Refunds are only available when theOperations Centre received Your request for a refundbefore Your scheduled Departure Date.Warning: Trip Cancellation, Trip Interruption/Trip Delayand Flight Delay benefits for this Certificate of Insurancecontain a Pre-existing Conditions Exclusion. All other benefits,2are offered by Allianz to You under an individual policywhich number corresponds to the last 4 digits of Your BMOMastercard number (referred to herein as the “IndividualPolicies”). The insurance described in this Certificate ofInsurance is for eligible Mastercard Primary Cardholdersof BMO whose Accounts are in Good Standing and wherespecified, their Spouses, Dependent Children and/or certainother persons (referred to herein as “You” or “Your”). Thisinsurance is administered by Allianz Global Assistancethrough the Operations Centre. BMO receives compensationfrom the sale of this insurance. This Certificate of Insuranceis effective on the date BMO receives and approvesthe Primary Cardholder’s request that this insurance beincluded as a feature of their Mastercard.Only BMO may determine who is Primary Cardholder,whether an Account is in Good Standing and whether theinsurance pursuant to this Certificate of Insurance hascome into or is in force.No person is eligible for coverage under more than onecertificate of insurance providing insurance coveragesimilar to that provided hereunder. In the event that anyperson is recorded by Us as an “Insured Person” undermore than one such certificate, such person shall bedeemed to be insured only under the certificate, whichaffords that person the greatest amount of insurancecoverage. In no event will a corporation, partnership orbusiness entity be eligible for this insurance coverage.This Certificate of Insurance supersedes any certificatepreviously issued to You.In this Certificate of Insurance, certain terms havedefined meanings. Defined terms are capitalizedthroughout this document. Those Defined terms areas follows.1 DefinitionsAccidental Bodily Injury means bodily injury causeddirectly and independently of all other causes by externaland purely accidental means. The accident must occurduring the Coverage Period and the loss to which theinsurance applies must result within three hundred andsixty-five (365) days of the date of the bodily injury andmust not result from any of the exclusions.Account means the Primary Cardholder’s Mastercardaccount, which is in Good Standing.Actual Cash Value means We will pay the lesser of: the actual purchase price of a similar item;3

the actual cash value of the item at the time of loss,which includes deduction for depreciation (for itemswithout receipts, the insurance will pay up to 75% ofthe determined depreciated value); or the cost to repair or replace the item.Baggage means luggage and personal possessions,whether owned, borrowed or rented, and taken by Youon the Trip.Benefit Amount means the Loss amount applicableat the time the entire cost of the Passenger Fare(s) ischarged to Your Mastercard Account.Certificate of Insurance means a summary of thebenefits provided under the Policy issued to BMOcovering accident and sickness.Common Carrier means any land, air or waterconveyance for regular passenger service, which is fullylicensed to carry passengers for compensation or hireand which undertakes to carry all persons indifferently asto who may apply for passage, so long as there is roomand there is no legal excuse for refusal.Departure Date means the date on which You depart onYour Trip.Immediate Family Member means the InsuredPerson’s Spouse, child including adopted children andstepchildren, parent, sibling, legal guardian, parent-inlaw, grandparents, grandchildren, daughter-in-law, sonin-law, brother-in-law and sister-in-law.Injury means any bodily harm caused by an accidentwhich results in a covered loss and which requires theimmediate medical care or Treatment of a Physician.Insured Person means those persons covered for thebenefits described in this Certificate of Insurance asspecifically defined in each of the benefit sections.Loss means, with respect to a hand, complete severancethroughout or above the knuckle joints of at least four(4) fingers on the same hand; with respect to a foot,complete severance through or above the ankle joint.We will consider it a loss of hand or foot even if they arelater reattached.Mastercard means a Mastercard card issued by BMO andfor which BMO has received and approved the PrimaryCardholder’s request to include this insurance as afeature of the card.Medical Condition means any Sickness, Injury or symptom.Dependent Child(ren) means an unmarried natural,adopted or stepchild of a Primary Cardholder dependenton the Primary Cardholder for maintenance and supportwho is:Mysterious Disappearance means when the article ofpersonal property in question cannot be located, and thecircumstances of its disappearance cannot be explainedand do not lend themselves to a reasonable inferencethat a