A signed copy of the addendummust also be included with the bid submittal.This addendum shall become part of the contract documents from this point forward. This Addendumconsists of 47 pages and includes the following changes:Questions by BiddersQ1. Could you tell me what the engineer estimate is for this project?A.1 1,600,000.00.Q2. General Conditions, 00800.A.2In the General Conditions, 00800, please add the following Section 6.25:“6.25 CONTRACTOR shall not hire/employ any independent contractors during theterm of this Contract without the express written approval of the City.”Q3. Supplementary Conditions, 00900.A.3Replace this section in its entirety with the attached revised section 00900.Q4. Also there is no qualifying comments (that I could find) as to the containment of the EngineeredWood Fiber(playground surfacing). Run along south side of North Park Roadfrom, Colbert Lane up to the entrance to the park, cross the road at the entrance tothe park and bring up conduit along north side of the road and terminate in therestroom/picnic pavilion structure; exact location to be coordinated with the Cityprior to installation. Add item no. 86, IT Pull Boxes, 10 Each - 17” x 30” x 18”. Locate no more than 1,500’apart and at changes in direction as required per Palm Coast Fiber Optic Standards.Q6. Our playground equipment (Landscape Structures) is one of the specified in your Waterfront Parkproject out for bid. It specifies Playsense #404 with green plastics and tan poles. It is our belief thatthe designer made an error. That is not available in those colors. It would be Green poles and tanplastics. It should be noted that you would not want green slides in Waterfront Park it would literallyfry the younger users skin; tan would be much better.A.6 If Landscape Structures only offers their equipment in these colors, that is acceptable.However if the other manufactures can provide the colors that are listed, that is alsoacceptable and even preferred.Q7. After reviewing the specifications we can not find any specific time of completion or liquidateddamages spelled out can you please provide this information?CONTRACT TIME: Construction time to achieve Substantial Completion is two hundredand seventy five (275) consecutive calendar days from the date of the “Notice to Proceed,”with an additional thirty (30) consecutive calendar days to achieve Final Completion.A.7LIQUIDATED DAMAGES: 1,000.00 per calendar day.Q8. Bid item #41 on the Palm Coast Water Front Park bid calls for 34,037 SF of 4" x 10' wide sidewalk.However I can only come up with less than 14,805 SF. Please advise as to which quantity we shouldbid.A.8 This quantity also includes the 10’ sidewalk from Colbert Lane to the entrance to thePark, see sheets TR-1 – TR-4.Q9. Will the bid date be extended.A.9Change all references for a bid due date from July 30, 2009 to August 4, 2009.Q10. I have attached our proposal and site layout for BCI Burke playground equipment. Also is a “cut out”from the plans for your comparison. I request that this layout and proposal be considered equal for thepurpose of bidding the playground equipment on this job.A.10 This is an acceptable ‘or equal’.Q11. The specs do not list a preferred manufacturer for the electrical equipment. Q12. The specs and Sheet A3 show hand dryers in the bathrooms, but nothing is shown on the electricalfloor plan or panel schedule. Are they being installed? If so, what is the voltage and amperage to befigured for wiring them?A.12 New electrical sheet is provided, which shows hand dryers.Q13. I am assuming the grinder pump is part of a package unit and the electrical contractor is to only supplyto it. Is this correct?A.13 The grinder pump is typically provided by general contractor, The electrical contractorshould coordinated with the GC during the bidding phase.Q14. There is not a detail of how the dock lights are to be mounted. The alternate price fixtures aredesigned to be mounted to the dock or railing, but the Stonco fixtures on the schedule are onlywaterproof if installed facing downward. Are the Stonco fixtures to be mounted on a pole or someother means (such as underneath the top rail of the dock)?A.14 Provide FD Cast Box, attach to pier post.Q15. There is a line item for Site Lighting on the bid form but no site lighting show on the plans. Will asite lighting drawings be issued?A.15 There are two lights at the end of the dock, refer to sheet E-1Q16. Please clarify that the 2” conduit on the bid form is for the 2” AT&T conduit shown on the plans.A.16 Line item #26 on the bid form should read 2” AT&T Conduit, and consists of installationof the conduit, shown on the plans including all labor, equipment and materialnecessary for installationQ17. Please clarify when Value Engineering Incentive shall be submitted.A.17 Value Engineering Incentive items shall be submitted by the vendor who has beenawarded the contract after said contract has been executed by all parties.Q18. Does local bid preference apply to this project?A.18 Yes, refer to specification section 00010 Section 18.13 Item (n).Q19. Please clarify what the requirements are for removal of