PLAY1 Contact Your Personal Care Navigatorto Schedule Surgery Center & Imaging VisitsFlexibility SavingsWhen you’re informed you need a non-emergency MRI, CT, PET scanor surgical procedure, contact your Health Plan to see if your surgeryis eligible before scheduling.Examples: Colonoscopies, Hernia Repair, Knee Replacement SurgeryThe Care Navigator will attempt to locate quality medical providerswith competitive bundled pricing (e.g. Surgeon, Anesthesiologist, etc.) inyour area. This process can take up to two weeks. If successful, they willpresent you with options, your financial responsibility and schedule yourprocedure.
PLAY2 Visit Any HospitalIf your surgery is urgent, is not eligible for Care Connex or you do notwish to use Care Connex, your Health Plan allows you to visit any hospitalor facility you want. You are no longer bound by the restrictions of“In-Network” or “Out-of-Network”.PLAY3 Our Advocates Are Here for YouWhen you visit a facility or hospital, the claim will be: Audited Fairly-priced Paid to the facilityIf the facility wants additional payment, they may send you a bill for theremaining balance (called a “balance bill”).If you are contacted for additional payments, immediately contact yourPatient Advocate at (800)425-9373.
Patient responsibility is acritical component of the Care Connex program.Make sure to pay the provider directly only for your patient responsibility(copayments, deductible and coinsurance), located on your Explanationof Benefits. If you can’t pay the entire bill, setup a payment arrangement.Failure to make timely payments consistently will limit your PatientAdvocate’s ability to protect you against balance billing and may evenexpose you to additional financial liability.
