The OB team works alongside the CommunityHealth advocates to connect patients with services and resources inthe community. Additionally, the department provides mental health andsubstance use prevention education, offers services and supportwithin primary care for patients contemplating services, providespatients with an intake appointment, engages patients who completed an intake into treatment services, and provides medicationassisted treatments and monitoring.The staff is a multidisciplinary team consisting behavioral healthand social service clinicians, including a psychiatrist (for patients18 years of age and older), psychologist, Community Advocates,Community Violence Intervention Program staff and domesticviolence staff.7

Community Health Advocates/Extension 8142Advocates can help those who have issues or need assistance withhousing, legal services, immigration issues, child support, healthinsurance, emergency aid, etc. They are a group of paraprofessionals that are multi-cultural and multi-lingual serving health centerpatients.Infant Car Seat Program/Extension 8381We can provide car seats to prenatal patients in their last trimesterof pregnancy. We offer infant/convertibles and high back boosterseats. Home Health Care/Visiting Nurse ProgramThe Home Health Care/Visiting Nurse program provides a widerange of services to homebound adults. A team of RegisteredNurses, Nurse Practitioners, Therapists and Home Health Aidesassess, plan and deliver care in collaboration with other communityproviders. This team helps to minimize unnecessary hospitalizations, shorten hospital stays and avoid nursing home placement,and maximize family support.This department provides skilled nursing, physical therapy,occupational therapy, speech therapy, home health aides,medical social services. This includes pediatric, teen, adult andprenatal/obstetric patients, as well as those receiving home healthand nursing visits. UCHC’s Nutrition Department providesoutpatient nutrition services which may also include Medical Nutrition Therapy (MNT), and diet and lifestyle education for avariety of diagnoses (e.g., cardiovascular disease, diabetes mellitus, gastrointestinal illness/GERD, HCV/HIV infection, hypertension, obesity) to all UCHC patients.9

The Nutrition Department also engages patients and families incooking demonstrations, walking groups and other activities inthe local community on an ongoing basis. SERVICES AT 636 COLUMBIA ROADDental ServicesUCHC’s dental department provides evidence based, high qualitydental care to a diverse patient population ranging in age from ninemonths to individuals in their late 80’s and beyond.The Dental Department provides emergency dental services,preventative dental care (teeth cleanings, sealants, fluoride treatments, oral hygiene instruction, and nutrition guidance), fillings forboth children and adults, complete and partial dentures, and minororal surgical procedures.The department’s array of services also includes root canals foranterior teeth, crowns, fabrication of both night and athletic mouthguards. Services involving surgical procedures related to full orpartial dentures can also be provided. Eye Care ServicesThe department offers optometry exams and glasses for both children and adults. Staff diagnoses and treats eye diseases such asglaucoma, cataracts, macular degeneration and retinal detachments. They also provide medical treatments for dry eyes,allergies and pink eye. UPHAM’S ELDER SERVICE PLAN/PACEOur mission is to provide comprehensive and continuous healthand social services that will support the independence and nurturethe spirit of frail, older individuals. In cooperation with communityand family caregivers, the PACE team provides interdisciplinarycare, with compassion, respect for dignity, and sensitivity to cultural differences.Upham’s Elder Service Plan (UESP) is a part of a national PACEProgram, a Program of All-inclusive Care for Elders, withcomprehensive health care and social services. It focuses oncoordinated, interdisciplinary geriatric care for people 55 years ofage and older, who have medical needs and want to continue tolive in the community for as long as possible. PATIENT SERVICESThere are certain services that will help patients manage their care,appointments and prescriptions. You should familiarize yourselfwith these as they will help with efficiency and patient satisfaction.My ChartMy Chart is a secure online health connection accessed through anapp. Through this tool, you can ask for medical advice, communicate with your doctor, schedule and view and manage appointments, get lab results and much more. In order to register for MyChart a patient must:1. Sign up in person at the clinic with the clinic staff or qualitynavigator;2. Sign up on their own with the code provided to them at theirappointment;3. Call 617-287-8000 ext 8233 to have a code generated for them.Televox (Text Messaging System)Patients can receive appointmet reminders by text message. Thisis available to patients that speak English and Spanish. They mustopt in for this service. They will only recieve notifications by textmessage only and not by phone. This is the opt-in information:TEXT “Uphams” to 622622 forRegular appointment remindersIVR (Messaging system for Pharmacy/Refills)The Interactive Voice Response system) let patients order theirrefills over the phone without talking to a live person. EMERGENCY CODES FOR UCHCCode Yellow – Physical or Verbal ConflictThe RESPONDER is responsible for: Assessing the situation on the scene carefully, Taking action by making sure that all staff members aresafe, Encouraging everyone to calm down, If the situation is confrontational, separating staff memberfrom patient, patient from patient, etc. If necessary, locating an interpreter to aid withcommunication, Calling outside authorities/emergency services, ifnecessary (i.e. panic button, if necessary, refer to Redbook for phone numbers). Advise immediate supervisor of incident and Filing an INCIDENT REPORT and send toExecutive DirectorCode Blue – Cardiac Arrest/Medical EmergenciesMedical emergencies can occur at any time and withoutwarning. Preparations for the unexpected, calmness and teamworkcan sometimes avoid or cut down on losses in emergencysituations. Examples of emergencies that could occur at a UCHCclinic or service site are: A person not breathing An unresponsive or unconscious person Someone bleeding profusely Someone who collapses for an unknown reason A person clutching their chest and having troublebreathing A person chokingWhen a concerned staff member or provider or other supportpersonnel (i.e. registered nurse, dental assistant, etc.) determinesthat a condition is an emergency, they are to immediately overheadpage, or tell the staff person closest to them to call, to announce21
