GENERAL INFORMATIONThe UFT Welfare Fund Participating Dentist Program isdesigned to provide you with comprehensive dental serviceswhile reducing or eliminating your out-of-pocket expenses.If you or a member of your family who is covered for dentalbenefits wish to use a Participating Dentist, select one fromthis Director of Participating Dentists and call for anappointment. Updated listings are also available atwww.uftdental.com. Be sure to identify yourself as aneligible member of the United Federation of TeachersWelfare Fund. Be aware that although several dentistsmay practice at the same location, only the dentist whosename appears in this Director is a UFT Participating Dentist.When you obtain your services from a Participating Dentistit does not in any way change the nature of your dental plan.Plan allowances are the same for services provided by aparticipating or non-participating dentist. However, if youuse a Participating Dentist, the Fund will pay your dentistdirectly. If you use a non-participating dentist, the Fund willpay up to the maximum allowance set forth in the dentalschedule and you will be responsible for the differencebetween that allowance and your dentist’s charge.This program was developed in cooperation with SIDSConsultants, who will monitor the performance of participatingproviders to ensure that appointments are freely given andhonored and that charges for services do not exceedMaximum Charges listed in the UFT Welfare Fund DentalSchedule. You will not be required to pay the dentist anymoney except in the following few instances.For a covered, but non-reimbursable service.There are services listed in the dental schedulewhich, at times, are not payable by the plan. Forexample: restorations for cosmetic improvement;services for which the Alternate Benefit Provision isapplied; where frequency limitations and/or planmaximums have been met. In these instances theParticipating Dentist may not charge more thanthe Dental Schedule would have paid for thoseservices, plus any applicable co-payments.For a specified co-payment.You are responsible to pay the dentist thepatient co-payment amount for services listedin red in the Dental Schedule for certain highcost services (crowns, bridges, dentures, roottherapy, orthodontic treatment, intravenousanesthesia and treatment appliances).

For a non-covered service.The Participating Dentist may not charge you morethan his/her usual and customary charge forprocedures that are not covered by the Dental Plan.Dental Implants: The surgical placement of an implant is nota covered expense. However, participating dentists haveagreed to limit their charge for placement of an implant to 1,200. Refer to the UFT Welfare Fund Dental Schedule Item14; UFT DISCOUNT ON NON-COVERED SERVICE(LISTED IN GREEN). Although the restoration orprosthesis placed on an implant is not covered, the planwill provide a Special Reimbursement to offset part of thecost for that service. Refer to the Dental Schedule Item15; SPECIAL REIMBURSEMENT (LISTED IN BLUE).Your dental plan applies certain payment limitations basedon frequency. For example, the plan will not pay for morethan one oral examination in a six-month period, or more thanone prophylaxis in a three-month period. Since it is notpossible to determine in advance whether benefits will beavailable for these services, the dentist is entitled to requestpayment from you pending submission and adjudication ofclaim. If benefits are available, that money will be refunded.If you are eligible for dental benefits under another groupdental plan in addition to the UFT Welfare Fund Dental Plan,the dentist is entitled to the benefits available from bothplans. The combined payment for any procedure may notexceed the dentist’s usual and customary fee, and paymentfrom the second plan will be applied first to reduce oreliminate any co-payments.It is important to understand that neither SIDS nor the UFTWelfare Fund recommends any particular dentist. You areresponsible to select the dentist of your choice, and shouldexercise the same care and apply the same criteria inselecting a Participating Dentist that you would in selecting anon-participating dentist.Maintaining high standards of care and patient satisfactionis fundamental to the success of this program.
