Welcome . . .You have been referred for aglaucoma or cataract consultationbecause you may have a problemwith your eyes that requires specialknowledge, testing and treatment.This brochure answers questionsabout Glaucoma Center of Michiganand what you can expect during yourvisit. If you have additional questions,please call us at (248) 356-0098.Our Specialists have over 80cumulative years of experienceproviding the highest quality ofcare for complex glaucoma andcataract problems.The Glaucoma Center of Michigan(GCM) is the first and only facilityin Michigan dedicated exclusivelyto the evaluation and treatment ofglaucoma and cataracts. We have over80 cumulative years of experience inproviding the highest quality of carefor complex glaucoma and cataractproblems.How long should yourconsultation last?The average consultation requires2 to 3 hours. If special testing orscheduling of surgery is required, youcan expect to spend an additionalhour at our facility. We make everyeffort to follow closely to our patients’scheduled times. Therefore, we askthat you arrive by your scheduledappointment time, and call us if youwill be unusually delayed.

What if you need toreschedule yourappointment?For the convenience of our otherpatients, we would appreciate24-hours notice of your cancellation.We generally can reschedule yourappointment within a few days.What can you expectduring your visit?You will first be asked to completeseveral forms providing us basicinformation about you, your eyes, anymedical problems, and your insurancecoverage. In addition to the name ofthe eye doctor who referred you tous, we would like the name, addressand phone number of your family orgeneral medical doctor. Our staff willassist you in completing the forms ifnecessary.When your registration has beencompleted, you will be directed fromour reception area to our examinationarea. One of our ophthalmictechnicians will review the reasonfor your consultation and take somepreliminary measurements of youreyes and your eyesight. Then you willsee the doctor, who will evaluate youreyes. Following that, you will havea visual field test which checks yourperipheral vision. The techniciansmay instill eye drops to dilate yourpupils. Dilation of the pupils is usuallynecessary in order to completelyexamine your eyes. It takes about 20to 30 minutes for your pupils to fullydilate after the drops have been given.When your pupils are fully dilated,you will again be examined by thedoctor. Do not hesitate to ask questionsabout your eye condition or itstreatment. Depending upon the natureof your problem, additional tests willbe performed the same day as yourconsultation. A complete report ofyour consultation and any test resultsare sent to both your eye doctor andgeneral medical doctor.

Will you be able todrive after your eyesare dilated?Dilating your pupils with eye dropsdoes cause temporary blurring ofvision and increased light sensitivity.You may be more comfortable havingsomeone accompany you to the officeso that he or she can drive you home.The major effect of the eye dropsgenerally wears off after 3 to 4 hours.We suggest you bring sunglasses.What types ofinsuranceare accepted?We participate in most medicalplans, including Blue Cross BlueShield and Medicare. Our patientaccount representatives will processthe appropriate insurance forms foryou. If you have an HMO insurance,it is your responsibility to bring thenecessary authorization form fromyour PCP’s (Primary Care Physician)office to your appointment.What should you bringwith you for your visit? Your eyeglasses. A list of medications that you take(including eye drops). The name and address of yourreferring eye doctor. The name, address and phonenumber of your general medical orfamily doctor. Information on your insurancecoverage or HMO/PPO referralforms. Someone to drive you home afteryour appointment. Remember, your appointment willlast 2 to 3 hours.

