You are to ensure that your initials are placed besideany changes that you make. Ensure that you have answered each questiontruthfully. All of the questions cover your entire life. Being embarrassed toanswer the questions truthfully today is no excuse and will be consideredintentional falsification, which will result in your disqualification.
