Foster strong links with colleagues working in external rehabilitative, wellbeing andpalliative care services.Key Responsibilities:Clinical Practice & LeadershipProvide a high standard of holistic and individualised patient care to patients with palliative careneeds. As the Lead for the Rehabilitation service, the post holder will be a key provider ofevidenced based clinical expertise and leadership and will: Be a champion for the Hospice’s Rehabilitation service while working in partnership withother organisations, establishing and developing working relationships with thoseinvolved in delivering rehabilitation services in all settings Provide line management to the Rehabilitation Team, supervising their work, ensuringcompetence and providing 1-1 support and development including annual objectivesetting and performance appraisal Work collaboratively as part of the multidisciplinary palliative care team with Hospicecolleagues and others involved in patient care Assess, plan, implement and evaluate care plans for patients with a view to empowering,maintaining and/or improving their present level of function and independence Act as a role model for others, exhibiting high standards of professional behaviour andcompetence and providing leadership, support, advice and education to colleagues Be a reflective practitioner, applying evidence based practice and critical thinking to allpractices and processes of careET 04/22 Draft 21

Provide support to and assist the Modern Matron (MM) and Director of Care and ServiceTransformation (DCST), with the development of the service.Ensure that appropriate data is collected, audited and shared to support servicemonitoring, development and quality assuranceMaintain effective relationships with external professionals and relevant organisationsliaising about patient care issues as appropriateMaintain an ethos of palliative rehabilitation and wellbeing which incorporatesphysiological, social and spiritual components in accordance with Hospice philosophy.Delegate appropriate tasks and provide guidance in the management of these tasksTake an active role in the recruitment of rehabilitation and wellbeing staffUndertake home assessments, supporting colleagues to do this where necessary.Liaising with the multi-disciplinary team, making recommendations about support andtreatmentOversee the triage and allocation process of referrals for rehabilitation and wellbeingSupport the rehabilitation and wellbeing team to set goals and to discharge patients,identifying other services that can best support the patient following completion of theirsupport by the HospiceIn collaboration with other Hospice services, review patients who require specialisttherapy input, referring on for generic advice and support where necessaryParticipate in developing guidelines and protocols to deliver a high qualitycomprehensive, evidence-based and patient-focused service for patients and familiesAssist patients in adjusting to their physical losses as their condition deterioratesAssist patients and their carers to ensure that their home environment is appropriate forthe patient’s current and future needsParticipate in locality and network forums as required, disseminating and sharing goodpractice and ensuring the service develops.Attend and actively participate in clinical and multidisciplinary meetings, nominating adeputy when unable to attendMaintain clear, concurrent and concise electronic patient records which comply with therelevant hospice policy and professional standards of record-keepingAct within any legal and statutory rules relating to occupational therapy or physiotherapypractices in the field of palliative care as set by the appropriate registering bodyWork within a budget to identify any equipment that may need to be purchasedInvestigate incidents, concerns and complaints when required by the HospiceActively contribute to the development and delivery of the Hospice strategic plan byworking with the Rehabilitation and Wellbeing Team, the Modern Matron and/or theDCSTCommunication Create a working environment within the team that is flexible and adaptable and whichfosters an approach in line with the Hospice’s Values, Mission and Vision Act as a link between the Hospice and partner rehabilitation teams and local equipmentservices, fostering positive working relations among all members of Hospital, Hospice,Social Care and Community Teams. Challenge poor practice wherever it is encountered Initiate and participate in clinical reviews of patient and family care as appropriate Within the multidisciplinary team, working to ensure that patient dignity is respected at alltimes both before and after death, including acting as the patient’s advocate Provide mentoring and coaching to staff and volunteers where required and supervisestudents on placement Be responsible for ensuring the maintenance, cleaning and safekeeping of rehabequipment.
