Boulder Creek Region Schools Communication ProtocolStakeholder communication plays an important role in the success of a school.Solutions are best found at the level closest to the issue or concern.As such, we have adopted the following region-wide protocol.TeacherCampusCounselorIf there is a concern or question about homework, classroom policies, events thathappened in class, or anything pertaining to the teacher or classroom in general, parentsmust first communicate with the teacher to find a solution. A parent teacher conferencemay be scheduled.If there is a concern or question about behavioral, social and emotional concerns such asstudy skills and organization, peer groups, health and independence, post secondarygoals parents must communicate with the school counselor to find a solution. A parentteacher and counselor conference may be scheduled.CampusAdministrationAfter the teacher and counselor are given the opportunity to resolve the concern orissue, and the situation has not yet improved, please contact administration.Communication with the teacher or counselor must happen prior to administrativeintervention. GENERAL INFORMATION/SERVICESBOOKSTOREStudents will get their textbooks the first week of school. Books that are lostor damaged become the student’s responsibility. Most of the texbooks aredigital and can be accessed using the iPad. The Bookstore will be openbefore and after school and during lunch hours for regular business. Hours ofoperation is 7AM to 3PM.Bookstore Manager: Jennifer Kang.Bookstore Clerk: Elizabeth DavidsonCAFETERIAThe cafeteria will provide free lunch and breakfast on all full days of school,and on the Friday early release days. Scanning a student ID is required. A lacarte items are also available for purchase at both breakfast and lunch. Allstudents have a lunch account that can be used in place of cash. Thecafeteria accepts checks and cash for student accounts. Deer ValleySchools participates with the EZ school pay system to allow parents to placemoney on the student’s account using a debit or credit card. The address iswww.ezschoolpay.com. No debit cards or credit cards are accepted in thecafeteria. The cafeteria staff will alert students when they are at a 5.00balance so that the student may bring in more money for their account.Students who do not have lunch money will receive a complimentary mealfrom the cafeteria for that day. Deer Valley Schools participates in the Freeand Reduced Lunch Program. Eligibility for this program is determined byfamily size and income. Students who may want to apply for this programshould obtain an application from the front office or cafeteria. Applicationsare also available online at www.ezmealapp.com. All applications should beeither mailed to the address listed on the application or given directly to thecafeteria staff. All information is confidential.DVUSD works hard to provide students meals that are nutritious, costeffective and delicious. All meals are developed to meet the USDA NationalSchool Breakfast and Lunch standards as well as meet the cost needs of ourfamilies and the tastes of our students. In order to support the increasedindependence of our high school students, they are offered a wide variety ofoptions for both breakfast and lunch. Options include yogurt, breakfastsandwiches and fresh fruit for breakfast and fresh salads, Mexican, Asian,Italian and deli items for lunch. The academic halls are closed in themorning except for student/teacher tutoring and student club meetings.5

COUNSELING CENTERStudents’ counselors are highly qualified educators available to guide them inacademic, vocational, and personal areas. To see a counselor, the studentshould go to the Counseling Center (before school, during lunch, or afterschool) to sign up for an appointment. Students will be called out of class witha pass delivered to them when a counselor is available to see them. If theteacher feels the student cannot afford to miss class at that particular time,the student must respect the teacher’s decision and reschedule theappointment. Parents are encouraged to call for appointments. SCHOOL IS A HEALTHY ENVIRONMENTThe nurse promotes a positive wellness environment to give each student thebest possible opportunity to learn and reach their potential. The nurse is theconsultant for students, parents and educational staff members concerninghealth issues. He or she may suggest further medical follow up when a healthproblem is suspected.HEALTH CENTERThe Health Center provides first aide and medical care primarily for illness orinjuries that occur during the school day. It is not a primary care facility like adoctor’s office. The nurse is qualified to collaborate with you and your child’sphysician to provide an educational environment in which your child canlearn and thrive. While the nurse does not make a medical diagnosis orprescribe medication or treatment, medication and treatment prescribed by aphysician may be stored and given in the health office.ILLNESS OR ACCIDENTThe Health Center cannot diagnose medical conditions. If a student feels illduring the school day, they should report their complaint to the classroomteacher for referral to the nurse. Students are not to contact their parentsdirectly via cell phone regarding illness unless prior arrangements have beenmade with the nurse. Please keep your child home if they have fever,diarrhea, vomiting, deep cough, or a potentially communicable disease.Children with a fever must be fever-free for 24 hours without the use offever reducing medications before returning to school. If your child hasbeen diagnosed with a contagious illness, please contact the Health Centerso parents of other students in the class can be notified. Please call yourchild’s doctor for advice on an ill child. Do not send ill students to school to beevaluated by the nurse.STUDENT HEALTH & RELEASE INFORMATION FORMSBack to School Forms are now available online for your DVUSD Students.This process replaces the majority of paper forms sent home at thebeginning of each school year. You will use the PowerSchool Parent Portalto access and submit these yearly forms at the start of the school year.Contact Boulder Creek High School at 623-445-8600 if you do not haveparent/guardian access to PowerSchool.Health forms must be on file inorder to attend BCHS. Updated emergency and health information assiststhe Health Center personnel in providing each student the most appropriateintervention and parent/guardian notification during an emergency. Familiesmay change emergency contact information by visiting the front office.7

