Planning your supportWe will work with you to planyour support.This is what we will do withyour help:We will listen to your goals andwhat you want to be able todo in the future.4

We will agree a plan of thingsyou will do.We will help you do the thingsyou have planned.We will find other people whocan also help you.5

We will meet with you regularlyto make sure your plan isworking for you.We will keep notes of howwell your plan is working and ifanything in the plan needs tobe changed.We will look over your supportwith you every six months, orsooner if anything changes orif you ask us to. We will alsouse this time to make sure wecan still give you the supportyou need.6

The sort of support you getAbility staff will support youwith things like:Moving in to your home.Organising, cleaning, cookingand shopping.Managing your money.7

Getting to know the area youlive in.Keeping safe.Contacting doctors or otherlocal services.Finding training or work.8

Choosing how you spend yourtime.Personal care.Other things you may needsupport in.9

What Ability must do:Ability Care & Support mustmake sure that informationabout you is kept private.We will ask you first if we cangive information about you toanyone else.Very rarely we might have totell someone information aboutyou. We will always tell youwhen we do this.This may be because there isa risk of serious harm to you orsomeone else, or if the policemight need to get involved.10

We must give you informationabout how we do our work.We must let you seeinformation about you we haveon file.We must make sure that youhave independent advice froman advocacy service if youwant it.11

We must support you to givefeedback about our service.We must talk to you aboutchanges to your help andsupport.We must complete a RiskAssessment to make sure wecan work safely in your home.12

Things you must do:You must provide any specialequipment we will need tosupport you, like a hoist.You must tell us about anychanges to the money you getthat might affect your benefitsor how you pay for care.You must accept supportfrom the service and tell usif your support needs to bedifferent.If you cannot meet staff at thetime you arranged you musttell them at least 24 hoursbefore.13

You must tell us if you don’twant any of the support wecan give you anymore.You must provide your ownstamps and phone.You must pay for things likeparking, mileage and activities.14

What to do if you want to end the agreementIf you don’t want support fromAbility anymore you can endthe agreement.We would like you to talk to usabout this first. You can talk toyour support worker or to themanager.Ability Care & Support mayend the support agreementtoo.15

These are the reasons wemight do thisIf you don’t need supportanymore.If you do not want to work withany of the staff.If you are abusive to staff or toother people.16

If we do this, we will explainthe reasons to you and talk toyou about other options youcan take.You have the right to appealagainst this. Your supportworker can explainhow to do this.17
