/ Orthopaedics // Orthopaedics /may still require narcotic medications, they can be prescribedin much lower doses, for a shorter period of time. “This is alltailored to each patient,” Dr. Booth explains. “Patients havedifferent issues, so you have to customize a package of paincontrol options for them.”In addition to total knee replacement, there are a variety ofRichard A. Balderston, MD, is a national leader in spine researchand spinal surgery. Among the first to perform disc replacement inthe country, Dr. Balderston provides customized surgical and nonsurgical options for patients with spine and disc injuries.Dr. Booth performs close to 1,000 TKA surgeries every year,as well as several hundred knee replacement revisions. “Myexpertise is knee replacement,” he says. “I'm now doing what Ilove, which is total knee replacements.”Also a designer of artificial knees, Dr. Booth helped developthe first-ever artificial knee created specifically for women,which was approved by the U.S. Food and Drug Administrationin 2006. Developed in collaboration with Zimmer , aWarsaw, Indiana-based manufacturer and one of the leadingdistributors of joint replacement products and technologies,the Gender Solutions Knee takes into account the smallersize of the female knee joint and tracks at a different angle toaccommodate women's wider hips. “Our design conceptswere the first that accommodated new varieties of anatomy,”Dr. Booth notes.alternatives for patients with lesser joint destruction. Partialjoint replacement and remarkable technological advances incartilage restoration have resulted in many more options forthe younger, more physically demanding patient, Dr. Bartolozzisays. “Cartilage is the holy grail of orthopaedics and jointpreservation is our primary goal."Dr. Bartolozzi has served as the team physician for many of thePhiladelphia area’s professional teams, including the PhiladelphiaEagles and the Philadelphia Flyers. He has also served as aconsultant for the U.S. Soccer teams. While developing new andinnovative techniques for state-of-the-art ACL reconstruction, hehas helped innumerable active individuals follow their passion.In addition to implants that are more tailored to the patient,another advance in joint replacement has been the movetoward operating via smaller incisions, which helps patients healfaster, according to Dr. Booth. Advances in anesthesia have alsohelped patients start rehabilitation sooner, he says, and exercisetheir new joints back into shape more aggressively.Patients recovering from knee replacement surgery hadtraditionally been medicated with narcotics for pain relief,which can have a host of serious side effects. The teamof physicians at 3B are able to ease patients' pain moresafely—and effectively—by pre-medicating them with antiinflammatory drugs, and administering nerve blocks duringthe operation that remain in place afterwards. While patients8EFFECTIVE TREATMENT FORBACK PAINAbout 80 percent of U.S. adults have struggled with back painat some time in their lives. The risk of back pain increases withage, and effective treatment can be very hard to find. Fortunately,patients who come to the Aria 3B Orthopaedic Institute can trustin the expertise of physicians who are seasoned experts in the artand science of diagnosing and treating back pain.Dr. Balderston specializes in treating disorders of the lower back,or lumbar spine, such as disc herniations, spinal stenosis, and adeformity of the vertebrae known as spondylolisthesis. “Patientsoften come to me because they're thinking of a surgical option,but very often we can offer a non-surgical or more conservativeoption that could be just as effective,” he says. This could includephysical therapy, pain management, or injections.And when a patient does need surgery, there are less-invasivetechniques available, many of which can be performed on anoutpatient basis. Dr. Maurer has developed a number of minimallyinvasive techniques for treating vertebral compression fractures,ruptured discs, spinal stenosis, and lower back problems. Oneprocedure for treating spinal stenosis, in which the bone beginsto impinge on a spinal nerve, causing pain and disability, involvesplacing a wedge in the area to enlarge the spinal opening. “Withina day of leaving the hospital after having the procedure, patientsare typically on their feet and returning to normal activities,”says Dr. Maurer. “We have also innovated and developed anoutpatient procedure for replacing the nucleus of a ruptured disc,and a minimally invasive technique for treating instability of thesacroiliac joint.”Jack W. Shilling, MD, has special interest in complex hip and kneereconstruction, including high-risk primary arthroplasty, revisionarthroplasty, periprosthetic fractures, and infection.Russell R. Bear, DO, FAAOS, specializes in disorders of the shoulder,elbow, and upper extremities, and treats patients from youngathletes with ligamentous injuries to older adults with fractures, aswell as those requiring joint replacement surgery.STAYING AHEAD OF THE CURVEWith special interest in hip and knee replacement, Dr. Shillingrecently came to Aria from Albert Einstein Healthcare Network,where he was Director of both the Joint Replacement Programand Orthopaedic Trauma.Philip M. Maurer, MD, is a nationally recognized expert with over 20years experience in treating symptoms related to cervical and lumbarherniated discs, painful degenerative discs, spinal stenosis, and vertebralcompression fractures. He has developed innovative techniques fortreatment of spinal disorders.“Aria had the vision to make this a world-class facility,” says Dr.Shilling. “They're really ahead of the curve, and I think that is wherewe want to be as patients, as physicians, and as Aria partners.”