theft occurred. twenty (20) years of age and under; orOccupying means in, upon, entering into or alighting from. twenty-five (25) years of age and under and afull-time student attending a recognized college oruniversity; or twenty-one (21) years of age or older andpermanently mentally or physically challenged andincapable of self-support and became so while eligibleas a dependent child.Essential Items means necessary clothing and/ortoiletries purchased during the time period in whichchecked Baggage has been delayed.Family Cardholder means a Primary Cardholder’sSpouse and/or Dependent Child who have been issued asupplemental Mastercard on the Account by BMO.Good Standing means being in full compliance with allof the provisions of the Cardholder Agreement in forcebetween the Primary Cardholder and BMO, as amendedfrom time to time.4Operations Centre means the Operations Centermaintained by Allianz Global Assistance. From Canadaand the U.S. call 1-877-704.0341. From elsewhere callcollect 1-519-741-0782.Passenger Fare means a ticket for travel on a CommonCarrier which has been completely charged to the Account.Tickets obtained through the redemption of loyalty pointsearned under the Mastercard reward program are eligibleproviding that all applicable taxes and/or fees have beencharged to the Primary or Family Cardholder’s Accountor paid through the redemption of loyalty points earnedunder the Mastercard reward program.Personal Effects means property normally worn ordesigned to be carried on or by an Insured Person solelyfor private purposes and not used for business.Physician means a person, other than an Insured Personor member of the Insured Person’s family (by blood ormarriage), who is a medical practitioner and whose legaland professional standing within his or her jurisdiction is5

equivalent to that of a doctor of medicine (M.D.) licensedin Canada.Primary Cardholder means the cardholder who hassigned an application for a Mastercard, as primarycardholder, and for whom the Mastercard Accountis established and for whom BMO has received andapproved a request to include this insurance as a featureof the Mastercard.Ticket(s) obtained through the redemption of loyaltypoints earned under the Mastercard reward program areeligible for coverage.Travel Advisory means a formal written notice issuedby the Canadian government to advise travelers againstnon-essential travel to a foreign country or a givenregion in that country. This does not include travelinformation reports.Recurrence means the appearance of symptomscaused by or related to a Medical Condition, whichwas previously diagnosed by a Physician or for whichTreatment was previously received.Travel Companion is any person who travels with thePrimary Cardholder for the entire Trip and whose farefor transportation and/or accommodation was entirelyprepaid at the same time as the Primary Cardholder.Rental Car means a land motor vehicle with four wheels,that is designed for use mainly on public roads and whichYou have rented from a commercial rental agency for Yourpersonal use for the period of time shown on the RentalCar Agreement. Certain motor vehicles are not covered,please refer to Section 4.1.4.2. With regards to the CollisionDamage Waiver (CDW) Benefit described under Section4.1.1 a Rental Car may also include a commercial carsharing program of which You are a member.Treatment means medical advice, care and/or serviceprovided by a Physician. This includes, but is not limited to,diagnostic measures and prescribed drugs (including pillsand inhaled or injected medications). It does not includecheckups or cases where You have no specific symptoms.Rental Car Agreement means the entire writtencontract that You receive when renting a car from acommercial rental car agency that describes in full allof the terms and conditions of the rental, as well asthe responsibilities of all parties under the Rental CarAgreement. With regards to the Collision Damage Waiver(CDW) Benefits described under Section 4.1.1a Rental CarAgreement may also include a commercial car sharingprogram of which you are a member and the terms andconditions thereof.Sickness means any sudden illness or disease requiringthe immediate medical care or Treatment of a Physician.Spouse means the person who is legally married to thePrimary Cardholder; or if there is no such person, theperson who has been living with the Primary Cardholderin a conjugal relationship and who resides in the samehousehold as the Primary Cardholder and is publiclyrepresented as the spouse of the Primary Cardholder. Forthe purposes of this insurance the Primary Cardholdermay have only one (1) spouse.Terrorism means the unsanctioned and illegal use offorce that causes destruction of property, injury or deathby an individual or group for the express purpose ofachieving a political, ethnic or religious goal or result.Ticket means evidence of full fare paid for travel on aCommon Carrier, which has been partially or completelycharged