Such provider must have an active unencumbered licensefor a health care profession which isissued by another state, the District of Columbia, or a possession orterritory of the United States and have never been subject todiscipline by a licensing agency in any state or federal jurisdiction andnever had their license or permit for controlled substancessuspended or revoked. Such providers must register with the relevantstate agency and only offer clinically appropriate, medically necessaryservices.
