Key PointPolio (IPV/OPV) The fourth dose of polio vaccine should be givenon or after the child’s fourth birthday and prior toentry into kindergarten.Effective school year 2011/2012 for kindergartenentry only, if the fourth dose of polio vaccine wasadministered prior to the fourth birthday, a fifthdose of polio is required.If the third dose of polio is administered on or afterthe fourth birthday, then the fourth polio dose isnot required.Students who started their immunizations afterseven years of age should receive a total of threedoses of polio vaccine.

Key PointTdapTetanus, diphtheria, and pertussis Studentswho started their immunizationsafter seven years of age should receive atotal of three doses of adult tetanusdiphtheria (Td) vaccine with Tdapreplacing one dose of the Td. This Tdap dose will meet the 7th graderequirement.

Key PointTemporary Medical Exemption(DH 680 Form Part B) Anychild who has incompletedocumentation of vaccination for therequired number of doses should beadmitted after the first dose(s) and issueda Temporary Medical Exemption (DH 680Form Part B) and scheduled for the nextdose(s) according to age and dosagespacing.

Key PointImmunizations and School Entry According to Florida School Health AdministrativeGuidelines, Section III, Chapter 9, certification ofimmunization or exemption is required of all studentsPRIOR to admittance or attendance in the publicand non public school. This will be in the form of: DH Form 680 Part A, B, or CDH Form 680 Certification of ImmunizationDH Form 681 Religious ExemptionStudents who are experiencing homelessness shall begiven a temporary exemption for 30 school days to complywith school entry requirements.An authorized school official may issue a temporaryexemption for a period not to exceed 30 days, to permitstudents who transfer into a new country to attend classuntil his/her records can be obtained (s. 1003.22(5)(e),F.S.)

Key PointWhat do I do if the DH680 form isincomplete, expired, or is notpresented for entry into school? Accordingto Florida School HealthAdministrative Guidelines, Section III,Chapter 9: Mandatory exclusion from school is requiredby law in Florida until acceptableimmunization documentation is presented.
