Kindergarten Physical Requirements–All children must have a current physical to enter Kindergarten–Requested to be turned in by August 1, 2022–No medical appointments accepted–Students that do not have current physicals will be not be able to startschoolThings to remember:–Physical signed (by Doctor, PA, APN) and dated 8/18/21 or after–Health history completed and signed by parent–Lead and diabetic screenings completed

Immunization Requirements–DPT/Td Booster on or after 4th birthday (4 doses)–Polio Booster on or after 4th birthday (4 doses)–MMR/Measles Booster -1st immunization on or afterage 1 and a booster at least one month (28 days) later–2 Varicella Vaccines (Chickenpox) on or after age 1 (atleast one month or 28 days apart)

Vision Requirements–Effective January 1, 2008: State Law SB641, provides thata vision exam is needed for all incoming Kindergarten students–Proof of the required eye examination by a licensedoptometrist must be submitted by Oct. 15th of each schoolyear–Exam must be dated August 18, 2021 or after–Form is in your registration packet

Dental Requirements–All children in Kindergarten will be required to have a dentalhealth exam performed by a licensed dentist–Must be on or after November 15, 2021–The state deadline is May 15, 2023–We request that they be turned in by May 1st

Medication Policies–Care plans need to be filled out by parents, doctor, and CSN for anystudents with diabetes, seizures, or other chronic health conditions–All medications (prescription & over the counter) must have districtmedication authorization form (MAF) signed by parents and doctor-If your child has a life threatening allergy or diagnosed with asthmaplease speak with the building nurse regarding allergy and asthmaactions plans that must be completed by the doctor.–Forms are in the registration packets as well as in the school office-Building nurse email mscardul@psd202.org

Student ComputersAll District 202 students are provided with a laptop.-Students bring to and from school each day.Have a plan as to where to store and charge the computer eachevening.Work with students to care for their laptop.
