Letter from the Chief Nurse ExecutiveLeading Through ChangeIt is a great privilege to share Inova Health System’s 2019 Nursing Annual Report,which highlights innovations and advancements in nursing practice, research andoutcomes across our health system.2019 was a year of unprecedented success for Inova Nursing. We built on ourstrong foundation of professional practice to achieve even greater levels of patientcare excellence. Inova was the only large health system in the nation with everyhospital rated five stars by the Centers for Medicare & Medicaid Services, andall five hospitals also earned a Leapfrog Hospital Safety Grade of A—the highestpossible ranking in the D.C. metro area and nationwide. None of which would be possible without thededication and commitment of our highly specialized nursing team. Nurses are key partners in Inova’s newtriad leadership structure, working with physicians and administrators to advance our mission and create aseamless, high-reliability system of care.And now, amid a global pandemic—unlike anything we’ve ever experienced before—I am more confidentthan ever of our ability to thrive regardless of the challenges we face. Our nurses are innovative problemsolvers who remain compassionate care partners for all patients who walk through our doors. They exemplifyour mission to provide world-class healthcare—every time, every touch—to each person we have theprivilege to serve. I have even greater clarity on what that privilege means during such challenging times.I know that the future is bright for nursing at Inova as we continue to learn, grow, excel, lead and innovate.Our journey is just beginning and I’m excited to see where it takes us. Inova Alexandria HospitalMagnet ComponentsTransformational LeadershipPromoting a culture that embraces transformationalleadership begins with a transformational chief nursingofficer who creates a safe environment for her team.Inova Alexandria Hospital’s chief nursing officer (CNO),Fadia Feghali, MS, BSN, RN, NE-BC communicatesa clear vision to her team that focuses on prioritizingand rewarding staff development as well as patientoutcomes. Results from the National Database ofNursing Quality Indicators (NDNQI) nurse satisfactionsurvey in 2019 demonstrated scores well beyond themean for the questions pertaining to CNO visibilityand accessibility, as well as equivalent power andauthority to other top-level hospital executives. Feghaliis the biggest advocate for nursing at Inova AlexandriaHospital (IAH). She promotes nursing evidence-basedpractice (EBP) and has supported numerous EBP projectsamong her other contributions to nursing practice.10I N O V A I NURSING REPORT 2019Inova Alexandria’s CNO, senior directors and clinicaldirectors are advocates for their own teams, many ofwhom are active participants in career coaching, mentoringand succession planning. Change is a constant within thehealthcare industry. Inova Alexandria Hospital’s nursingleadership exemplifies leading with a clear mission, vision,values and strategic plan that aligns with the organization’spriorities.Structural EmpowermentInova Alexandria Hospital is continually evolving instructural empowerment. Along with all Inova hospitals,Inova Alexandria Hospital participated in a Nursing SharedGovernance study to evaluate our internal degree of sharedgovernance. Results from this study will help us improveand strengthen our Shared Governance decision-makingstructures and processes. In 2019, the Strategic NursingInitiative Group, commonly known as the “SING” group,restructured the Shared Governance meeting design.The most notable change to the hospital’s nursing

As part of the monthlymeeting, individual units meet in their respective clinicalpractice council (CPC) groups followed by a hospital-wideShared Governance meeting that includes the chairs andco-chairs of all CPCs. Within the hospital-wide SharedGovernance meeting, breakout sessions focus on variousareas of nursing interest.Supporting nurses’ professional development is animportant aspect of structural empowerment. In InovaAlexandria Hospital’s 2019 NDNQI survey, “professionaldevelopment opportunity” and “professional developmentaccess” were two of the highest performing measuresrelative to the national NDNQI mean. A dedicated androbust nursing professional development team, bothwithin Inova Alexandria Hospital and throughout Inova,contribute to the development opportunities nurses at InovaAlexandria Hospital value.Inova provides a nursing residency program supportedby Vizient. In 2019, 50 new nurses completed the residency.Exemplary Professional PracticeInova Alexandria Hospital is proud to have a culture thatpromotes safety, high-quality care and continual qualityimprovement. We achieve this through collaboration withall disciplines, both within the hospital walls and beyond.Nurses analyze data submitted to externally manageddatabases including NDNQI, Press Ganey, Vermont Oxfordand others in order to benchmark outcomes and makesustainable improvements. One example of a nursing bestpractice that Inova Alexandria Hospital has embraced is thePatient and Family Advisory Council (PFAC). Partnershipswith patients and families resulting from work within thePFAC have driven efforts to focus on patient-centered careand have promoted interprofessional collaborative practice.An example that highlights how nurses participatewith an interprofessional team to achieve exemplaryprofessional practice is a recent effort in the operatingroom to demonstrate a safe and thorough time out priorto a procedure. adhere to a proper and regimentedtime out. She worked with theperioperative clinical mentor, physicianand nursing leadership, and mediaservices to create and disseminatea best practice video. Empoweringnurses across all levels to act on bestpractice recommendations is a centraltenet of nursing practice at InovaAlexandria Hospital.New Knowledge andInnovationInova Alexandria Hospital supportsnursing research and EBPimplementation in numerous ways.Each month, the Inova AlexandriaHospital nursing team facilitatesa research and EBP meeting. Allnurses are welcome to attendthese meetings, and those seekingpublication or presentation of researchor EBP are required to attend. TheVizient nurse residency program isanother venue for nurses to practicetheir research and EBP skills. As partof the nurse residency program, allnew graduate nurses work on anEBP project within a small group toguide best practice within the hospital.Nurse resident EBP projects are notprescribed to the residents. Rather,they are guided by the resident’sown interest and identified needs.Resident EBP projects at IAH haveincluded technology implementationto enhance patient experience and12I N O V A I NURSING REPORT 2019nursing practice and also focused onworkflow improvements to improveefficiencies in care. Nurse residentperspectives are particularly helpful inidentifying improvement opportunities,because new staff bring an objectiveperspective.and supplement active learning. Thisis an example of nursing innovation inaction at Inova Alexandria Hospital.For example, after reviewing theAugust 2019 Hospital ConsumerAssessment of Healthcare Providersand Systems (HCAHPS) scoresfor a medical-surgical unit at InovaAlexandria Hospital, nurses identifiedan opportunity to improve theway staff explains to patients whatmedications are used for and theirpossible side effects. A group ofnursing residents sought to improveHCAHPS scores by using medicationeducation stickers applied to thereusable patient water bottles. Afterpartnering with the hospital pharmacy,medication stickers for the mostcommonly prescribed medicationswere created and disseminatedto the unit. To supplement routinemedication education provided bythe nursing staff, stickers were placedon patients’ drink containers. Thischange resulted in a remarkableimprovement in HCAHPS scores.Scores improved on the two questionsrelated to medication education 23rdand 4th percentile to the 97th and45th percentile, respectively, in onlyone month.
