A Messagefrom our President/CEO and Board ChairDear Friends,Our mission of clinically excellent, compassionate care to enhance the health, well-being,independence and dignity of residents of our community is our beacon every day. Thisyear saw a continuation of the expansion of our services throughout Fairfield County. Weprovided compassionate and skilled home health and hospice care and volunteer visitsto nearly 1,800 individuals and families in the setting where they want it most – in theirown homes.Our Board of Directors and leadership staff have worked tirelessly to position the agencyto meet the growing home health care, community health and hospice care needs ofthe residents of Fairfield County. The generous support of our benefactors enables us todeliver care to all those in need regardless of their personal circumstances. This year we once again provided over 1 million of charitycare to individuals and families in their time of greatest need.We participated in patient-centered initiatives with an increasing number of health care partners to improve the health and well-beingof the population we serve. This year we have been selected as a preferred partner with several additional health care and hospiceorganizations, providers and hospital systems.We redoubled our deep commitment to the community this year with the purchase of and relocation to our newly designedand renovated building in Wilton; the hub of our care. The new space offers a warm and welcoming environment as well as atechnologically-advanced setting for the members of our communities, our staff and our volunteers to gather.Our ability to provide advanced, complex clinical services, a continuum of community-based care and our deep and abidingcommitment to and involvement with our served communities to deliver care to all those in need distinguishes us in the field of homehealth and hospice care providers.We are honored to continue our second century of compassionate caring throughout Fairfield County.Warmest regards,Sharon M. Throughout the year, ouragency conducts blood pressure screenings, flu and pneumonia clinics, public healthand emergency preparedness services and a wide array of health educational programsand support groups throughout Fairfield County. This past year, thousands of individualsbenefitted from these offerings. In the public and private schools in Wilton, our schoolnurses care for and educate over 4,000 students each day as well as attend to the needsof parents, school faculty and staff.

BuildingServices to the CommunityS c h o o l N u rs i n gOur school nurses treat minor injuries and care for a wide array ofhealth conditions of the students in their care. Our nurses are liaisonswith school, home and community resources. We not only care forand educate students, but also tend to the health needs of parents andstaff and work to meet the ever-growing needs of the community.C o mm u n i t y H e a lt hAs public health nurses of Wilton, we are guardians of the public’shealth and wellness. We conduct blood pressure screenings, flu andpneumonia clinics and cholesterol screenings. We also provide a fullrange of educational programs and engage in emergency preparednessactivities.E d u c at i o nVisiting Nurse & Hospice of Fairfield County serves as a clinical sitefor several schools of nursing, providing a unique, community-basedexperience for undergraduate and graduate nursing students.6

Our compassionate team of nurses, socialworkers, spiritual care providers and volunteers offer patients and theirfamilies the following services: Managing pain and symptoms to provide comfort Counseling and companionship Spiritual comfort and guidance Personal care including bathing, dressing and meal preparation Arranging for medical equipment and nutritional guidance Bereavement support Providing hospice and palliative care at Fairfield County House, aresidence in Stamford dedicated to care at the end of life.B ere av emen t S u pp o r tThe journey through grief can be long and difficult. Losing a lovedone can impact a person physically, emotionally, intellectually, socially,spiritually and financially. We support family members and loved onesthrough the first year of loss and beyond. Our grief counselors workwith bereaved individuals and groups to help them cope with their lossand begin to heal.Ce l ebr at i o n o f L i feWe are grateful for the privilege of caring for our patients and their lovedones. We honor the patients we have served with an annual Celebration ofLife memorial service which includes poetry, readings and music as well as acandle-lighting ceremony conducted by our team of nurses, counselors andcaregivers.7

BuildingRelationships with VolunteersD ed i c at ed V o l u n t eersEach year, we honor our volunteers at a special awards ceremony inrecognition of their commitment, compassion and dedication to service.This year, Deb Rakich received the agency’s highest honor -- The Carol B.Bauer Community Spirit Award -- for her outstanding service as a hospicevolunteer.A cherished community resource since1930, The New Canaan Thrift Shopis housed at its landmark location at 2Locust Avenue in New Canaan. It is oneof the oldest volunteer-run thrift shopsin the United States and all proceedsdirectly benefit our patients.Photo by Bruce AndoThe New Canaan Thrift Shop offersquality goods at bargain prices andcustomers travel from far and wide toshop for treasures. The Thrift Shopprovides a valuable service to arearesidents and it provides our agency withessential income to help provide homehealth and hospice care to those in need.The Turnover Shop of Wilton was founded in 1950 to benefit VisitingNurse & Hospice of Fairfield County and the Wilton PTA. The Stamford couple was married in 1995 and between them,they have seven children and 15 grandchildren.Photo by Bruce AndoOver the course of her career, Phyllis worked as a patient advocate at MemorialSloan Kettering and then at the Leukemia/Lymphoma Society. She has been ahospice volunteer with Visiting Nurse & Hospice of Fairfield County since 2012and has recently joined the Board.“It is truly an honor to help people at the most vulnerable time of their lives,”said Phyllis. “As a hospice volunteer, I visit patients in their homes. Often theseindividuals feel isolated by their illness and I offer comfort and companionship toease their loneliness.”Tom is a partner with Sterling Equities in New York, where he has worked since 1975. He recently underwent a complicated,nine-hour spinal surgery and needed intensive home care when he was discharged from the hospital after four days.“Visiting Nurse & Hospice of Fairfield County had a nurse at our home right away,” said Tom. “She assessed my needs anddetermined what assistance and equipment would be necessary for my recuperation. In addition to her nursing visits, shearranged for a physical therapist and an occupational therapist to work with me twice a week. I was severely restricted andthey assisted me in every necessity. They were helpful, compassionate, competent and confident. The care they provided wasexceptional and totally exceeded my expectations.”“To be on the receiving end of this level of care makes me proud to be a part of this agency,” continued Phyllis. “They provideeducation, support and guidance to patients and their families. You don’t think about it until you are in the situation. You can’tanticipate what it will be like or what you will need. We felt totally reassured. We had someone to call.”In addition to supporting the agency on an annual basis, Phyllis and Tom have made provisions for Visiting Nurse & Hospice ofFairfield County in their estate planning and are members of the Lillian Wald Legacy Society.“The agency provides free services for those who are unable to afford the care they need,” said Phyllis. “These services are vitaland, as a non-profit agency, they need our support. We consider this to be a social justice issue. We want the agency to continueto serve others in the way that we have benefitted from their care. What would happen if they weren’t there to meet this criticalcommunity need?”11