items associated with clearing of site.A.19 Burning of any debris is not allowed. Refer to Section 02110, Site Clearing, for properclearing of site.Q20. Should we have a line item for AT&T 2" raceway?A.20 Line item #26 on the bid form should read 2” AT&T Conduit, and consists of installationof the conduit, shown on the plans including all labor, equipment and materialnecessary for installationQ21. Should we have a line item for AT&T handholes?A.21 See answer A16.Q22. Should those items in question 20 & 21 be furnished and installed?A.22 Yes.Q23. For the Waterfront Project, can you confirm the following as being required coverage: Builders Risk;Owners and Contractors Protective Liability Insurance; Flood Insurance. PALM COAST WATERFRONT PARKSECTION 01275 - MEASUREMENT AND PAYMENTPART I1.011.02GENERALDESCRIPTIONA.The Contractor shall receive and accept the compensation provided in the Contract as fullpayment for furnishing all materials, labor, tools and equipment for performing all operationsnecessary to complete the work under the Contract, plus any miscellaneous items andservices that may not be specifically identified in the Contract Drawings and Specificationsbut that can be inferred from the Contract Drawings and Specifications and are necessary toproduce a completed Work that is usable in a manner for which it was intended. If any itemsfor a complete work are omitted or not shown, the Contractor shall furnish and install themwithout additional cost to the Owner, and also in full payment for all loss or damages arisingfrom the nature of the Work, or from any discrepancy between the actual quantities of Workand quantities herein estimated by the Landscape Architect, or from the action of theelements or from any unforeseen difficulties which may be encountered during theprosecution of the Work until the final acceptance by the Owner.B.The prices stated in the Bid Form include all costs and expenses for taxes, labor, equipment,materials, commissions, transportation charges and expenses, patent fees and royalties, laborfor handling materials during inspection, together with any and all other costs and expensesfor performing and completing the Work as shown on the Drawings and specified herein.The basis of payment for an item at the unit price shown in the proposal shall be inaccordance with the description of that item in this Section.C.The Contractor’s attention is again called to the fact that the quotations for the variousitems of Work are intended to establish a total price for completing the Work in itsentirety. Should the Contractor feel that the cost for any item of Work has not beenestablished by the Bid Form or Payment Items, he shall include the cost for that Workin some other applicable bid item, so that his proposal for the Project does reflect histotal price for completing the Work in its entirety.D.The Contractor shall prepare and submit an Application for Payment once per month.MEASUREMENT1.1.03The quantities for payment under this Contract shall be determined by actual measurement ofthe completed items, in place, ready for use and accepted by the Owner, in accordance withthe applicable method of measurement therefore contained herein. The Contractorshall submit invoices substantiating the cost of mobilization with each pay request.2. Measurement: This item shall be measured as lump sum.3. Basis of Payment: Payment will be made at the Contract lump sum price for the item.Bid Item No. 2 – General Conditions1. Description: This measurement and payment item includes all General Conditions, allInsurance requirement costs, the costs of all bonds and all administrative costs associatedwith acquiring and maintaining the necessary coverage as described in the ContractDocuments.2. Description: This payment item includes the cost of all permits and fees associated withcommencement of the project per the General Conditions of the Contract forConstruction.2. Basis of Payment: Payment will be made at the allowance price listed on the Bid Form.Bid Item No. 4 – Excavation/Disposal1. Description: This Measurement and Payment Item consists of supplying all labor,equipment and materials, including all fittings, required for the installation of thewater service piping, fire hydrant & valves, backflow preventers, tap into existinglines, and all other appurtenances to provide a functioning system to service waterfountains and the picnic pavilion building.2. Measurement: The unit of measurement for this item will be ‘linear foot’ for thepiping, and ‘each’ for the hydrant w/valves, water service, backflow preventers, gatevalves w/boxes, and connection to existing water main.3. Basis of Payment: Payment shall be made based on the total number of these unitsproperly installed for a fully functioning water service, tested and backfilled.Bid Item No. 20 - 23 – Sewer Service1. Description: This Measurement and Payment Item consists of supplying all labor,equipment and materials, including all fittings, pipe, connections, etc., required forthe installation of the pump station, force main, clean outs and all otherappurtenances to connect into the existing force main.2.