LES I. SIEGEL, M.D., F.A.C.S.Les I. Siegel, M.D., founder of theGlaucoma Center of Michigan, is anophthalmologist specializing in theconsultation and treatment of glaucomaand cataracts.Born in Detroit, Dr. Siegel attendedMichigan State University where hegraduated with High Honors whilereceiving his Bachelor of Science inPhysiology. He then attended the WayneState University School of Medicinewhere he received his M.D. with Honors.He completed his internship at WilliamBeaumont Hospital in Royal Oak, hisophthalmology residency at the KresgeEye Institute of Wayne State Universitywhere he served as Chief Resident and hisglaucoma fellowship at the WashingtonUniversity School of Medicine and BarnesHospital in St. Louis, Missouri.Dr. Siegel is on the medical staff atWilliam Beaumont Hospital in RoyalOak, Eye Surgery Center of Michiganaffiliated with St. John Hospital, DetroitMedical Center (DMC). He is onthe teaching faculty at those medicalcenters as well as an Assistant ClinicalProfessor of Ophthalmology at WayneState University, Michigan StateUniversity and at Oakland UniversityWilliam Beaumont School of Medicine.Board Certified by the AmericanBoard of Ophthalmology, Dr. Siegelhas a professional affiliation with theAmerican Academy of Ophthalmology,American Glaucoma Society, theAssociation for Research in Vision,Michigan Ophthalmology Society, theAmerican Medical Association, andboth the Michigan State and OaklandCounty Medical Societies. In addition,he is a Fellow of the American Collegeof Surgeons (F.A.C.S.).Dr. Siegel has made numerouspresentations and written variouspublications on the diagnosis, treatmentand surgical procedures pertaining toglaucoma and cataract related subjects.He has a particular interest in combinedcataract surgery with glaucomalaser procedures (ECP) as well asincorporating the newest lens implanttechnology for his patients.

MARC J. SIEGEL, M.D., F.A.C.S.Marc J. Siegel, M.D. is anophthalmologist who specializes inmedical treatment of glaucoma andcataracts.A native Detroiter, Dr. Siegelreceived both his Bachelor of Scienceand M.D. from the University ofMichigan with High Honors.He completed a general surgeryinternship in Boston at Tufts NewEngland Medical Center. He servedhis residency in ophthalmology atMt. Sinai Medical Center in NewYork. Dr. Siegel then served as ChiefResident and completed his glaucomafellowship training at Mt. Sinai as well.He received the Heed Foundation andFight for Sight Fellowship Awards inGlaucoma.Currently, Dr. Siegel holds privilegesat William Beaumont Hospital in RoyalOak, Eye Surgery Center of Michiganaffiliated with St. John Hospital andDetroit Medical Center (DMC). Heis also an Assistant Clinical Professorof Ophthalmology at Wayne StateUniversity, Michigan State Universityand the Oakland University WilliamBeaumont School of Medicine.Board Certified by the AmericanBoard of Ophthalmology, heincludes the American Academy ofOphthalmology, the Association forResearch in Vision, the MichiganOphthalmology Society, the AmericanMedical Association and both theMichigan State and Oakland CountyMedical Societies on his growing list ofprofessional affiliations. In addition, heis a Fellow of the American College ofSurgeons (F.A.C.S.).Dr. Marc Siegel has had multiplescientific publications and presentationson topics relating to glaucoma diagnosisand management. He has a particularinterest in complex glaucoma andcataract surgical problems as wellas new, innovative astigmatic andaccommodating implants for cataractsurgery. He also has extensive glaucomasurgical experience with trabeculectomy,express shunts and Baerveldt implants.