DISEASE PREVENTION/IMMUNIZATIONSWritten proof of compliance with immunization requirements is a part of theregistration process in Arizona Schools. State and Federal Law states astudent must be immunized or they are unable to be in school, or a parentmust sign an exemption form. If you are unsure of the requirements forImmunization, see your doctor, visit the School Health Center or call theMaricopa County Health Dept. at (602)-506-6767 or (602)-263-8856 forlocations of clinics and hours of operation. Immunizations are free of chargefor children under the age of 18.PARENT AND EMERGENCY CONTACTS/HEALTH HISTORYParents are required to complete Emergency Contact and HealthHistory Forms online for each of their children every year. These formscontain vital information should your child becomes ill or injured at school.Students will be released to ONLY those persons indicated on the form.Photo ID is required to pick up any child during the day at school.INSURANCEStudent accident insurance is available to all students at a low premium. It isup to your parents/guardians to secure the insurance. The school district maynot, according to law, provide student insurance but it may make theinsurance available. Insurance information and applications are madeavailable to you at the time of registration. You may also request theinformation from the Athletic Office. Upon completion of the enrollment form,it may be turned in with your premium to the Athletic Office. Studentinsurance or proof of insurance is required for participation in athletics.HOMEBOUND PROGRAMStudents who must be absent from school because of physical disabilities orextended illness may qualify for homebound instruction. A doctor’s (not a PAor NP, only a MD, DO, or Psychiatrist) written statement that the student willbe absent 90 or more school days will be required. Forms are available in theHeath Center and must be returned to the Nurse to be approved by District.PE EXCUSESStudents requesting to be excused from P.E. must bring a note signed bytheir parent to the Nurse. The Nurse will then write an “excused from P.E.”pass for the student to take to P.E. Any request for an excuse for three ormore PE classes must be accompanied by a Physician’s written order. Allinjuries requiring any type of orthopedic support or device oncampus must be reported with a medical excuse and cleared throughthe Health Center.ELEVATOR KEYTo obtain an elevator key, you must have documentation from a physicianstating such. Documentation remains in the student health file. The schoolnurse dispatches keys after cleared by the nurse. If student losses the key,a debt will be added to the students account of 5.8

CHRONIC HEALTH CONDITIONSIf your child has been diagnosed with a chronic health condition pleasecontact the nurse immediately. He or she will inform you of the DVUSDprotocols/procedures necessary to ensure a healthy and safe environment foryour child during the school day. Student must be fever-free for 24 hoursbefore returning to school.NOTICE ABOUT MEDICATIONSIn compliance with ARS 15-344 and ARS 32-1901, over-the-countermedications *are not available in the Health Center*. Tylenol, Ibuprofen,Calamine, Neosporin, topical or oral Benadryl, cough drops, antacids or anylike remedies are not stocked. Students are not permitted to have thesemedications in their possession at any time.Prescription and over the counter medications can be stored in theHealth Center for student use during the school day.1.Over-the-counter Medications needed by students during the school daymust be checked in to the Health Center by a parent/guardian. Forms areavailable in the Health Center. Herbal preparations must have a doctor’sorder – excluding marijuana. Each bottle of medication must be in theoriginal container and a medication form must be filled out and on file at theHealth Center.2.Prescription medications can be brought to the Health Center but must be inan original prescription container labeled for the student with a current date.The label must state directions that coincide with the request being made bythe parent3.Inhalers and self-administered “Epi-Pens” may be stored in the HealthCenter and/or carried by the student if the proper medication form is filledout and placed on file in the Health Center. Please note – the product musthave a prescription label attached and the student should carry a copy ofthe doctor’s order at all times.4.Over the counter (OTC) medications consist of any product containing“active ingredients” i.e., Eye drops, lotions, contact solution, homeopathicremedies etc It is the responsibility of the parent/guardian to pick up any unused ordiscontinued medications promptly from the health center. At the end of theschool year, any medication remaining in the Health Center will be discarded.Medications may not be shared with friends! Failure to follow these rules,when carrying medication on campus, could lead to consequences includingpossible suspension or expulsion.9

