Communities with the lowest income and educationallevels experience the greatest limitations to accessing oralhealth care. Oral health in America: A Report of the SurgeonGeneral (2000) refers to this as the as the “Profound andconsequential disparities of the oral health care of our citizens.”He likens this to a “silent epidemic” and an “oral health crisis.”Critical to the solution of the oral health disparities in thiscountry will be providing increased educational opportunitieswithin our communities for prospective graduates of anaccredited Dental Hygiene program. Toward that end, the goalof the Dental Hygiene Program at Fortis College is to educateoral health care professionals capable of providing preventiveand therapeutic oral health care services in diverse communities.PROGRAM AND POLICY CHANGESFortis College reserves the right to make changes in organizationalstructure, policies and procedures, equipment and materials, andmodify the curriculum as circumstances dictate. Students are expected to be familiar with theinformation presented in this Catalog and applicable StudentHandbooks.FACILITIES AND EQUIPMENTCRITICAL STRENGTHS OF FORTIS COLLEGEFortis College occupies a 30,000 square-foot facility that is locatedat 3949 South 700 East, Suite 150 and 200, Salt Lake City, Utah,84107.Career-oriented programs: The College’s programs have beendeveloped and are periodically reviewed in conjunction withindustry advisory boards to ensure that they continue to preparegraduates according to current needs and expectations of thecommunity of employers served by Fortis College.The Medical Training Lab is equipped with microscopes,stethoscopes, blood pressure cuffs, EKG machines, a spirometer,an autoclave, surgical instruments, examination tables, and injectionarms/skins, and a variety of CLIA waived testing supplies andequipment.Qualified, caring faculty: In their academic credentials andprofessional experience, faculty members are qualified to teachthe courses assigned to them, and all are committed toproviding the extra assistance students may need to achieve theircareer goals.The Nursing Skills Lab is equipped with low fidelity mannequinsand a simulation lab that simulates actual patient scenarios withhigh fidelity mannequins, camera system, fully replicated patientrooms, and instructional debriefing rooms.The Dental Assisting and Dental Hygiene programs also have a fullyoperational clinic equipped with 15 operatories, sterilization centers,dental materials lab, panoramic x-ray machine, digital x-ray, dentallaser, dark room and reception area/front office training area.Graduate employment assistance: Students approachinggraduation receive, at no additional charge, career andemployment assistance in finding entry-level positions in theirpreferred careers. While the primary responsibility for securingsuch employment rests with the student, the Career ServicesDepartment is available for information, contacts, and guidance.Small classes and personal attention: A small student-to-facultyratio helps students obtain the most from their educationalinvestment by ensuring easy access to instructional equipmentand to attentive and helpful faculty.The following campus administrators should be consulted to-3-

2018-2019 COLLEGE CATALOGThe Heating, Ventilation, Air Conditioning, and Refrigeration Labincludes a boiler, hydronic heating systems, oil furnaces,electric furnaces, gas furnaces, air conditioning units, heatpumps, refrigeration units, HVACR library, and a tool storagearea. All AC condensers and heat pumps are located outsideon a dedicated patio which provides regular operatingconditions for these units. The HVACR program also has aresidential test facility in a nearby home with 9 separate heatingsystems and three AC systems installed, providing students withreal-world experience in a continuously-occupied home.Education Amendments Act of 1972, which prohibits discriminationon the basis of sex in any education program or activity receivingfederal financial assistance. TheConsumer Information Guide is available online at:The lecture rooms have internet connectivity, full wall-sizedwhiteboards, media cabinets, and LCD projectors, to enhancethe lectures with access to specialized medical web sites anduse of web-based materials. Library resources and libraryinformation services are available to students and faculty. Thelearning resource center is available with internet access,computer stations, web- based resources, health referencebooks, and periodicals. There is a student lounge with vendingmachines, a microwave, refrigerator, and student resourcematerials. Administrative offices include career services,financial aid, registrar, admissions, education, and businessoffice. There are interview rooms for student conferences.Clinical and externship sites are in area physician’s offices,hospitals, and other professional medical facilities. Theapplicant must provide documentation of graduation fromhigh school or college in the form of a valid high schooldiploma or higher earned degree, transcript, or otheracceptable documentation which confirms that theapplicant meets or exceeds the academic achievementequivalent to a standard high school diploma earned inthe USA as defined by the State where the diploma wasearned. All documents from foreign countries must also betranslated into English and evaluated to be equivalent orhigher than a USA high school diploma by a credentialevaluation service.Degree Programs:Associate Degree Dental AssistingAssociate Degree Dental HygieneAssociate Degree Medical Assisting& Basic X-Ray TechnicianAssociate Degree in Nursing13161316**Applicants to the Associate Degree in Nursing program whohold a current Vocational Nursing or Practical Nursing licenseor who are graduates of a Fortis College/Institute orMedVance Institute VN or PN program are exempt from takingthe Wonderlic SLE exam.2.The applicant must be seventeen years of age or older atthe time he or she starts his or her program of study. Theapplicant for a nursing program must be eighteen years ofage or older to be accepted into the program.3.The applicant must complete an applicant information form.4.The applicant must interview with anrepresentative and/or other administrative staff.admissions7.Applicants must pay the enrollment fee and complete all tuitionpayment requirements.5.Applicants, who otherwise meet the requirements to pursuea selected program of study, will be