CATALOG RIGHTS POLICYThe Institution must retain the flexibility to improve itscurriculum; therefore, course offerings may be changedduring a student’s education. If a course required undera previous catalog is no longer offered, a student eligibleto graduate according to that catalog should consult hisor her academic advisor or Dean to identify anothercourse that may be used to fulfill the requirement. Coursesubstitutions in the degree program are permitted onlywith the approval of the Dean through the departmenthead or program director. Furthermore, the Institutionreserves the right to make any changes in requirementsit may consider necessary and desirable by due notice inthe catalog. Substitutions for discontinued courses maybe authorized or required by campus administration.Information contained in this catalog is subject tochange at the discretion of the University without priornotification.In the event of any inconsistency between theinformation contained in this catalog and any othermaterial, the information contained in the catalog(including any addenda) shall take precedence. Smith as theIntermountain Business College. The college’s purposewas to teach commercial subjects and place graduatesin business positions. The college was known over thefollowing 68 years as Smithsonian Business College,Moench University of Business, and Ogden/WestHaven Business College. In 1959 the name was changedto Stevens-Henager College. The University has several computer laboratories,a medical lab, a surgical lab, a pharmacy lab, X-ray lab,three nurse labs, a library resource center, a conferencecenter, a student lounge, and lecture classrooms. WirelessInternet access is available throughout the campus. Thecampus has an elevator, ramps for handicapped students,and ample parking. Located at 1890 South 1350 West in5AccreditationIndependence University (IU) is accredited by theAccrediting Commission of Career Schools and Colleges(ACCSC), 2101 Wilson Boulevard, Suite 302, Arlington,VA 22201. ACCSC is listed as a nationally recognizedaccrediting agency by the United States Departmentof Education under the provisions of Public Law 82-550and subsequent legislation that requires the evaluationof such agencies and issuance of an official list by theDepartment.Independence University is approved by the UtahDepartment of Commerce, Division of ConsumerProtection, and is exempt from registration with theUtah State Board of Regents, as allowed in section 53B-5105-5.2e of the Utah Postsecondary Proprietary SchoolAct, revised February 17, 2011.The Surgical Technologist program at the groundcampus is accredited by the Commission onAccreditation of Allied Health Education Programs(CAAHEP) upon recommendation of the AccreditationReview Committee on Education in SurgicalTechnology (ARC-ST). ARC-ST is sponsored by theAmerican College of Surgeons and the Associationof Surgical Technologists. The Commission onAccreditation of Allied Health Education Programs:is located at 25400 US Highway 19 North, Suite 158,Clearwater, FL 33763, (727) 210-2350.In 2018, ACCSC placed CEHE’s colleges, includingIndependence University, on probation due to concernsover adherence to portions of ACCSC’s Standards.[During the period of probation, CEHE believes thatit has satisfactorily addressed the items listed in theACCSC probation - including implementing severalstrategies that have significantly improved currentcohort graduation rates to a level that is forecasted tomeet the ACCSC benchmarks for all programs.] OnApril 22, 2021 CEHE received notice that ACCSC votedto withdraw accreditation for some of CEHE’s colleges,including Independence University. CEHE filed a

Notice of Intent to Appeal with ACCSC. IndependenceUniversity’s accreditation remains in place during theentirety of the appeals process.Disaster Affecting theUniversity’s OperationsGovernmental AgenciesIn the event of an “Act of God” affecting operations(e.g., fire, flood, hurricane, tornado, etc.), the Institutionreserves the right to suspend training for a period not toexceed 90 days.Governmental AgenciesThe Rehabilitation Division of the State Departmentof Public Instruction in Utah recognizes IndependenceUniversity for vocational rehabilitation training by therespective state departments of education. Programsare approved for veterans’ benefits by the Utah StateApproving Agency. Individuals with temporarydisabilities (e.g., having a broken arm, recovering fromsurgery) do not qualify under the law as someone witha disability, and as such, are not entitled to reasonableaccommodations.Program ModificationsEligibility for ServicesThe Institution prepares its students for employmentin the technology, business, graphic arts, and medicalcommunities. To best meet the needs of these employers,periodic revision of our courses and majors is necessary.The Institution, therefore, reserves the right to add toor delete material from courses, alter program content,cancel a program if there is insufficient enrollment, andchange faculty, as circumstances indicate.In order to receive accommodations, students must meetthe following criteria:Calendar6Disability Services1. Provisional eligibility, which allows students to receivelimited services and accommodations, may be grantedin the following situations:3. The student provides documentation that isoutdated or incomplete and is in the process ofobtaining updated documentation, or4. The student does not have ready access to requireddocumentation, but provides some other form oflegitimate evidence of disability and the resultinglimitations and agrees to provide the requireddocumentation in a timely fashion.Documentation RequirementsStudents are required to provide the Institution withmedical or psychological documentation in order toreceive accommodations. All medical informationreceived by the Institution remains confidential andis released to other Institution personnel only with thestudent’s written permission. Specific documentationrequirements may vary slightly depending on thedisability. Generally, students should use the followingguidelines in determining what constitutes adequatedocumentation:3 Statement of disability, including diagnosis,instruments/tests used to reach that diagnosis,and current medications (if any) used to treat thecondition and their potential side effects, signedby a qualified medical/psychological professional;3 Statement of the current impact of the disabilityon academic performance;3 Recommendations for appropriate academicaccommodations.If the student’s documentation is not current (within thelast three to five years), or if the student has a learningdisability and the most recent testing was prior to age16, the student will be asked to seek more current testingand/or diagnostic information. UTAH CAMPUS ADMISSIONSUtah CampusAdmissionsRequirementsApplicants for admission must have graduated froman accredited high school, private secondary school, orhave completed the equivalent (GED, HiSET, CHSPE,or TASC). High school transcripts in a foreign languagemust be translated and evaluated by a credentialevaluation service for equivalency. Applicants mustpay tuition in advance if they do not qualify forfinancial assistance programs.Students who are selected for verification ofdocumentation for funding purposes regarding proofof high school graduation or equivalent (POG), maysubmit an unofficial proof of high school graduationwith a signed Transcript Request Form if they cannotprovide a copy of their high school diploma or GED.Once the student is able to access official POG theymust provide it to the Institution immediately.
