and must contain thesame information found in the Public Notice or Advertisement Inviting Bids.5

The Agency shall supply a completed IFB to each bidder whoresponds to the notice.NOTICE INVITING BIDSSealed bids will be received by Centro de Salud de la Comunidad de San Ysidro inc. dba San Ysidro HealthAGENCY NAMEat1601 Precision Park Lane, San Diego, CA 92173AGENCY ADDRESSuntil5:00 p.m. on July 22, 2020for meals to be served in the center(s).TIME AND DATE FOR BID CLOSING PERIODAt the time and place advertised, and promptly thereafter, all bids that have been duly received will be publiclyopened and read aloud.Description of Product for Bid:COMPLETE DESCRIPTION OF PRODUCT DESIRED: TYPES OF MEALS OR PRODUCTS NEEDED, NUMBER OF DAYS SERVICE IS DESIRED, NUMBER OFDELIVERY POINTS, AND PACKAGINGBidder will provide breakfast and lunch at (i) SYH’s San Ysidro Health Adult Day Health Care Center located at 3364 Beyer Blvd, SanYsidro, CA 92173 and (ii) SYH’s Adult Day Health Care – Chula Vista located at 880 3rd Ave, Chula Vista, CA 91911.Bidder must comply with the following: 1) Provide Renal Diet, ADA, and any other prescribed modifications; 2) provide alternative dailymeals which address participant allergies and preferences to include lactose intolerance and vegan preferences; 3) Hot or cold meals mustbe delivered in bulk containers only; 4) Bidder must provide highest quality of meal service to include appetizing appearance andorganoleptic standards; whole grain and whole grain rich, fresh fruits and vegetables are mandatory requirements. 5) Bidder must followethnic meal preference a majority of the time (at a minimum 4 out of 5 meals per week) with holiday alternatives; 6) Bidder will not deviatefrom menu or protein alternatives unless requested by Agency. 7) Bidder must submit to and get approval for standardized recipe fromAgency which will include amounts of protein vegetable and grains per serving; 8) Frozen foods are not acceptable unless preapprovedby Agency and Child Nutrition labels will be provided to Agency prior to menu approval; 9) Bidder must follow portion sizes outlined inCACFP minimum requirements (included in this packet) with the exception of the serving of protein which must be a minimum 4 ounces ofprotein per serving; 10) Bidder shall perform biannual satisfaction surveys and report on any necessary corrective action plans; 11) Biddershall provide 3 four week cycle menus that provide for variety, ethnic preferences and alternative meals that address participant allergies andpreferences to include lactose intolerance and vegan preferences; 12) Bidder shall deliver hot food at the minimum required temperature of165 degrees Fahrenheit and cold food not to exceed 40 degrees Fahrenheit. Meals that do not comply with the temperature requirements willbe rejected and will not be compensated; 13) Bidder shall provide proper storage container to maintain appropriate food temperature upondelivery and hold food at proper temperature for up to 45 min after delivery. 14) Vendor shall maintain full and accurate records includingnumber of meals provided/delivered, all foods provided to meet required meal components for each meal, serving size information for eachfood item, the total quantities of all food, daily delivery slip records, daily production records, standardized recipes and nutrient analysis, CNlabels or production formulas statements for commercially prepared food, receipts for meals costs and billing statements; which will be madeavailable to the Agency or any state or federal official upon request.Breakfast and lunch meals shall be provided on a daily basis for a minimum of 100 individuals per site, Monday through Friday, 52 weeks ayear.Delivery of meals shall be made to Agency at the following locations i) SYH’s San Ysidro Adult Day Health Care Center and ii) SYH’s SanYsidro Adult Day Health Care –Chula Vista sites in bulk containers exclusive of milk. Hot meals must be delivered at a minimum temperatureof 165 degrees Fahrenheit and cold foods not to exceed 40 degrees Fahrenheit. Meals that do not comply with temperature requirements6

California Department of EducationNutrition Services DivisionChild and Adult Care Food ProgramCACFP 19 (REV. 02/2018)Invitation for Bid (Delivery)will be rejected and will not be compensated.As a part of providing services Bidder must agree to comply with all policies and procedures of Agency and to (i) furnish only thoseservices authorized by Agency interdisciplinary team; (ii) accept payment from Agency as payment in full and not bill program participants,CMS, STATE MEDICAID DEPARTMENT or private insurers; (iii) hold harmless CMS, STATE MEDICAID DEPARTMENT, and Agencyprogram participants if Agency cannot or will not pay for services performed by Vendor under this Agreement; (iv) not assign thisAgreement or delegate duties under this Agreement unless prior written approval is obtained from Agency; (v) submit reports as requiredby Agency; (vi agree to perform all duties related to its position as specified in 42 C.F.R. Part 460; (vii) participate in Agency’sInterdisciplinary team meetings as required; (viii) agree to be accountable to Agency; (ix) comply with the competency evaluation programand direct participant care requirements specified in 42 C.F.R. 460.71 (if applicable); (x) Bidder must make themselves accessible toAgency participants; (xi) meet Federal, State, Medicare & Medicaid requirements and comply with service delivery, participant rights andQuality Assurance and Performance Improvement (“QAPI”) activities of Agency; (xii) and designate an official liaison to coordinateactivities between Bidder and Agency.All the requirements of this bid shall be incorporated into, and become a part of the final Agreement between the parties upon executionof “Section A” of bid documents as further described therein.ORThe following types and quantities of meals:BREAKFAST, A.M. Submission of Bids(a) Bidders are expected to examine carefully the specifications, schedules, attachments, terms,and conditions of this IFB. Failure to do so shall be at the bidder's own risk.(b) Bids shall be executed and submitted in triplicate with one copy being marked original. Ifaccepted, this IFB will become a part of the contract and one copy of the accepted bid/contractwill be forwarded to the successful bidder with the notice of award.
