The IHI Triple Aim is a framework that describes anapproach to optimizing health system performance. The IHI believes new designs must be developed to simultaneouslypursue three dimensions: Improving the patient experience of care (including quality and satisfaction); Improving thehealth of populations; and Reducing the per capita cost of health care. This presentation provides an overview of theChinle Service Units’ experiences with embracing the IHI Triple AIM Collaborative redesign concepts.After attending this activity participants will be able to: Assess own health care facility for opportunities to improve the patient experience; improve care, and reduce costs. This session willprovide participating leaders of IHS, Tribal and Urban health care facilities with an overview of the organizationalplanning and preparation steps in order to facilitate the implementation of the 2014 Certified Electronic Health Record,ensure compliance with Stage 2 Meaningful Use requirements, and prepare for the transition to ICD-10 coding.After attending this activity participants will be able to: Initiate activities to “clean up” RPMS EHR problem lists in preparation for the 2014 Certified EHR. Plan and implement a Clinical Documentation Improvement activity at their facility in preparation forICD-10.
