Should you require more than 60continuous days hospitalization, ManhattanLife will pay the coinsurance amounts up to the150th** day of confinement and also for thefirst 3 pints of blood each year. Additionally, ifyou use your lifetime reserve days, ManhattanLife will provide coverage for up to an additional365 days.Part B Coinsurance pays the Medicare PartB coinsurance amount, reducing your out-ofpocket expenses when you require medicalservices. Your Manhattan Life BenefitsMedicare Part A Hospital CoverageDeductible - Manhattan Life Plans B, C, D, F, G and Nall pay the 1,288 inpatient hospital deductible foreach benefit period. Plan M pays 50% of the Part ADeductible.First 60 Days - After the Part A deductible, Medicarepays all eligible expenses for services from your 1stthrough 60th day of hospital confinement. Servicesinclude semi-private room and board, generalnursing and miscellaneous hospital services andsupplies.Coinsurance - All Manhattan Life Plans pay up to 322 a day when you’re hospitalized from the 61stthrough the 90th day. And when you’re in thehospital from the 91st through 150th day, ManhattanLife Plans pay you up to 644 a day for each LifetimeReserve day used.Extended Hospital Coverage - When you’re in thehospital longer than 150 days during a Benefit Period,and you’ve exhausted your 60 Medicare LifetimeReserve days, all Manhattan Life Plans pay the Part AMedicare eligible expenses for hospitalization, paidat the Prospective Payment System (PPS) rate orother appropriate standard of payment, subject to alifetime maximum benefit of an additional 365 days.Benefit for Blood - Medicare has one calendar yeardeductible for blood that is the cost of the first threepints needed. All Manhattan Life Plans pay thisdeductible.Skilled Nursing Facility CareFirst 20 Days - Medicare pays all eligible expenses.Coinsurance - Manhattan Life Plans C, D, F, G, M andN pay up to 161 a day from the 21st through the100th day during which you receive skilled nursingcare. You must enter a Medicare-certified skillednursing facility within 30 days of being hospitalizedfor at least three days.Hospice Care - After you meet Medicare’srequirements, including a doctor’s certification ofterminal illness, Medicare pays all but very limitedco-payment or coinsurance for outpatient drugsand inpatient respite care. Manhattan Life Planspay the Medicare co-payment or coinsurance.ML-MEDSUP 2016Medicare Part B Physician’s Servicesand SuppliesDeductible - Manhattan Life Plans C and F pay the 166 calendar year deductible.Coinsurance - After the Part B deductible, allManhattan Life Plans generally pay 20% of Medicareeligible expenses for physician’s services, hospitaloutpatient services and supplies, physical and speechtherapy, and ambulance service. Plan N requires theinsured to pay a portion of Part B coinsurance orco-payments: up to 20 co-payment for each coveredoffice visit, including specialists, and up to 50co-payment for each covered Emergency Room (ER)visit. The ER co-payment will be waived if admittedto any Hospital and the ER visit is covered as a PartA Expense.Excess Benefits - Your bill for Part B services andsupplies may exceed the Medicare Eligible Expense.When that occurs, Manhattan Life Plan F and G pay100% of the difference, up to the charge limitationestablished by Medicare.Benefit for Blood - Medicare has one calendar yeardeductible for blood that is the cost of the first threepints needed. All Manhattan Life Plans pay thisdeductible.Additional BenefitsEmergency Care Received Outside the U.S. After you pay a 250 calendar year deductible,Manhattan Life Plans C, D, F, G, M and N payyou 80% of eligible billed expenses incurred foremergency care that began during the first 60consecutive days of each trip outside the U.S.,that would have otherwise have been covered byMedicare if provided in the U.S., up to a lifetimemaximum of 50,000. Benefits are payable foremergency health care you need immediatelybecause of a covered injury or illness of sudden andunexpected onset.

Your Plan - The FactsManhattan Life helps pay some eligible expensesnot paid for by Medicare Part A and Medicare PartB. There may be charges above what Medicare andManhattan Life pay.Medicare Part A eligible expenses for hospital/skilled nursing facility care include expenses forsemi-private room and board, general nursing,miscellaneous services and supplies.Medicare Part B eligible expenses for medicalservices include expenses for physician’s services,hospital outpatient services and supplies, physicaland speech therapy, and ambulance service.Medicare eligible expenses means expenses of thekind covered by Medicare, to the extent recognizedas reasonable and medically necessary by Medicare.A benefit period begins the first full day you arehospitalized and ends when you have not been ina hospital or skilled nursing facility for 60 days in arow.Coinsurance is the portion of the Medicare eligibleexpense you have to pay. It does not include Part Aand B deductible amounts not paid by Medicare.As Medicare deductibles and coinsurance increase,your Manhattan Life benefits will automaticallyincrease. Manhattan Life benefits will not duplicatebenefits paid by Medicare.Benefits are paid to you or to your hospital or doctor.You have 31 days from your renewal date to payyour premium. Your policy will stay in force duringthis 31 day grace period.Your policy is guaranteed renewable. Your policycannot be cancelled. It will be renewed as long asthe premiums are paid on time.ML-MEDSUP 2016Premium rate adjustments may be made based oncurrent health care cost experience for benefitspaid. Manhattan Life reserves the right to establishnew premium rates for all insureds based on a classbasis, but only after giving you advance notice.However, we will not increase premiums basedon your own claims. Rates may be based on yourage and premiums may increase automatically oneach policy anniversary date, based on the age youattain*.You’re covered immediately. There is no waitingperiod for pre-existing conditions. Benefits will bepaid from the time your policy is in force.Manhattan Life Medicare Supplement Plans will notpay for: Expenses incurred while the policy is not in forceexcept as provided in the Extension of Benefitssection; Hospital or skilled nursing facility confinementincurred during a Medicare Part A benefit periodthat begins while the policy is not in force; That portion of any expense incurred which ispaid for by Medicare; Services for non-Medicare eligible expensesincluding, but not limited to, routine exams,take-home drugs and eye refractions; Services for which a charge is not normally madein the absence of insurance; or Loss or expense that is payable under anyother Medicare Supplement insurance policy orcertificate.THIS IS A BRIEF DESCRIPTION of your coverage.