DVUSD 2021-2022 COVID-19 Parent/Guardian ExpectationsWe all must work together to make our school campus as safe aspossible for students, teachers, and staff. While the Deer Valley UnifiedSchool District is taking measures to reduce the risk of spreadingCOVID-19, we need our families to do the same.You and your child are expected to follow the COVID-19 Code ofConduct as described below. Please read each statement. Please notethat the District reserves the right to amend these expectations at anypoint during the school year.1. I will take my child’s temperature every day prior to coming toschool and conduct a daily screening of my child for COVIDlike symptoms prior to my child arriving at school.2. I will keep my child home from school if my child has any ofthe following symptoms that are not related to an alreadydiagnosed condition or illness: (This list may be updated bypublic health authorities in the future. Parents should routinelyvisit the CDC’s symptoms page.) Fever of 100.0 or higher Chills Cough Shortness of breath or difficulty breathing New loss of taste or smell Sore throat Fatigue Muscle or body aches Headache Congestion or runny nose Nausea or vomiting Diarrhea3. I understand that my child will be sent home if showing any ofthe above symptoms while at school. I agree that I will pick upmy child within 1 hour from being notified by the school thatmy child is being sent home. I understand my child may need towear a mask while waiting to be picked up by aparent/guardian.10

4. I will keep my child home from school if my child has closecontact with someone (e.g. household member) who has testedpositive for COVID-19 or has COVID-19 like illness andfollow recommended quarantine guidance which can be foundbyvisiting ts?bidId unless my child meets one of the exceptions thatare outlined in public health guidance.5. I have read the Maricopa County Department of Public Healthguidance which can be found byvisiting /Home-Isolation-Guidance?bidId .6. I understand that this low threshold for keeping kids at homemay mean that my child may be away from school more oftenthan in the past, and I will plan for such contingencies.7. I understand that face masks are not required, but optional. Thecurrent exception to this is that a federal mandate requiresstudents and staff to wear a face mask when utilizing Districttransportation.8. I understand that the school will require my child to wash theirhands, use hand sanitizer, and socially distance according to theprotocols established by the Deer Valley Unified SchoolDistrict.9. I understand that the Deer Valley Unified School District willfollow the Maricopa County Public Health Department’sprotocols on the reporting of COVID-19 illness at the school.10. I will follow the District’s COVID-19 reporting procedureswhich can be found at www.dvusd.org/covidreporting. I willreport to the District’s COVID-19 reporting form which can befound at https://url.dvusd.org/COVID19ReportingForm andcontact the campus nurse as well as the campus’ designatedCOVID-19 point of contact. At your campus, the COVID-19designated point of contact is [Insert Designated Person (e.g.,school nurse, front office or Designated Phone Number] ifmy child has a confirmed positive COVID-19 test result. LATEX “FREE” CAMPUSBoulder Creek is a latex (natural rubber) “free” school; this includes all buildings,grounds, and parking lots. No items containing latex are allowed i.e; Rubbergloves, office products, balloons, or rubber balls and toys.We say “free” as we do not restrict latex in shoes or some other known productsat less risk because they do not produce airborne latex proteins. However, we dorestrict the use of latex in classrooms and common areas. Thank you all forhelping us to make a safe environment free from any potential latex items.DELIVERIESClasses will not be interrupted for delivery of items. Flowers, birthdayitems, pizza, etc. will not be accepted from businesses for delivery at BCHS.Restaurant deliveries are also not allowed. Parents will need to wait for theirstudents to be called to the office between classes to receive the item that hasbeen brought for them by a parent or family members for their child only.PHYSICAL EDUCATION/DANCEParent excuses are accepted in writing only, for up to three days. After three(3) days a doctor’s written excuse is required. These excuses should state:the student’s full name, grade, the date the excuse was written, thereason and the number of days for which the excuse is written for.The statement “excused until further notice” is not acceptable from thedoctor.All excuses must be received at the Health Center before the beginning ofthe school day or during that sport class. These rules also apply to danceclasses.IDENTIFICATION CARDSAt the beginning of the school year, each student will be issued an ID card.I.D. cards are required for admittance to all school activities. The replacementcost is 5.00 and may be purchased in the Bookstore12

