COMMUNITY &HOME SERVICESPARAMEDICS PROVIDINGPALLIATIVE CAREPROGRAMIn this program, trained paramedics are able toprovide care to patients within their home whenthey are having an emergency associated with theircondition, such as pain or symptom control.The goal of the program is to support people livingwith life limiting conditions by providing the carethey need at any time, day or night, and to assistthem to remain comfortable at home, if this is whatthey wish, for as long as possible. The program isoperational in the St. John’s metro, with plans forexpansion to other regions in 2021. The focus of palliative care is toaddress symptoms, and concerns that impact the person’squality of life. It is delivered by a health care team and caninclude: Relief of pain and other symptoms causing discomfort Advance care planning/goals of care discussions Support for family and informal caregivers Support of cultural needs Support for emotional, social and spiritual needs Counselling and grief support.Palliative care is not only for the end stage orlast days of an illness. ENDOF LIFEAT HOMEIn the Western Region, palliative and endof life care services are also available forpeople who wish to stay at home.End of Life Care is the part of palliative care that is providedto clients in their last 8 weeks of life. It is an enhancedservice in which the health care teamworks togetherto manage the illness, improve comfort, and enable theperson to stay at home for as long as possible or to die athome if that is their wish. ADDITIONAL SUPPORT SERVICESMAID- Medical Assistance in DyingMedical assistance in dying became legal in June2016 and is governed under federal law. It is theadministering by a doctor or nurse practitionerof a substance to a patient, at their request, thatcauses their death or, the prescribing or provisionby a doctor or nurse practitioner of a substance toa patient, at their request, so that they may selfadminister the substance and in doing so causetheir own death.Although MAIDis not offered by the palliative careteam, it can be part of end of life discussions. Apatient can request information about MAIDfromany member of their health care team.