MATTHEW E. CITRON, D.O., F.A.C.S.Dr. Matthew E. Citron, is anophthalmologist who specializes in theconsultation and treatment of glaucoma andcataracts.A long time Detroit area resident,Dr. Citron received a B.A. in History atUniversity of Michigan. He then achieved hisDoctor of Osteopathic Medicine degree fromMichigan State University with the honor ofmost outstanding surgical extern. Dr. Citroncompleted his internship in internal medicinethrough Michigan State University whileserving at Botsford Hospital.Following that, he served as Co-ChiefResident in Ophthalmology throughMichigan State University at the DetroitOsteopathic Ophthalmology Consortiumin Detroit, Michigan. He then went on toserve as the Clinical Glaucoma Fellow atthe Wayne State University’s Kresge EyeInstitute in Detroit, Michigan.Dr. Citron is an Assistant Professorof Ophthalmology at Michigan StateUniversity, Oakland University WilliamBeaumont School of Medicine and aninstructor of Glaucoma at the BeaumontEye Institute. Prior to joining GlaucomaCenter of Michigan, he was at the WayneState University’s Kresge Eye Institute asan Assistant Professor of Ophthalmologyand Local Residency Program Director atSinai-Grace Hospital.He is currently on staff at WilliamBeaumont Hospital in Royal Oak,Eye Surgery Center of Michigan,St. John Hospital, the Detroit MedicalCenter (DMC), Botsford Hospital andProvidence Hospital.Board Certified by the AmericanOsteopathic Board of Ophthalmology,he includes American Academy ofOphthalmology, American GlaucomaSociety, American College of OsteopathicOphthalmology, American College ofSurgeons, American Society of Cataract &Refractive Surgery, American OsteopathicAssociation, American MedicalAssociation and both Michigan State andOakland County Medical Societies. MICHAEL J. SIEGEL, M.D.Michael J. Siegel, M.D. is anophthalmologist specializing in themanagement and treatment of glaucomaand cataracts.A native of the Detroit area, he attendedMichigan State University’s Honors Collegewhere he graduated cum laude with aBachelor of Arts in History. Following ayear of research at the Harvard Institute ofMedicine in Boston, he received his M.D.from the Wayne State University School ofMedicine in Detroit.Dr. Siegel completed his internship atSt. John Hospital & Medical Center inDetroit and his ophthalmology residencyat the Kresge Eye Institute at Wayne StateUniversity, where he served as ChiefResident. Following his residency,Dr. Siegel completed his glaucomafellowship at Mt. Sinai Hospital inNew York.Prior to joining the Glaucoma Center ofMichigan, Dr. Siegel spent a post-fellowshipyear at Allegheny General Hospital inPittsburgh. Under the guidance of Garry P.Condon, M.D., he focused on the surgicalmanagement of complicated anterior segmentissues, cataracts and glaucoma.Dr. Siegel is an Assistant ClinicalProfessor of Ophthalmology at both WayneState University School of Medicine andOakland University/William BeaumontSchool of Medicine. He is Co-Chief of theglaucoma service at Oakland University/William Beaumont School of MedicineDepartment of Ophthalmology and part ofthe teaching faculty of both the Kresge EyeInstitute and the Beaumont Eye Institute,where he works weekly with the residents.He is on the medical staff of the DetroitMedical Center (DMC), William BeaumontHospital, St. John Hospital and is a partof the Wayne State University PhysiciansGroup. He also holds operative privilegesthrough the Eye Surgery Center of Michigan.Dr. Michael J. Siegel has written for peerreviewed publications and made numerouspresentations nationally. He has a specialinterest in resident education as well as in thecomplex surgical management of combinedcataract and glaucoma issues. Board certifiedby the American Board of Ophthalmology,he is a member of the American Academyof Ophthalmology (AAO) and a memberof the American Glaucoma Society (AGS).He also is a member of the Association forVision Research (ARVO), American MedicalSociety, Michigan Society of Eye Physiciansand Surgeons (MiSEPS) and Michigan StateMedical Society (MSMS).

Where is GCM located?Handicap parking is directly acrossfrom the building entrance. Thereis ample parking in front (east) ofthe building and in the multi-levelparking structure west of the building.Enter the building, and our office isconveniently located just to the right inSuite 102.Our office to serve Eastside patientsis located on the south side of TwelveMile, just east of Little Mack andapproximately 1/4 mile east of I-94 inthe St. John Surgery Center complex.After entering through the Westentrance doors, GCM is the first officeon the right. Ample free parking is alsoavailable.Southfield OfficeGlaucoma Center of Michigannow has three convenient offices. Ourflagship Southfield office is locatedin the Comerica Building near thenorthwest corner of Telegraph Roadand West Twelve Mile Road, just northof the Northwestern Highway/I-96interchange. It is accessible fromeither Twelve Mile or Telegraph Road.Handicap ramps and ample parking areavailable. Inside, elevators take youto our third floor suite. After exitingthe elevator, turn right and proceeddirectly into our office.Eastside OfficeOur Dearborn location serves theWestern Wayne and Downriver areas.It is in the 3rd large building (calledEastpointe) on Garrison which leadsoff of Outer Drive.
