Ispoke with a nurse at the agencyand she explained that theirCare Management program wasespecially suited for people whowish to be cared for at homeand want to maintain theirindependent lifestyle.“She is a very selfsufficient person andinsisted on living at homeand continuing to doall the things that sheenjoys.”Safe and Secure at HomeMy mother has alwaysbeen a pillar ofstrength. She isspirited, capable andfiercely independent. She enjoysbeing physically active and loves towork in her garden. I was concernedlast year when she became easilywinded and seemed more tiredthan usual. She was diagnosed withcongestive heart failure and I wasafraid that it would change thequality of her life. She refused to letthis stop her though, and when Iasked her to come live with me andmy family, she would have none ofit. She is a very self-sufficient personand insisted on living at home andcontinuing to do all the things thatshe enjoys.The next day, a Care Managermet with us and put together acustomized plan-of-care for mymother. A nurse now visits withher once a week and she alsoreceives physical therapy at home.Our Care Manager coordinateswith my mother’s cardiologist andme and makes certain that mymother’s needs are met.Best of all, Visiting Nurse & Hospiceof Fairfield County monitors myContinued on page 31

The agency has purchasedthe former Hitchcock FurnitureStore and the building willundergo a major renovation. Thebuilding will provide about 8,400square feet of space to accommodate the agency’s clinical andadministrative offices.“We have a long and rich historyin Wilton,” said Agency Presidentand CEO, Sharon Bradley. “Inaddition to providing homehealth care and hospice servicesto the members of the community, we serve as the school nursesand the public health nurses forthe town of Wilton. We havebeen leasing office space at I-Parkfor the past ten years and priorto that, we were housed at theComstock Community Center forseveral decades. This is our firstopportunity to purchase a building for a more permanent homeand we will move into it thiswinter.
