The Chief AcademicOfficer is responsible for accepting students and determining whether applicants, including thosewith special needs, can benefit from the training.FACILITY AND EQUIPMENT DESCRIPTIONJacksonville Health & Beauty School is in the city of Jacksonville, Florida at 5161 Beach Blvd. Suite 1& 2, Jacksonville, FL 32207. It is easily accessible by public or private transportation, with adequatefacilities for faculty and students. Our facility occupies approximately 3600 square feet and containsa reception area, 2 skincare clinic rooms, 2 restrooms, two study areas with a small referencelibrary, 10 nail stations, 3 pedicure chairs and 4 large manicure tables that accommodate up to 8. Theschool is fully air-conditioned to provide a comfortable learning environment.The latest equipment is available on-site to allow a hands-on training approach in all programs. Thefacility and equipment used fully complies with all federal, state and local ordinances andregulations, including requirements for fire safety, building safety, handicapped access and health.LIBRARYThe school has a small reference library on campus open to students. Students have access toreference texts and materials useful in completing the program of study. Materials are to be used oncampus and are available during normal class hours. A dictionary and thesaurus are vauilable uponrequest. These textbooks provide additional information on essential skills and procedures withbibliographical references and indexes, and will complement our programs by practicallysupporting and reinforcing the instruction given in class. Students may check out reference booksby completing a “Book Checkout Form”. All books and publications are of current use. ADMINISTRATION AND FACULTY MEMBERSTuyet-Nga T. Nguyen is the owner and Chief Executive Officer and Director of Student Services witha Bachelor of Science degree in Hospitality from Saigon University. She is a licensed full specialisttechnician (FS864182) with seventeen years of experience in the salon/spa industry whograduated from Hollywood Nails in Orlando, Florida. She will teach Facial Specialty, Full Specialtyand Nail Specialty.Mee Ja Kim is the Chief Academic Officer. In 1989 she graduated from MBC Academy in Seoul Koreawith a diploma in cosmetology. She is a licensure cosmetologist (CL1202186) with 27 years ofexperience in the beauty industry who graduated from the Academy of Health and Beauty inOrlando, Florida. She will teach Cosmetology.Cam Van Thi Nguyen will teach Facial Specialty, Full Specialty and Nail Specialty. She is a licensedfull specialist technician (FS884043) with five years of experience in the salon/spa industry whograduated from the Academy of Health and Beauty in Orlando, Florida.Long Phi Nguyen will teach Facial Specialty, Full Specialty and Nail Specialty. He is a licensed fullspecialist technician (FS861029) with five years of experience in the salon/spa industry whograduated from the Hollywood Nail school in Orlando, Florida.All administrators and faculty members are full-time.DRUG-FREE POLICYFor the protection and welfare of all students and staff, the school has established the followingdrug-free policy. All students are hereby notified:1. That the unlawful manufacture, distribution, possession, or use of a controlled substance in theschool is prohibited;2. That violations of this prohibition will result in discharge or other appropriate actions;3. That as a condition of enrollment, each student agrees that he/she will abide by the terms of theabove statement, and will notify the Chief Executive Officer of any criminal drug statute convictionfor a violation occurring in school no later than five days after such conviction;4. All employees and students must certify that, as a condition of enrollment, or employmenthe/she will not engage in the unlawful manufacture, distribution, dispensation, or the use of acontrolled substance.Potential students with certain felony convictions should contact the Florida Board of Cosmetology;Department of Professional & Business Regulations to determine their eligibility to obtain licensureprior to enrollment at the Academy. Requirements: Cosmetology.ADMISSIONS POLICY Applicants for Cosmetology course must present evidence of a high school diploma, highschool certification, or GED. Applicants who do not have a high school diploma, high school certification or GED and are17 years of age or older must demonstrate the ability to learn from the program ofinstruction. Applicants must pass the Wonderlic Basic Skills Test with a minimum verbalscore of 200 and a minimum math score of 210. Applicants must receive a catalog one week prior to signing an enrollment agreement. Applicants must comply with the "Drug Free" School policy.The Chief Executive Officer has the responsibility to review and approve an enrollment agreementand to ensure that the applicant has met the proper criteria in accordance with admissionstandards. The professions of Cosmetology, Full Specialist, Facial Specialist and Nail Specialistrequire prolonged hours of standing and various manual movements of the hands that can exertpressure. Nail Specialists work long hours often sitting and in the presence of strong chemical odorsand vapors. Full Specialists tend to incorporate most of the above. Correct posture is of extremeimportance for the physical health of the professional. Safety requirements entail diligentsterilization of tools and equipment used, good personal hygiene, and in some cases the use ofgloves and facial masks.THE APPLICATION PROCESSThe application for admissions process is as follows:1)2)3)4)5)Complete an admissions application.Complete a personal interview with an admissions representative.Tour the school facilities with an admissions representative.When necessary, take a Wonderlic test.Receive and read all required pre-enrollment disclosures. TRANSFER OF CREDITJacksonville Health & Beauty School has the right to accept or deny the transfer of clockhours/credits received from another school. The granting of credit for prior learning or examscannot exceed seventy five percent (75%) of any program. Programs and tuition will be adjustedaccording to the number of hours accepted by Jacksonville Health & Beauty School. Any studentwho began a program at Jacksonville Health & Beauty School and wishes to transfer to anotherprogram will receive credit for courses passed and have his/her tuition adjusted accordingly.Students who transfer out of Jacksonville Health & Beauty School to another institution may receivetheir transcript reflecting their hours and grades, if all financial obligations to Jacksonville Health &Beauty School have been met. The acceptance of the transferability of credits