LOST & FOUNDLost and found is maintained in the administration office through the securityteam. Lost books are sent back to the bookstore. Unclaimed items will bedonated to charities at the end of each semester.STUDENT RECORDS/POLICY STATEMENTThe records of District students are confidential and may be released and/orreviewed only for educational purposes that are of benefit to the student. Therelease of student records will be in compliance with Arizona RevisedStatutes and all applicable Federal laws. Further information on this subjectmay be obtained from the Office of the Registrar. Parents or students over 18years of age may view records at any time.CLOSED CAMPUSBoulder Creek High School is a closed campus. Students must remain oncampus during lunch time. A parent or guardian signature and photo ID isrequired to release a student for personal reasons during school hours.NORTH VALLEY REGIONAL LIBRARYThe library used by BCHS students is a shared-use library. Thismeans it is a public library, as well as the school library. Schoolrules for behavior and dress code apply to students in library.You must have a Maricopa County library card to use thelibrary computers or check out materials. To get a library card,a parent or guardian must come to the library with you and providea photo ID and proof of Maricopa County residence.For the public we are open:Monday—ThursdayFriday—SaturdaySunday9 am— 8 pm9 am—5 pmClosedThe library opens for students only at 7:00 amThe library has a limited number of lunch passes each day.13

Academic Requirements for Participation inExtracurricular ActivitiesIn order to represent the District in an extracurricular activity as described in“No Pass No Play”, the following guidelines apply:The student and the parent/guardian shall be notified when:Ineligibility is pending. Ineligibility is determined to be necessary. Support services shall be made available to students who becomeineligible for extracurricular programs as well as to students notified ofpending ineligibility.The same general standards shall apply for special education students,except that such eligibility shall be determined on a case-by-case basis inrelationship to the respective student’s individual education programs.In implementing this policy, the following clarifications apply to allstudents: Students must successfully complete all classes in which they areenrolled; consequently, any student who withdraws from a class with a“W/F” or loss of credit due to discipline or any other reason will beineligible for the duration of the semester. During an appeal process for loss of credit, the student will be ineligibleto compete in extracurricular activities. Further clarifications as they apply to more specific activities are asfollows:AIA requires students (9, 10, & 11) to be enrolled in five classes forparticipation. Seniors who have sufficient credits to graduate may enroll in as fewclasses as needed for graduation credit. ATHLETICSAll athletic teams-freshmen, junior varsity, and varsity are coveredby this policy. Athletes may be allowed to practice with the team during period ofineligibility. Inelgible students past pending will not be able to play in gamesor travel All Spirit line members are covered by this policy. FINE ARTSCompetitive activities in music (band/choral) are as determined in the StateAssociation Handbook. Specifically, these include: Ensemble Festival, AMEARegional Band/Orchestra/Choir Festivals, AMEA Arizona All-State Auditions,and all University Band Days.STUDENT ACTIVITIESAny student eligible may belong to any school club and participate in localclub activities.15

ATHLETICSARIZONA INTERSCHOLASTIC ASSOCIATIONFinal authority and ultimate responsibility in all matters pertaining tointerscholastic activities of each shall be vested in the principal. Theprincipal’s responsibility shall include, but shall not be limited to, the following: HOST SCHOOL – The basic responsibility of the host school is totake every possible measure at any athletic or non-athletic activity toassure courteous, friendly, safe, and fair treatment to visitingplayers, school representatives, fans, and game officials. ADMINISTRATIVE CONTROL – The administration andsupervision of all facets of all activities in the high school programshall be entirely controlled by the properly constituted schooladministrator. During all post season and/or play-off competitionheld at a neutral site, an administrator or administrator’s designeefrom each participating school shall be in attendance. CONDUCT OF PARTICIPANTS – The responsibility for the conductof the coaches, players and spectators at any athletic or non-athleticcontest shall lie with the administrators of the schools whose teamsare participating in the contest.It is a privilege to be an athlete. Athletes should be among the best and mostreliable students on campus. Athletic packets, which include forms, instructions, etc.,are available in the Athletic Office or on Boulder Creek website (campus life).16

To participate in interscholastic athletics, you must meet ArizonaInterscholastic Association (AIA) requirements. The following are a few of thespecifics.NO PASS/NO PLAYAll students participating in AIA sanctioned events must maintain a passinggrade in all classes. “NO PASS/NO PLAY” guidelines as established by theState Board of Education are in effect.Teacher’s academic records will be surveyed every week and a studentfailing will receive notification of “pending ineligibility”. A studentfailing the same class for two consecutive weeks will be ineligible toparticipate (Monday-Saturday), (unless a subsequent check after oneweek indicates a passing grade.) The “ineligibility” period will then beone week (Monday-Saturday). The duration of the grade check will beThursday – Thursday.At coaches’ discretion, students will be able to practice during the ineligibilityperiod and may suit up for competitions.AGE LIMITIf a student becomes nineteen (19) years of age on or before September 1 stof that current year, he/she is not eligible to participate in interscholasticathletics.DATE OF ENROLLMENTA student enrolling after the semester begins or transferring from anotherschool may not participate until cleared through the Athletic Office.AIA BYLAWS; ARTICLE 1515.3.6In order to establish eligibility, a student’s initial enrollment shall be no laterthan the 14th official school day of the semester.