given the opportunity totake the Wonderlic Scholastic Level Exam (SLE).Applicants to the College who do not achieve a passingscore are eligible to immediately retake another version ofthe SLE. (See below for the SLE minimum scorerequirement for each program of study.) In the event thatthe applicant fails to achieve a passing score on thesecond administration of the SLE, the applicant is eligibleto take the SLE a third time using another version;however, a minimum of seven days must elapse after thesecond testing date before the third SLE may beadministered. In addition, this administration andsubsequent administrations requires approval by theCampus President. A fourth and final administration ofanother version of the SLE is permitted only after aminimum of 180 days have elapsed since the date of thethird test administration. Applicants who choose to take theentrance test for the fourth and final time are stronglyencouraged to pursue remediation in reading and mathprior to testing for the fourth and final time to assist instrengthening their critical thinking skills.8.Accepted applicants must agree to and sign the Fortis CollegeEnrollment Agreement.6.Note: In the event an applicant desires specialaccommodations for the SLE, the applicant must follow thepolicies in this catalog to request those accommodations.Campus staff members are not authorized to approveaccommodations for admissions testing.ADDITIONAL ADMISSIONS REQUIREMENTS FOR THEDENTAL HYGIENE PROGRAMIn addition to the General Admissions Requirements, the following areadditional admissions requirements for Dental Hygiene programs.1.After achieving a passing score on the SLE, the applicant will begiven the opportunity to take the HESI Evolve Reach AdmissionAssessment Exam in English and math. The proctoredexamination, must be monitored by a trained staff person whodoes not report within the admission department. An applicantmust achieve a combined score of 75 in order to be eligible foradmission to the Dental Hygiene Program.2.Applicants are also required to complete the Personality Profileand Learning Style components of the HESI Exam.3.Prior to taking the HESI, students are encouraged to purchase aHESI review workbook to prepare for the assessment. To obtainthe current edition of the HESI Evolve Reach AdmissionAssessment Exam Review, an applicant may visit the websiteOnce a passing score is earned, the SLE score is valid-5-

2018-2019 COLLEGE i.html or consult theDental Hygiene Program Director for information on how toorder the review workbook.4.If the applicant is dissatisfied with his or her HESI scores afterthe first exam attempt, he or she will be directed by theprogram director to complete the HESI online remediationsession, before being allowed to take the exam again. The reexamination can be administered during one of the regularlyscheduled examination sessions on campus. The score usedin the admission’s decision and will be the higher of the twoscores. Students are allowed a total of two attempts at takingthe HESI exam within a twelve-month period.5.The applicant must complete a 2 page typed essay (doublespaced in New Times Roman 12) outlining (1) why theapplicant wants to enter into the Dental Hygiene professionand (2) why he or she should be admitted to the DentalHygiene program. The Dental Hygiene Program Director mayelect to include an additional topic. The essay may becompleted offsite. It must be submitted by email to the DentalHygiene Program Director or his or her designee no less thanone week prior to the interview with the program director or hisor her designee. After the essay is completed, the applicant willmeet with the program director. The program director or his orher designee scores the essay using the Essay Rubric.6.The applicant must schedule and complete an interview withthe Dental Hygiene Program Director and/or his or herdesignee.7.The applicant must submit a current and valid governmentissued form of picture identification. For example, a current andvalid driver’s license or U.S Passport.8.9.The applicant must submit either a valid medical insurancecard or a sign a medical waiver form stating he or she isresponsible for the costs of all medical services he or sherequires.Applicants must submit to a drug-screening test and the resultmust be negative in order to pass. The results must be in theapplicant's admission file before starting the program.Inconclusive test results (such as dilute sample and insufficientsample) will require the applicant to be retested at his/herexpense. The retest must be completed within 24 hours ofreceiving the results on the first test. A second inconclusivetest result (such as a dilute sample and insufficient sample) willbe considered a positive drug test Applicants who test positiveon a drug screening test will not be eligible for admission intothe program and are not eligible to reapply.10. Applicants must submit to and pass a healthcare studentcriminal background check prior to starting the program.Results must be in the applicant’s admission file before startingthe program. The applicant should note that a history ofcriminal convictions may prevent the student from attending orcompleting the clinical/externship requirements of the programor may prevent his or her credentialing in the profession. Ifthere are any questions regarding the results of the criminalbackground check, the applicant must make an appointmentwith the program director to discuss the consequences of theresults.12. When all the above admissions requirements are completed withthe exception of immunizations as noted, the Applicant RatingForm and Rubric along with the application file will be submittedby the Director of Admissions to the Director of the DentalHygiene Program for consideration by the Dental HygieneAdmissions Committee. The Dental Hygiene AdmissionsCommittee consists of the Dental Hygiene Program Director or hisor her designee as the chairperson of this committee, designatedDental Hygiene or other faculty member(s), and the CollegePresident or his or her designee. The primary purposes of theDental Hygiene Program Admissions Committee are: (1) to rankthe applicants and (2) to make the final decision as to whichapplicants are best suited for the Dental Hygiene program. Thisincludes selecting applicants who are most likely to be successfulweighing the applicant’s background and experiences with thestandards of the profession and school.
