While New York has “re-opened” and “recovered,” the emotional and physicalimpact of the pandemic has taken an extraordinary toll on many of you—our essential heroes. However, we remain committed to providing youwith quality and nurturing child-care resources and programming. We encourage you to review the broad range of programs that CCF has madeavailable for your child(ren), from infancy to young adulthood. A detailed listing of these programs is provided in this Registration InformationBooklet, along with information on eligibility requirements, registration process, locations, hours and program specifics.With the current COVID-19 pandemic still in effect, we have implemented strategies for social distancing, identification and onsiteregistration controls and flexibilities. Like last year, we are encouraging you to register online through our secure website, beginningon Wednesday, August 25, 2021. If you need assistance or would like to speak with a dedicated online registration representative,call (646) 473-8367, Monday through Friday, from 9:00 am to 5:00 pm. Registration is simple: upload or email copies of yourtwo (2) most recent pay stubs, dated within the last 30 days; a copy of the Social Security card(s) of any child(ren) NOT alreadyregistered for CCF programs; and a copy of your 1199SEIU Health Benefits ID card, listing your children. Child Care Funds (CCF) IntroductionThe 1199SEIU/Employer Child Care Fund (CCF) was founded in 1992. In 1998, the Greater New York Nursing Home Associationadded child-care benefits, creating the 1199SEIU/Greater New York Child Care Fund (GNY-CCF). Given its history and experiencein offering quality programs, the 1199SEIU/City of New York Education, Child and Eldercare Fund (CNY-CCF) selected this entityto administer its child-care benefits. Together, the CCF, GNY-CCF and CNY-CCF are now known as the Child Care Funds (CCF).Employers contribute to the CCF to provide 1199SEIU members with child-care benefits. The contribution is negotiated duringcollective bargaining and, as a result, the employer pays a percentage of gross payroll into the Funds. The CCF provides a varietyof child-care benefits. They include after-school care, day care, summer day camp, and cultural arts, holiday and teen programs.Members must meet certain eligibility requirements in order to apply for CCF benefits.Eligibility and Consideration for CCF BenefitsTo apply for benefits, you must: Be employed at a participating 1199SEIU institution/agency/facility or collective bargaining unit on a full-time orpart-time basis.» CCF: two-fifths (2/5) of a workweek» GNY-CCF: three-fifths (3/5) of a workweek» CNY-CCF: minimum of 70 hours in a 28-day cycle Have passed the required probationary period at your place of employment by Monday, November 1, 2021:» CCF and GNY-CCF: 90-day probationary period» CNY-CCF: one-year probationary period CCF and GNY-CCF: Not be on workers’ compensation or short-term disability for more than 26 weeks by Monday,November 1, 2021. CNY-CCF: Not be on workers’ compensation for more than 26 weeks by Monday, November 1, 2021. Complete and submit the application and required documents by Monday, November 1, 2021, no later than 4:00 pm.Applications received after the deadline will not be considered. Required Documents for RegistrationTo register, you MUST bring or submit copies of the following documents. If you are missing any of the documents listed below,the CCF cannot register your children.Note: First-time applicants have different requirements from other applicants.For all applicants: Two (2) pay stubs are required. All pay stubs must be dated within 30 days of registration. If you do not have pay stubs, you must submit a letter from your employer.» Employer letter must include: Member’s name, last four (4) digits of member's SocialSecurity number, hourly pay rate, date of hire and weekly work hours. The expecteddate of return must be included for members on short-term disability or workers’compensation.Pay Stubs If you are on short-term disability (not applicable to CNY-CCF) or workers’ compensation,you MUST submit a copy of your last pay stub AND your most recent disability stub.If the pay stub is not available, a letter from your employer is required.For all applicants: The CCF cannot register any child, including newborn children, without aSocial Security number/card. There are no exceptions to this rule.Children’s SocialFor first-time applicants: A copy of the Social Security card of each child for whom you areapplying is required. Registering for CCF ProgramsRegistration PeriodRegistration for the CCF benefit occurs once a year. It begins on Wednesday, August 25, 2021, and continues through Monday,November 1, 2021, until 4:00 pm. You must submit an application on an annual basis to be considered for benefits in the following year. You must submit the required documents in order to start the registration process. Applications will not be accepted after the posted deadline. If you have any outstanding balances, you are not eligible to register for programs until your accounts have beenpaid in full. It is your responsibility to do the following: Choose programs that meet the needs of your family and register for programs that you are certain your childrenwill use; Apply for more than one program, as you may not receive your first choice; and Observe different programs and meet with providers to make informed decisions.Help with Choosing a Program that Is Right for Your ChildLet Child Care Resource and Referral (CCR&R) Services HelpChild Care Resource and Referral (CCR&R) Services helps families take the guesswork out of finding child-care, summer day campand after-school programs. If you are not approved for your first choice, it may be possible to receive your second choice. If monies are available, you may receive more than one benefit.There are no special exceptions made in the following cases: If there are two (2) CCF-eligible parents in the same family working at the same or at different institutions/agencies/facilities, each parent may apply for the programs of his/her choice. However, the CCF will not approve the samebenefit for one child. In the case of multiple births during a single pregnancy, for example, twins. Special Needs Child CareThe CCF works to ensure that children with special needs have access to all programs and services offered. If you are approved for abenefit for a child with special needs, you will receive a Program Status Packet indicating "next steps" with instructions for submittingeligibility documents.If you are currently using an unlicensed provider for special needs care (i.e., friends, neighbors or relatives—with the exception of aspouse or the child’s parent), you may be eligible for the reimbursement rate for a licensed provider (service provided by a person ora center licensed by or registered with the city, county or state).To be considered for the licensed reimbursement rate, you must provide the CCF with the following: A copy of your child’s current Individualized Education Plan (IEP), indicating diagnosis and recommended education plan; A doctor’s note, indicating diagnosis and treatment; and A notarized Provider Verification Form (provided by the CCF), indicating the amount paid and the special certifications/trainings of the caregiver.The above documents are used to determine if your child meets the CCF’s special-needs criteria.If you have questions or wish to submit documents for approval, contact CCR&R Services at (212) 564-2220. Members’ weekly co-payment is based on a sliding scale. Acceptance is determined by the CCF’s eligibilityguidelines, availability of space and your child’s age.FALC is specially designed to provide a nurturing educational environment that connects children to the natural and real world.Its Plant-A-Dream (PAD) curriculum celebrates the whole child and provides developmentally appropriate, museum-quality,socio-dramatic learning stations within the world of work for our Pre-K students, and strength-based sensorial learning centers fortoddler and pre-school students. Each area is equipped with relevant materials that foster physical, socio-emotional, cognitive andcultural development. Please note: FALC currently is not able to accept special-needs children.If your child receives the FALC benefit: None of your dependents will be eligible for other programs for that year, except for the Holiday Programs. The FALC benefit cannot be transferred to any other program. You must reapply annually if your child is currently in a subsidized slot.CCF trustees have established a maximum amount for all FALC subsidies.This site is located in the Bronx.If you are accepted to FALC, the CCF requires that you also apply for the Facilitated Enrollment Subsidy, if you are eligible.