is the decision of thereceiving institution. It is the student’s responsibility to confirm whether or not credits will beaccepted by another institution of the student’s choice. Jacksonville Health & Beauty School doesnot recognize acquired life experience and prior experiential learning as a consideration forenrollment or granting credit towards any of its programs. Jacksonville Health & Beauty Schooldoes not have an articulation agreement or transfer agreement with any other school, college oruniversity.TRANSFER CREDIT POLICY/PROCEDUREJacksonville Health & Beauty School will consider credits from other licensured institutions. Creditstaken at other institutions of higher learning may be accepted as ‘transfer credit’, if these coursesare in subjects comparable to courses offered and applicable. ‘Transfer credit’ is given for courseswith a “C” or better. Transfer credit must be approved by the Chief Executive Officer. Only officialtranscripts from other schools may be used to award credit. Students must request that an officialtranscript from ALL previously attended institutions be mailed to: Jacksonville Health & BeautySchool 5161 Beach Blvd., Suite 1 & 2, Jacksonville FL 32207. Complete and submit a Transfer of Credit Application to the Chief Executive Officer. Provide an official transcript and catalog with course descriptions of the prior postsecondary training. Official transcripts are required to post transfer credits. Courses from licensed post-secondary training programs that correspond directly incontent, scope and length to Jacksonville Health & Beauty School courses will be consideredfor credit. Only training courses with a grade of C or 2.0 or above will be considered for credit. The Chief Executive Officer’s decision will be final. ACADEMIC POLICIESGRADING SYSTEMPractical work is graded based on written criteria using school-prepared practical evaluation forms.The form specifies the grading criteria and point value for each application. Each skill/applicationhas a specific point value assigned to it, which is then totaled and converted to a percentage.Students are graded on theoretical knowledge at the end of each course. A final examination will begiven at the completion of each program. The exam will consist of two (2) parts: theory andpractical. Both portions of the examination are graded according to the following scale:A 94% - 100%4.00 ExcellentC 75% - 80%2.00 SatisfactoryBDF 81% - 93%70% - 74%0% - 69%3.001.000.00 GoodPoorFailingSATISFACTORY PROGRESS STATEMENTStudents will be judged as making satisfactory progress by maintaining a cumulative grade pointaverage (GPA) of 2.0 at the end of each scheduled evaluation point of their program. To avoidacademic probation, students must maintain a minimum GPA of 75% or a C. Students maintainingless than a 75% GPA shall be counseled by their instructor, which will be noted on their progressreport. Students receive a copy of their satisfactory progress report after each evaluation point.Students not meeting making satisfactory progress will be scheduled for a conference with the ChiefExecutive Officer, which will result in probation. COURSE RETAKE POLICYThe course retake is offered to students who have unsuccessfully completed a course and receivedless than 75%. A student may retake a course twice at no charge. A student will automatically bewithdrawn after failing any course in the program twice during the one enrollment period.MAXIMUM TIME IN WHICH TO COMPLETEStudents are not allowed to attempt more than 1.5 times, or 150%, of the number of clock hours intheir program of study. The requirements for the rate of progress are to assure that students areprogressing at a rate at which they will complete their programs within the maximum time frame.MEASUREMENT OF ACADEMIC PROGRAMSThe school measures all its academic programs in terms of clock hours. A clock hour is defined as aminimum of 50 minutes of instruction during a 60 minute period in the presence of an instructor.ATTENDANCEStudents are expected to attend all scheduled classes. In order to obtain permission for an excusedabsence, the student must obtain prior permission from the instructor. For emergency situations, astudent may bring in a doctor’s note, a police report, a court order, or any other legitimate excuse inorder to obtain an excused absence. All other absences are considered unexcused.Students may not have more than five unexcused absences within a calendar month. After fiveunexcused absences, the student will be advised by the Chief Executive Officer and will be placed onprobation for 30 days. If another unexcused absence occurs during the probation period, thestudent will be terminated. The State of Florida requires students to complete a set number ofhours to graduate; therefore, all class time missed must be made up by the student (prior tograduation).TARDINESSStudents must come to class on time. Tardiness is defined as arriving more than 15 minutes afterthe start of a regularly scheduled class. Late arrivals may be excused at the discretion of theinstructor. Once a student has three (3) unexcused late arrivals, the student must be formallycounseled. A counseling session will determine what actions should be taken by the student and theschool to reduce tardiness. Habitual tardiness (more than six (6) unexcused late arrivals) will leadto probation.Revised 03/01/2215 P a g e

MAKE-UP WORKStudents who have been absent for any reason are required to make up any missed classes beforegraduating. A student must make up missed time by attending another class in session.LEAVE OF ABSENCEA student may be granted a leave of absence for a reasonable amount of time as determined by theChief Executive Officer. The total leave of absence period may not exceed 180 days. A request for aleave of absence must be in writing, and the date of expected return must be specified. If a studentdoes not return within the specified return date and did not notify the School, the student’s contractwill be terminated on the date of the scheduled return.UNOFFICIAL WITHDRAWALIf the student fails to attend school for more than 10 consecutive days, the school will consider thestudent a drop and automatically withdraw him/her from the program.AUTOMATIC WITHDRAWALA student will automatically be withdrawn from the program for the following reasons: Failure to attend school for 10 consecutive class days.Failure to return from an approved leave of absence on the scheduled return date.Failure to maintain satisfactory progress for two consecutive modules.Failing any course in the program twice during one enrollment period.PROBATIONGrades: Students who have unsuccessfully completed a course and received less than 75% will beplaced on probation.
