This presentation will provide abasic overview of the LEMSA’s role in EMT disciplinaryinvestigations and the step-by-step process to ensure theLEMSA’s investigation process can withstand scrutiny. This talk will highlightclinical applications of mobile stroke units, operationallogistics, geospatial mapping of operating locations and mostimportantly, integration into the fabric of EMS andprehospital care.3) Advanced Practitioner Response Units as aComponent of Mobile Integrated HealthSaman Kashani, MD, LA County FDThe traditional EMS model of “you call, we haul” is dated andinefficient. Increasing demand for 9-1-1 services needs tobe met with novel programs. The Los Angeles County FireDepartment’s (LACoFD) Advanced Provider Response Unit(APRU) Program partners a nurse practitioner with a firefighter/paramedic (FFPM) on a specialty unit that respondsto low-medium acuity calls for emergency medical services(EMS). The APRU is able to perform assessments and simpleinterventions in the pre-hospital setting and triage patientsto appropriate levels of follow-up based on the acuity of theircondition and access to healthcare services. In addition, theAPRU performs in-home assessments for high-risk patientsidentified by other department personnel and high systemutilizers. By working with these patients, the APRU programis able to identify and intervene on the social impedimentsto care and improve the patient’s health while reducing theirneed to access critical safety net services.

Historically, institutionshave failed to prepare first responders for this outcome, and“training” for these situations are experiential, at best. As aresult, crews continue to default into a “transport for the family,”“CPR” or “just for show” mentality that has only contributed tothe cultural resistance we face. Because of this practice, wehave failed to recognize our unique role in the grieving (healing)process, which begins for the survivors at the moment that wedetermine time of death of the patient. Keynote SpeakersSteve Berry, BA, NRP – Steve, a former teacher and interpreter for the hearing impaired, hasbeen in EMS for over 37 years and is currently a working paramedic and instructor for SouthwestTeller County EMS in Colorado. Believing humor is an essential amino acid in survivingprehospital care, Steve has written over 140 humor articles for JEMS along with cartoonillustrations for JEMS and EMS World magazines. He is the author of the cartoon book series,“I’m Not An Ambulance Driver”. As both a national and international speaker for emergencyhealth care providers, Steve seeks world peace and frequent flyer mileage.In a recent interview, he shared -- I would add that this kind of humor must be “earned and learned” by thoseentering the field. Many tears may have to be shed before this kind of humor can be appreciated and integrated.There is a balance, and laughing does not negate the need for crying. Not everything is a joke. Timing is everything.Most important, one should not apologize for their tears. It’s proof that you’re emotionally alive, and not just in it forthe tubes and IVs.Dan Sun – Daniel was born in Alberta, Canada. After taking up global residency as an Englishlanguage teacher in Egypt, Mexico, Indonesia and Japan, he became a firefighter and paramedicfor the City of Leduc. His love of photography began on his travels, and as his techniquedeveloped, he started to produce a blend of photography and graphic art, which he calls PhotoArt. The majority of Daniel’s work involves emergency services photography and an emotionalconnection to the artist as most of his pieces are based on actual emergency calls he hasattended as a paramedic and firefighter.He is passionate about reducing the stigma of PTSD for first responders and he uses his art to raise awareness ofoccupational stress injuries worldwide. He is a published artist/writer and has two art books produced of his works.Articles of his work have been features in Canadian Paramedicine, JEMS Fire Rescue Magazine in Greece and WMSWorld Magazine. His art is recognized worldwide and he travels internationally speaking on his art and personalexperiences with occupational stress injuries. He is a prehospital educator, a professional public speaker,photographer and photo editor. He enjoys combining the technical and artistic elements of his work. Each piece cantake several days or weeks of processing until he feels it is ready.