“My practice will focus predominantly on primary hip and kneereplacement, as well as complex cases involving those joints,”adds Dr. Shilling.With a strong background in trauma and general orthopaedics,Dr. Bear is fellowship trained in sports medicine. / Orthopaedics /Dr. Mayer and Dr. Waterman, both fellowship trained in sportsmedicine, specialize in evaluating and treating sports injuries.“The first person you would see is probably Dr. Mayer or me,”Dr. Waterman says. “We play the role of detectives in thatway. We try and figure out the diagnosis. A lot of what we dois seeing patients, taking a strong history and ordering anyconfirmatory tests.”Adds Dr. Mayer, “A lot of what happens in sports injury ultimatelyis not surgical, but it is often hard to tease out what is surgical andwhat is not surgical. In most cases it is a non-operative situationfrom the outset, so I am able to then diagnose and coordinatecare and design treatment regimens that are specific to thatindividual's condition."Jason J. Waterman, MD, specializes indiagnosing and treating athletic injuriesthrough non-operative measures.Dr. Mayer, herself a runner and tennis player, has a special interestin treating female athletes. “There are certain issues that arepertinent and unique to the female athlete, and often overlooked,”she explains. For example, when a young female athlete developsstress fractures, it is important to evaluate her nutritional andhormonal status, because both diet and hormones can affectbone health.Initial treatment can involve physical therapy, lifestyle and activitymodification, injections, medication, or braces, Dr. Watermanexplains. “We realize that most active patients with joint disordersdo not need surgery,” he adds. “Our goal is to keep patients asactive as they want to be. If someone does require surgery, wework closely with our surgical partners to streamline the process.”“I'm excited about the partnership with Aria, because as a groupwe have had long-term goals that I believe will come to fruitionand enable us to expand our sports medicine services in theregion,” Dr. Mayer says.Catharine Mayer, MD, is a sports medicinespecialist focused on the diagnosis and nonoperative management of sports and activityrelated injuries. Sagerman, MD, Section Chief of Ultrasound,is a member of Aria's Breast Evaluation Center Committee.Fast-Track ProgramFollowing an abnormal mammogram, a patient is often anxious,worried, and seeking answers regarding diagnosis and next stepsas quickly as possible. To address these needs, Aria developedits Fast-Track Program to expedite the process for schedulingadditional mammograms, an ultrasound, or both.Here's how it works: Following an abnormal mammogram result,the patient will be scheduled for a fast-track appointment as soonas possible, usually within the next few days. After undergoingadditional imaging studies, the patient can either schedule anappointment with the breast surgeon, or leave knowing thateverything is normal, says Sonja Cerra-Gilch, MD, Radiologist andSection Chief of Breast Imaging and Breast Intervention at Aria.“The Fast-Track Program is designed to provide comprehensiveevaluation and definitive answers quickly,” says Sun Yong (Sunny)Lee, MD, FACS, Breast Surgeon and Director of Aria’s BreastHealth Program. “For example, the program includes not only theDiagnostic Radiology Department, but also a patient navigator andthe early expertise of a surgeon who specializes in breast cancer.”“Our goal is to alleviate the anxiety that comes with having anabnormal mammogram,” says Rosemarie Rahill, the program’sPatient Navigator and Aria’s Women’s Health Coordinator.“We initially launched the Fast-Track Program at our TorresdaleCampus, and have since expanded to Aria’s Bucks County Campusand provide access to patients at our Frankford Campus as well.”Women undergoing screening mammograms or having additionaltesting can be confident that they are receiving the very best inmammography and ultrasound services at Aria.14In addition, Aria's pathologists make reading the results of breastbiopsies a priority. “Patients often have a lot of angst. They wantto know what their results are, and the surgeons want answers asquickly as possible as well in order to develop the best plan for thepatients,” says Behnaz Toorkey, MD, a Pathologist at Aria.Says Dr. Cerra-Gilch, “Every step of the way we've streamlined theprocess. It's a great program designed around our patients, whichhas been very well received by the community.”“We have a program through which answers are formulated assoon as possible,” says Dr. Lee. “We coordinate care so patientsdo not need to figure out which physician to call, or where to gofor answers.”Breast Evaluation CenterWomen who are diagnosed with breast cancer and seek treatmentat Aria can expect personalized, individualized, and carefullycoordinated care through the Breast Evaluation Center.The Center’s dedicated specialists include medical oncologistsAllen E. Lord Terzian, MD, Director, Division of Hematologyand Medical Oncology, Medical Director of the Aria HealthCancer Center; Julia Kennedy, DO; and Frederick G. Dold, MD;radiation oncologists Scot A. Dr. Lee also completed a Fellowship in Traumaand Surgical Critical Care at the University of Pennsylvania Medical Center.Dr. Lee received her training as a Fellow in Breast Surgery at Thomas Jefferson University Hospital. In addition, she practiced as abreast care specialist at Hartford Hospital in Hartford, Connecticut, where she was recognized as one of the area's Top Doctors inbreast care.George P.