to the Primary or Family Cardholder’s Account.6Trip means a defined period of travel of definitelength for which the full cost, or portion of the cost,of Your Ticket has been charged to the Primary orFamily Cardholder’s Account. Tickets obtained throughthe redemption of loyalty points earned under theMastercard reward program are eligible for coverage.We, Our, Us means Allianz Global Risks US InsuranceCompany (Canadian Branch).You or Your means the Insured Person.2 Insurance effective andtermination dateExcept as otherwise stated herein, this insurance shallcome into effect on the date BMO receives and approvesthe Primary Cardholder’s request to include the BMOTravel Protection with Trip Cancellation as a feature oftheir Mastercard.Except as otherwise stated herein, this insurance shallterminate on the earliest of:1.the date the Insured Person is no longer eligible toparticipate;2.the date the eligible Account is defined as ineligibleby BMO;3.00:01h of the date the Primary Cardholder cancelsthis coverage, or otherwise chooses to close theMastercard Account; or4.the date the Policy is terminated.7

3 EligibilityTo be eligible for this insurance You must be a resident ofCanada with a Mastercard Account in Good Standing.4 Coverage period and descriptionof benefits4.1 CAR RENTAL BENEFITSCoverage EligibilityThe Car Rental Benefits apply when You enter into anon-renewable Rental Car Agreement for a four-wheelpassenger vehicle, where the total rental period does notexceed forty-eight (48) days and is subject to limitationsand exclusions (as outlined in Section 4.1.4) and thefollowing requirements:1.the Rental Car must be rented by the PrimaryCardholder, or a Family Cardholder; and2.the Rental Car must be rented from a commercialcar rental agency; and3.the full cost, or portion of the rental cost, mustbe either charged to the Account or paid throughthe redemption of loyalty points earned under theMastercard reward program. An eligible Rental Carincluded in a pre-paid travel package is coveredif the full cost, or portion of the cost, of the travelpackage was charged to the Account or paidthrough the redemption of loyalty points earnedunder the Mastercard reward program; and4.You must not rent more than one vehicle at a timeduring a rental period; and5.You must decline the collision damage waiverbenefits (or similar provisions, such as “loss damagewaiver”) offered by the rental agency (when notprohibited by law). If there is no space on the RentalCar Agreement to decline coverage, You must writeon the contract “I decline the CDW provided by theRental Agency”. If such coverage is not availablefrom the rental agency, then CDW benefits are notavailable under this Certificate of Insurance; and6.8the Rental Car must have been operated by thePrimary Cardholder, a Family Cardholder, the PrimaryCardholder’s Spouse or Dependent Child listed onthe Rental Car Agreement and authorized to operatethe Rental Car under the Rental Car Agreement inaccordance with its conditions, when the loss occurs.Coverage PeriodFor a Rental Car to be eligible for coverage, this insurancemust be a feature of the Mastercard prior to enteringinto the Rental Car Agreement or prior to the InsuredPerson taking control of the Rental Car. Insurancecoverage begins as soon as the Primary Cardholder, theFamily Cardholder, the Primary Cardholder’s Spouse orDependent Child who is authorized to operate the RentalCar under the Rental Car Agreement takes control ofthe Rental Car. The total rental period must not exceedforty-eight (48) consecutive days. In order to break theconsecutive day cycle, a full calendar day must existbetween rental periods. If the rental period exceedsforty-eight(48) consecutive days, coverage under thisCertificate of Insurance will be void.Insurance coverage ends at the earliest of:1.the time when the rental agency assumes control ofthe Rental Car, whether it be at its place of businessor elsewhere. Rental keys left in a locked dropbox does not constitute that the rental agency hasassumed control of the Rental Car;2.the end of the chosen rental period; or3.the date on which the Primary Cardholder’scoverage is terminated in accordance with the“Insurance Effective and Termination Date”provision set out above.4.1.1 COLLISION DAMAGE WAIVER (CDW) BENEFITSInsured Person means the Primary Cardholder, a FamilyCardholder, Primary Cardholder’s Spouse or DependentChild(ren).Coverage BenefitsSubject to the terms and conditions, You are covered forRental Cars with a Manufacturer’s Suggested Retail Price(MSRP), in its model year, up to a maximum of 65,000 for:1.damage to the Rental Car; and2.theft of the Rental Car or any of its respective partsor accessories; and3.rental agency charges for valid loss-of-use whilethe Rental Car is being repaired; and4.reasonable and customary charges for towing theRental Car to the nearest available facility.This coverage does not provide any form of third partyautomobile, property damage or per
